Artificial Intelligence in Occlusion-Oriented Digital Reconstruction of Maxillofacial Fractures: Current Applications and Translational Challenges.

Maxillofacial fractures require reconstruction of a functional craniofacial unit rather than isolated realignment of fractured bone. Stable occlusion, mandibular movement, temporomandibular joint position, facial contour, and fixation-device adaptation should be considered as interdependent treatment targets. Digital workflows incorporating CT or CBCT reconstruction, virtual surgical planning, CAD/CAM, 3-dimensional printing, patient-specific implants or plates, and navigation have improved visualization and surgical transfer. However, comminution, bilateral injury, loss of anatomic references

Cost-effectiveness of current treatment strategies for lumbar spinal stenosis: nonsurgical care, laminectomy, and X-STOP.

OBJECT Standard treatment options for patients with lumbar spinal stenosis include nonoperative therapies as well as decompressive laminectomy. The introduction of interspinous decompression devices such as the X-STOP has broadened treatment options, but data comparing these treatment strategies are lacking. The object of this study was to provide a cost-effectiveness analysis of laminectomy, interspinous decompression, and nonoperative treatment for patients with lumbar stenosis. METHODS The authors performed a structured literature review of lumbar stenosis and constructed a cost-effectivene

Cervical locked facet: operative management.

Cervical locked facet represent 5.6% of cervical lesion. If an identity of views is present in the use of tractions, we can't say as for the setting fixation techniques. Our cases histories (13 cases) and the experience of other authors incline us to support a precocious surgical treatment characterized by a posterior approach and then anterior in the not reduced cases and a single anterior approach in the reduced cases in anesthesia. The good alignment and the attain stability with the double access, but still more the convinction that the lesions of intervertebral disk and ligamentous tissue

Standard and fenestrated endograft sizing in EVAR planning: Description and validation of a semi-automated 3D software.

An abdominal aortic aneurysm (AAA) is a pathological dilation of the abdominal aorta that may lead to a rupture with fatal consequences. Endovascular aneurysm repair (EVAR) is a minimally invasive surgical procedure consisting of the deployment and fixation of a stent-graft that isolates the damaged vessel wall from blood circulation. The technique requires adequate endovascular device sizing, which may be performed by vascular analysis and quantification on Computerized Tomography Angiography (CTA) scans. This paper presents a novel 3D CTA image-based software for AAA inspection and EVAR sizi

[Vertebroplasty as a treatment for painful pathological fractures of the vertebral column].

Vertebral fracture due to a metabolic bone disease or a neoplastic disease is a common and debilitating condition occurring mostly due to osteoporosis or metastatic bone disease. Some of the patients suffering from such fractures continue to complain of back pain and deformity despite optimal medical therapy. Vertebroplasty, i.e. transcutaneous injection of bone cement into the vertebral body can serve as an internal fixation device and allows restoration of mechanical strength and partial restoration of the vertebral height. During the year 2000, 17 vertebrae in 12 patients were injected. The

The challenge of implant integration in partial meniscal replacement: an experimental study on a silk fibroin scaffold in sheep.

PURPOSE: To restore meniscal function after excessive tissue damage, a silk fibroin implant for partial meniscal replacement was developed and investigated in an earlier sheep model. After 6 months implantation, it showed promising results in terms of chondroprotection and biocompatibility. To improve surgical fixation, the material was subjected to optimisation and a fibre mesh was integrated into the porous matrix. The aim of the study was the evaluation of this second generation of silk fibroin implants in a sheep model. METHODS: Nine adult merino sheep received subtotal meniscal replaceme

A minimally invasive stabilizing system for dorsal pelvic ring injuries.

