Malacoplakia of the bladder combined with infected renal calculi: A case report.

INTRODUCTION: The malacoplakia of the bladder is a rare chronic acquired infection- associated granulomatous disease and even less common in combination with urinary stones. CASE PRESENTATION: We report the case of a 58-year-old female patient with malacoplakia of the bladder combined with renal calculi. The patient was admitted to the hospital with bilateral low back pain for one month and space-occupying lesions of the bladder for three days. Preoperative imaging suggested space-occupying lesions of the bladder: high probability of bladder cancer. Following the anti-infection treatment, the

Clinical aspects of antimicrobial prophylaxis for invasive urological procedures.

The essential value of antimicrobial prophylaxis is to defend the patient undergoing invasive diagnostic procedures or surgery against infectious complications by reducing the bacterial load. Escherichia coli remains the predominant uropathogen (70-80%) isolated in acute community-acquired uncomplicated infections, followed by Staphylococcus saprophyticus (10 to 15%). Klebsiella, Enterobacter, Proteus species, and enterococci infrequently cause uncomplicated cystitis and pyelonephritis. The pathogens traditionally associated with UTI are altering many of their features, particularly because of

Total endoscopic management of the encrusted ureteral stent under a single anaesthesia.

OBJECTIVE: Stented patients lost to follow up may return with large stone burdens encrusting the stent. The study describes total endoscopic management under one anaesthesia to remove such stents. METHODS: A similar approach was done in all cases. Cystoscopic Holmium: YAG (Ho) or electrohydraulic lithotripsy (EHL) removed the bladder calculus. The bladder portion of stent was cut with endoscopic scissors and removed. The patient was repositioned prone for percutaneous access and nephrolithotomy (PCNY), using either ultrasound, EHL, or Ho. The remaining cephalic portion of stent was removed per

Combining Retroperitoneal Laparoscopic Ureterolithotomy and Flexible Ureteroscopy for Effective Removal of a Large Ureteral Stone: A Case Report.

BACKGROUND Large ureteral stones (LUSs) pose significant health risks, including chronic obstruction and renal impairment. Timely intervention is critical to preserve kidney function. While the optimal treatment for LUS is debated, retroperitoneal laparoscopic ureterolithotomy (RLU) is often preferred over percutaneous nephrolithotomy (PCNL), ureteroscopy, or extracorporeal shockwave lithotripsy due to its effectiveness. Combining RLU with antegrade flexible ureteroscopy (aFURS) can enhance stone clearance rates, addressing the challenges of intrarenal access and guidewire passage. CASE REPORT

Beyond Candida: A clinical insight into rare fungal pyelonephritis in urological practice.

INTRODUCTION: Non-Candida fungal infections of the urinary tract, such as Aspergillus and Mucor, though uncommon, pose significant diagnostic and therapeutic challenges. Their variable presentation, antifungal resistance, and frequent association with immunocompromised states or prior instrumentation necessitate a high index of suspicion. This study aims to describe the clinical profile, diagnostic approach, and management outcomes of non-Candida fungal pyelonephritis in urological practice. MATERIAL AND METHODS: A retrospective observational analysis was conducted on four patients diagnosed

Surgical management of urolithiasis in france from 2016 to 2022: epidemiological data from the national PMSI database.

PURPOSE: The incidence of urolithiasis is rising in France, with a prevalence approaching 10% in the general population. Over the past decades, minimally invasive endourological techniques have profoundly transformed surgical management. Although current guidelines recommend extracorporeal shock wave lithotripsy (ESWL), ureteroscopy (URS), and percutaneous nephrolithotomy (PCNL) based on stone and patient characteristics, little is known about their nationwide use. This study aimed to describe recent trends in the surgical management of upper urinary tract stones in France. METHODS: Data from

Conventional ultrasonography enabled with augmented reality needle guidance for percutaneous kidney access: an innovative methodologies randomized controlled trial.

IMPORTANCE: Successful needle puncture of the renal collecting system is a critical but difficult procedure in percutaneous nephrolithotomy (PCNL). Although fluoroscopy and ultrasound are the standard imaging techniques to guide puncture during PCNL, both have known limitations. OBJECTIVE: To assess the feasibility and safety of a new navigation system for needle puncture in ultrasound-guided PCNL. DESIGN: This study employed a single-center randomized controlled trial (RCT) design to assess the feasibility and safety of a new navigation system for needle puncture in ultrasound-guided PCNL.

