Absent otoacoustic emissions predict otitis media in young Aboriginal children: a birth cohort study in Aboriginal and non-Aboriginal children in an arid zone of Western Australia.

BACKGROUND: Otitis media (OM) is the most common paediatric illness for which antibiotics are prescribed. In Australian Aboriginal children OM is frequently asymptomatic and starts at a younger age, is more common and more likely to result in hearing loss than in non-Aboriginal children. Absent transient evoked otoacoustic emissions (TEOAEs) may predict subsequent risk of OM. METHODS: 100 Aboriginal and 180 non-Aboriginal children in a semi-arid zone of Western Australia were followed regularly from birth to age 2 years. Tympanometry was conducted at routine field follow-up from age 3 months.

Chondrosarcoma of the temporal bone.

Chondrosarcoma of the temporal bone is a rare disease, with only 36 cases of this tumor having been reported in the English language literature. We report the case of a 52-year-old woman with myxoid chondrosarcoma of the temporal bone whose only symptom was progressive hearing loss. The tumor was removed using aspiration, mastoidectomy and tympanotomy, with retention of the jugular bulb. No recurrence has been noted 45 months after the operation. Diagnosis was established from histological examination of the surgical specimens. The clinical characteristics and management of this rare lesion ar

A case of an inflammatory myofibroblastic tumor of the mastoid presenting with chronic suppurative otitis media.

Inflammatory myofibroblastic tumors, also known as inflammatory pseudotumors, include a diverse group of lesions characterized by inflammatory cell infiltration and variable fibrotic responses. Their occurrence in the temporal bone is unusual. We present a case of an inflammatory myofibroblastic tumor of the mastoid bone in a 55-year-old female who complained of repeated otorrhea and a hearing impairment. On an otoscopic exam, her tympanic membrane was perforated. The computed tomographic imaging revealed a soft-tissue density-filled middle ear and mastoid with sclerotic bony change. The initi

Membranous band between the tympanic membrane and the external auditory canal.

Two cases of a membranous band between the tympanic membrane and the external auditory canal are presented. These characteristic structures are rare, but observed in several first branchial cleft anomalies. Neither patient presented with an infection. In Case 1, an eleven-year-old girl has this structure in her right ear. In Case 2, a thirteen-year-old boy has this structure in his left ear. Both patients demonstrated slight air-bone gaps in the affected ear with a pure-tone audiometric test, due to limited vibrations of the tympanic membrane.

Relationship between tympanic membrane retraction and habitual sniffing in patients with cholesteatoma.

CONCLUSION: Habitual sniffing affects the pathogenicity and recurrence of cholesteatoma. Postoperative instructions requesting patients to cease sniffing may reduce the retraction and recurrence of cholesteatoma. OBJECTIVE: To examine the relationship between tympanic membrane retraction and habitual sniffing in patients with cholesteatoma. METHODS: We recruited 98 patients (102 ears) who were surgically treated for cholesteatoma by canal wall-down tympanoplasty (22 ears) or canal wall-down tympanoplasty with reconstruction methods (80 ears). We classified these patients into two groups on t

An unusual dark pigmentation on the tympanic membrane.

OBJECTIVE: To report an extremely rare case of dark pigmentation on the tympanic membrane due to alkaptonuria, and to discuss the probable association between this condition and hearing loss. CASE REPORT: A 58-year-old man with alkaptonuria was admitted with tinnitus and hearing loss in both ears. Physical examination showed bluish-black pigmentation on the helixes of both ears and both sclerae. Otoscopic examination revealed dark discolouration of both tympanic membranes. Audiological evaluation revealed mixed high frequency hearing loss in both ears. Tympanometric examination revealed type

Primary cartilage tympanoplasty: our technique and results.

UNLABELLED: Cartilage has shown to be a promising graft material to close tympanic membrane perforations. However, due to its rigid quality, doubts are raised regarding its sound conduction properties. It has been suggested that acoustic benefit may be obtained by thinning the cartilage. We describe our innovative method for harvesting tragal cartilage from the same endaural incision and also describe preparation of the graft by slicing it. We present our 3-year experience of shield cartilage type 1 tympanoplasty using sliced tragal cartilage-perichondrium composite graft. AIM: The aim of thi

The extent of hearing impairment amongst Australian Indigenous prisoners in Victoria, and implications for the correctional system.

The hearing status of 109 Indigenous prisoners was investigated at five prison locations in Victoria, using audiological methods and face-to-face interview. The study found predominantly mild, sensorineural hearing loss. The rate of conductive hearing impairment was consistent with an age-matched general adult population (UK). All eardrums were intact, and 89% of middle-ears were normally air filled. Results showed 12% of prisoners had a hearing loss (average. 0.5, 1, 2, & 4 kHz >or=25 dB) in at least one ear, compared with 5% in an age-matched Australian adult population. More than a third (3

Efficacy of Otoendoscopy for Residual Cholesteatoma Detection During Microscopic Chronic Ear Surgery.

The aim of this article is to determine the efficacy of otoendoscopy during microscopic cholesteatoma surgery on residual cholesteatoma rates postoperatively. The medical records of patients (aged 4-90) with primary acquired cholesteatoma who underwent microscopic cholesteatoma surgery (exclusively transcanal approach or canal wall-up tympano-mastoidectomy) with subsequent otoendoscopic examination (80 ears) for intraoperative cholesteatoma residues were retrospectively reviewed. All cases with mixed microscopic/endoscopic, fully endoscopic, or fully microscopic dissection were excluded, as we

Crural attachment to promontory case report: implications for stapes development.

Understanding of the embryologic origin of the stapes remains controversial. Theories diverge upon whether the entirety of the stapes arises from a single source versus the footplate and suprastructure arising from distinct sources. A 12-year-old boy with left-sided conductive hearing loss had computed tomography of the temporal bone, revealing an inferiorly displaced left stapes, and a nonspecific density in the left Prussak's space. Exploratory tympanotomy revealed the crura of the stapes to be attached to the promontory. The stapes footplate was located in the oval window and was mobile.

Diagnostic inaccuracy and subject exclusions render placebo and observational studies of acute otitis media inconclusive.

BACKGROUND: Diagnostic accuracy and appropriate inclusion/exclusion criteria representative of children at greatest risk is of paramount importance in trials to evaluate placebo or observation as an option for acute otitis media (AOM) management. METHODS: Twelve observational studies spanning the time frame 1958-2005 and 13 natural history studies spanning the time frame 1968-2006 were evaluated for the diagnostic criteria, inclusion criteria, and exclusion criteria applied within the study design. RESULTS: Although a bulging or full tympanic membrane (TM) with effusion is the best indicatio

Hearing outcomes in children with primary ciliary dyskinesia--a longitudinal study.

Primary ciliary dyskinesia (PCD) is a congenital abnormality of ciliary structure or function. The otological manifestations of the disease include otitis media with effusion (OME). To date, the severity of hearing loss and natural progression of OME in this select group of patients has not been documented. In this retrospective observational study, we looked at the tympanograms and audiograms of all children with PCD attending the Royal Brompton Hospital multidisciplinary clinic. Our results show an improvement in both hearing thresholds and tympanograms with age (p<0.001). Most cases resolve