BACKGROUND: Open reduction and stabilization of dorsal pelvic ring injuries is accompanied by a high rate of soft tissue complications. Minimally invasive techniques have the potential to decrease soft tissue trauma, but the risk of iatrogenic nerve and vessel damage through the reduced surgical exposure should be considered. We treated these injuries using a transiliac internal fixator (TIFI) in a minimally invasive technique characterized by implantation of a pedicle screw and rod system, bridging the sacroiliac joints and the sacral area. QUESTIONS/PURPOSES: We asked whether (1) we could a

[Unstable pertrochanteric femur fractures. Failure rate, lag screw sliding and outcome with extra- and intramedullary devices (PCCP, DHS and PFN)].

AIM OF THE STUDY: The dynamic hip screw (DHS) often shows a high incidence of therapeutic failure and an impared outcome in the treatment of the unstable pertrochanteric femur fracture (31A2). Therefore often an intramedullary device is recommended. In a retrospective clinical study we examined whether the percutaneous compression plate (PCCP, Gotfried) provides advantages following unstable fractures in comparison to DHS and PFN. METHODS: From January 2002 to April 2007 135 patients with unstable pertrochanteric femur fractures underwent internal fixation with the PCCP (n = 46, age 78.3, ASA

Two-year delayed reconstruction of chronic distal biceps tendon rupture with allograft: a case report.

Chronic distal biceps tendon ruptures present significant reconstructive challenges due to tendon retraction, scarring, and muscle atrophy. Allograft-based reconstruction is a viable option for delayed presentations, particularly in high-demand individuals. We report the case of a 33-year-old male bodybuilder who presented 2 years after sustaining a distal biceps rupture. Examination revealed a reverse Popeye deformity, marked weakness, and magnetic resonance imaging evidence of complete tendon rupture with substantial retraction and muscle atrophy. Surgical reconstruction was performed using

Effectiveness of the Dynamic Internal Fixator System in Metaphyseal and Epiphyseal Femoral Fractures: 10 Years of Experience and Future Expectations.

Angular stability plates are largely used for treating complex fractures, but the incidence of nonunion is high. The innovative dynamic internal fixator (DIF) system is comprised of a plate with angular stability that combines the principles of internal fixator with dynamic device. In this study, we evaluate the feasibility of surgical reconstruction using the DIF in adult patients with epiphyseal-metaphyseal femur fractures and examine complications and outcomes at long-term follow-up. We retrospectively analyze 26 patients (14 females and 12 males), with a mean age of 64 yr, who were treated

Surface-mediated bone tissue morphogenesis from tunable nanolayered implant coatings.

The functional success of a biomedical implant critically depends on its stable bonding with the host tissue. Aseptic implant loosening accounts for more than half of all joint replacement failures. Various materials, including metals and plastic, confer mechanical integrity to the device, but often these materials are not suitable for direct integration with the host tissue, which leads to implant loosening and patient morbidity. We describe a self-assembled, osteogenic, polymer-based conformal coating that promotes stable mechanical fixation of an implant in a surrogate rodent model. A singl

Lesser digital arthrodesis.

Arthrodesis at the proximal interphalangeal joint is a surgical procedure useful when dealing with semireducible or nonreducible contractures of the lesser digits. When arthrodesis of the proximal interphalangeal joint is performed, there is conversion of the retrograde dorsal buckling force of the long extensor tendon to a plantargrade straightening force of the flexor tendon. This occurs as a result of the rigid strut created at the arthrodesis site. If metatarsal equinus is present, arthrodesis can help reduce this deformity and eliminate plantar pressure contributing to submetatarsal tylom

Biomechanical comparison of different interspinous process devices in the treatment of lumbar spinal stenosis: a finite element analysis.

BACKGROUND: Lumbar spinal stenosis (LSS) is a common disease among elderly individuals, and surgery is an effective treatment. The development of minimally invasive surgical techniques, such as the lumbar interspinous process device (IPD), has provided patients with more surgical options. OBJECTIVE: To investigate the biomechanical properties of different IPDs, including BacFuse, X-Stop and Coflex, in the treatment of LSS. METHODS: Based on the computed tomography images of a patient with LSS, four finite element (FE) models of L3-S5 were created in this study. The FE models included a surgi
