PURPOSE: Historically dilation of the female urethra was thought to be of value in the treatment of a variety of lower urinary tract symptoms. Subsequent work has more accurately classified these complaints as parts of various diseases or syndromes in which scant data exist to support the use of dilation. Yet Medicare reimbursement for urethral dilation remains generous and we describe practice patterns regarding female urethral dilation to characterize a potential quality of care issue.

The perineal approach was used in ten patients for the repair of fistulas involving the bladder or prostatic urethra. In the case of radiotherapy-induced (n=2) or recurrent fistulas (n=4) fecal diversion and interposition of the gracilis muscle was performed. In addition in three patients prostatectomy was performed. All fistulas were repaired successfully with minimal morbidity.

CASE DESCRIPTION: A 1-year-old castrated male German Shepherd Dog was evaluated because of a history of hematuria and stranguria secondary to recurrent urinary tract infections.
CLINICAL FINDINGS: Physical examination revealed hypospadias with penile and preputial aplasia. The urethral orifice was just ventral to the ventral aspect of the anocutaneous junction. Ascending urinary tract infections, secondary to fecal contamination of the urethral orifice, were the presumed source of recurrent bouts of cystitis that developed despite periodic antimicrobial treatment.

OBJECTIVES: We report a clinical case of inverted papilloma of the anterior male urethra.
METHODS/RESULTS: 40-year-old male without significant past medical history presenting with dysuria and referring three episodes of urethral bleeding. Preliminary studies included voiding cystourethrogram (VCUG) which revealed a filling defect in the anterior urethra. Urethrocystoscopy was performed under anesthesia, with resection of the tumor. Pathologic study reported a urethral inverted papilloma.

The authors used as a standard a one-stage method of microsurgical reconstruction of the penis, urethra and glans in 38 transsexual patients, a combination of Biemer's method, Trengove-Jones and Horton's suture of the glans. The reconstruction method is described in detail. Although reconstruction of the penis and urethra involved a group of 49 patients and accounted for less than 10% of all microsurgical tissue transfers in traumatology, oncological surgery and congenital defects, they are an important activity in systematic surgery.

Endoscopy and radiological investigations, using contrast medium, have been the only available methods in the study of urethral pathology. A case of sensitive urethra treated with continent vesicostomy is reported. She suffered from atonic bladder due to a previous pelvic surgery and could not continue clean intermittent self-catheterization because of pain and discomfort, or sensitive urethra. Continent vesicostomy using detrusor tube following Mitrofanoff principle was performed. Postoperatively, the daily life activity improved markedly. Mitrofanoff operation is considered to be applicable to the cases with sensitive urethra after sufficient explanation and consent.

30 patients with strictures of the urethra (n = 19) and bladder cervix (n = 11) received combined treatment including surgical endoscopic correction of the stricture with Ho:YAG laser, pre- and postoperative endourethral radiation with low-energy laser, instillations of collagenase and dimexide solution into the urinary tract. The operation was made by Ho:YAG laser (2.09) microns, 4 J, 15 Hz, 300 mc). Persistent response for 10-12 months was achieved in 93.3% of patients.

The author gives theoretical validation for placing a longitudinal suture instead of a cross suture on the bladder cervix in cystourethral shunting. Cross sutures preclude deviations in the shunt region thus reducing probability of urethral stricture recurrences. For conduction of the suturing a special device "needle bougie" has been developed. Surgical treatment of 11 patients with recurrent strictures of the prostatic urethra with the use of the above unit secured positive results in all those treated.
