Poststreptococcal erythema nodosum and arthritis in two elderly women.

We describe two patients, aged 70 and 71, respectively, with poststreptococcal erythema nodosum and arthritis. The arthritis affected the ankles in both patients and the knees in one of them. In view of the pronounced erythema nodosum in both patients, the arthritis is considered as an associated symptom of poststreptococcal erythema nodosum. Therefore, this is an example of a third variant of poststreptococcal arthritis, in addition to rheumatic fever and poststreptococcal reactive arthritis. This type of arthritis can be seen even in the geriatric population.

Incidence of spondyloarthritis subtypes: a systematic review.

OBJECTIVES: Several epidemiologic studies of spondylarthritis (SpA) and its subtypes have been reported during the last decades. The majority of these studies provided prevalence estimates and showed a considerable variation in the reported frequency of SpA subtypes. Most systematic reviews published in this field aimed to summarise the results of prevalence studies, however, incidence studies are important for an accurate picture of a disease occurrence in a defined population. We conducted a systematic review regarding the incidence of SpA subtypes on studies published during the last 25 yea

Classical rheumatoid arthritis in a patient with Reiter's syndrome.

A 44 year old white male presented with sterile urethritis, anterior uveitis, painless oral ulcers, low back pain, and synovitis of the knees and left ankle. Unilateral sacroiliitis and limited spondylitis further supported a diagnosis of Reiter's syndrome. Serum rheumatoid factor was present. The patient subsequently developed morning stiffness, symmetrical polyarthritis of the hands and wrists, and an olecranon subcutaneous nodule. Histology of the nodule and synovium was consistent with rheumatoid arthritis. Histocompatibility antigen typing established the presence of HLA B27. The occurren

[HIV-induced clinical conditions: rheumatic conditions].

Many retroviruses have been shown to be involved in the etiology of rheumatic diseases both in animal models and in humans. In particular, HIV infection has been correlated to a greater incidence of Reiter syndrome, psoriatic arthritis, sicca syndrome and other seronegative polyarthritides. Aim of this paper is to discuss clinical findings, diagnostic strategies and therapy of these diseases. We will describe how the profound abnormalities in the immune system brought about by the HIV infection can induce the development of reactive arthritides, inhibit the clinical expression of rheumatoid ar

Infectious mimics of rheumatoid arthritis.

Rheumatoid arthritis (RA) can have various infectious mimics. As immunosuppressive agents used in treatment can aggravate the underlying infections, correct diagnosis of RA and ruling out infections is important. Numerous viral infections (Parvovirus B19, Hepatitis B, Hepatitis C, Chikungunya and other alphaviruses, human immunodeficiency virus (HIV) and various other viruses), mycobacterial infections (Poncet's disease, tubercular septic arthritis, and leprosy), bacterial arthritis, brucellosis and Lyme disease are among common infections that mimic RA. Widespread travel and tourism, especial

Spondyloarthropathies in childhood.

The concept of spondyloarthropathies in childhood is emerging. It should now be more easily recognized since more specific diagnostic criteria are available for young patients. It probably accounts for about 20% of the whole group of chronic arthritides seen in paediatric rheumatology clinics. Juvenile ankylosing spondylitis stricto sensu is very rare in childhood. Most children who present with peripheral arthritis at onset meet the diagnostic criteria of undifferentiated spondylo-arthropathies such as those of the SEA syndrome, or those of the ESSG or Amor criteria. At follow-up, quite a lar

Nail changes in the seropositive and seronegative arthridities.

There are a number of nail changes that can represent the presence of an underlying arthritic pathology. The changes may be secondary to the underlying pathology or its treatment. The astute practitioner should learn to recognize these findings in addition to other clinical presentations of the disease to aid in making a diagnosis. The podiatrist sees hundreds of nails each day. Knowledge of nail changes along with the salient features of these arthridities will aid the practitioner in diagnosing the appropriate arthritic process.

Role of ultrasound and inflammatory factors in the management of pediatric hip joint effusion.

BACKGROUND: Septic arthritis is an important differential diagnosis of hip joint pain. Joint aspiration analysis is a necessary diagnostic measure for septic arthritis. In order to reduce the need for joint aspiration, we compared the combination of ultrasound findings and laboratory findings to separate septic arthritis from reactive arthritis. METHODS: Children aged < 14 years who were referred to Akbar pediatric hospital in 2020-2022 with hip pain or limping were included in this longitudinal study. Participants underwent ultrasound examinations of the hip and blood samples were obtained f

Reactivation of cutaneous leishmaniasis in the context of non-HIV immunosuppression: a literature analysis.

BACKGROUND: Immunosuppression increases the risk of primary cutaneous leishmaniasis (CL) infection and its reactivation. This systematic review examined CL reactivation in immunosuppressed patients, including those with autoimmune diseases and solid organ transplant recipients. METHODS: A comprehensive literature search was conducted across multiple databases for studies published between May 1990 and May 2024. RESULTS: Nine cases were selected, involving CL reactivation associated with immunosuppressive therapy for autoimmune diseases, such as rheumatoid arthritis, ankylosing spondylitis an

[Reiter's disease--a review of an interdisciplinary disease].

Over the past 20 years the term 'Reiter's disease' has been expanded from the classical triad of sterile urethritis, conjunctivitis and arthritis to include a broader range of reactive arthritides. Indeed, the classical syndrome as described by Reiter is rarely seen in its complete form, and the presence or absence of active infection in the joint is being redefined in the light of recent techniques of molecular biology. The terminology of Reiter's syndrome, reactive arthritis and seronegative spondylarthropathy are defined here using case examples.

Mechanisms of disease: infection and spondyloarthritis.

There is compelling evidence that some infections can initiate a chronic nonseptic arthritis. This has proved to be an important area of investigation into gene-environment interactions, particularly since HLA-B27 confers increased susceptibility to reactive arthritis. This research has investigated the microbiology of these events, and the strategies used by pathogens to induce chronic joint inflammation. Insights into the HLA-orchestrated immune response in this context have also shed light on the impact of HLA-B27 on immunity, which might provide insights into the mechanism of other HLA-B27

The role of intracellular organisms in the pathogenesis of inflammatory arthritis.

Inflammatory arthritis is a condition which is characterised by recurrent episodes of joint pain and swelling. It encompasses a spectrum of disorders ranging from rheumatoid arthritis to ankylosing spondylitis. In these conditions, for reasons that are poorly understood, the immune system raises an inflammatory response within the joint space. In some cases, autoantigens have been identified (e.g., anticitrullinated peptides in rheumatoid arthritis), but the absence of these, in the seronegative arthritides, for example, raises question as to the underlying pathogenesis. Interest has, therefor