The impact of antegrade guidewire positioning outside the external urethral meatus in supine PCNL: the through and through technique.

This study aims to evaluate the impact of through-and-through (T-T) guidewire positioning versus conventional guidewire positioning in percutaneous nephrolithotomy (PCNL) with endoscopic combined intrarenal surgery (ECIRS) on complication rates (CR), surgical outcomes, and predictors of post-operative complications. We analyzed data from patients who underwent PCNL in our institution. Patients were divided into the T-T group (guidewire positioned through the urethral meatus) and the No-T-T group (guidewire positioned within the renal pelvis or down the ureter). Pre-, peri-, and post-operative

Access to the upper calyx in supine position: breaking a myth.

The feasibility and safety of upper calyx access in supine percutaneous nephrolithotomy (PCNL) remains controversial. We aimed to detail our technique and describe the outcomes in effectivity as well as safety in a large cohort. The authors analyzed patients with renal and proximal ureteral stones who underwent supine PCNL using upper calyx access at Rio Hortega University Hospital Valladolid (Spain) between 2013 and 2023. Patients were placed in the Galdakao-modified supine Valdivia position. Transpapillary percutaneous access to the upper calyx was guided by a combination of ultrasound (US)

Urolithiasis in patients with cancer.

Urolithiasis is increasingly common, with rising rates driven by obesity, diabetes and metabolic syndrome. Patients with cancer have additional, unique risks of stone formation owing to effects on fluid and electrolyte balance, systemic cancer therapies, tumour lysis syndrome and anatomical alterations after urinary diversion or nephrectomy. Moreover, urolithiasis itself has been linked to increased rates of renal cell carcinoma, urothelial carcinoma and bladder cancer, potentially mediated by chronic inflammation, recurrent infections and shared metabolic or environmental factors. Management

Retrograde intrarenal surgery with intelligent control of renal pelvic pressure for staghorn calculi: a case report.

Percutaneous nephrolithotomy is the gold standard treatment for staghorn calculi. However, this study reviews a case of an almost complete removal of staghorn calculi following one session of retrograde intrarenal surgery with intelligent control of renal pelvic pressure (RIRS-ICP). A 45 years-old female patient with an 8.3 × 4.5 cm complete staghorn stone was infected with Proteus mirabilis. Two sensitive antibiotics, piperacillin tazobactam and etimicin, were administered for 3 days. Semirigid 7/8.4 Fr ureteroscope was used to treat the renal pelvis and upper calyceal calculi for 57 min. A 5

Urine microscopy revealing a metabolic disorder: a case report.

BACKGROUND: Primary hyperoxaluria type 3 is a rare autosomal recessive disorder caused by HOGA1 mutations, leading to recurrent calcium oxalate nephrolithiasis. Diagnosis is often delayed, and 24-hour urine oxalate measurements can be unreliable in patients with normal kidney function. Urine microscopy can detect high-density calcium oxalate monohydrate crystals, providing a rapid diagnostic clue. CASE PRESENTATION: A 31-year-old woman with recurrent nephrolithiasis since age 9 had undergone prior percutaneous nephrolithotomy and ureteroscopy. Urine microscopy revealed abundant calcium oxalate

Successful Laparoscopic Ureterolithotomy to Remove a Large Ureteral Stone in an Infant: A Case Report.

Pediatric urolithiasis, while rare, can result in significant morbidity; it typically presents with pain, irritability, and complications such as hydronephrosis and urinary tract infections. Conventional management techniques include extracorporeal shock wave lithotripsy (ESWL), ureteroscopy (URS), and percutaneous nephrolithotomy (PCNL), with the choice guided by stone size, location, and patient age. In select cases - particularly those involving large or impacted stones where traditional methods may be inadequate - laparoscopic ureterolithotomy can offer a safe and effective alternative. We

An uncommon cause of acute kidney injury in young children: cystinuria.

UNLABELLED: Bilateral obstructive nephrolithiasis is a rare cause of acute kidney injury (AKI) in early childhood. As soon as the identification of AKI secondary to ureteral stone is made, it will necessitate an emergency treatment. PATIENTS: We report three infants with AKI caused by bilateral obstructive ureteral cystine stones. They were diagnosed with acute post-renal injury due to obstructive bilateral ureteral stones based on ultrasound scan findings. Immediately, bilateral ureteral stents were inserted for urinary drainage. Once renal function recovered to normal, each patient underwen
