[Prenatal screening for severe congenital heart defects in Denmark].

In Denmark, screening for congenital heart defects is a key component of the second-trimester ultrasound with > 95% of pregnant women participating. The screening boasts high detection rates, identifying > 80% of severe congenital heart defects before birth. Early detection enables accurate diagnosis and genetic testing, as well as timely planning and intervention. A national virtual conference platform connecting paediatric cardiologists and foetal medicine specialists enhances the detection and coordination of treatment, improving outcomes for children with congenital heart defects.

Anomalous Single Ostial Origin of Entire Coronary Arteries From the Right Coronary Sinus: A Rare Case of ST-Segment Elevation Myocardial Infarction Treated With Primary Percutaneous Coronary Intervention.

This report is about managing ST-segment elevation myocardial infarction (STEMI) in a patient with a coronary anomaly of single ostial origin of entire coronary arteries from the right coronary sinus. To date, no cases have been reported of such an anatomical irregularity in a comparable clinical situation that managed with emergent revascularization. The administration of such cases, whether through primary percutaneous coronary intervention (PPCI) or emergent cardiac surgery, underscores the comprehensive understanding of uncommon anatomical variations among cardiologists.

Evolution of mechanical ventilation of the newborn infant.

Artificial ventilation of the newborn infant is the foundation of neonatology. Early practitioners included pediatricians, anesthesiologists, cardiologists, respiratory therapists, and engineers. The discovery of surfactant, followed by the death of Patrick Kennedy, jump-started the new area, with investment and research rapidly expanding. The ever more complex design of mechanical ventilators necessitated a more thorough understanding of newborn pulmonary physiology in order to provide support with minimal associated injury. This piece briefly reviews and highlights this history.

Microcatheters: A valuable tool in the presence of a challenging coronary anatomy in the setting of acute coronary interventions. Case report and mini review.

Popularity of microcatheters (MCs) is growing among interventional cardiologists, especially when complex coronary anatomy is involved. However, MCs are still considered by many as a niche tool and their value in common clinical practice and in the setting of acute coronary interventions has not been fully appreciated. This case report highlights the decisive role of MC use in the successful completion of a primary percutaneous coronary intervention. Characteristics and indications of the most commonly used MC are briefly discussed.

Secundum atrial septal defect with Eisenmenger syndrome: A scarcely reported disease in Nigeria.

Atrial septal defect (ASD) may be rarely associated with Eisenmenger syndrome (ES), the most advanced form of pulmonary vascular disease to complicate a congenital heart disease. In spite of availability of pediatric cardiologists and two-dimensional-echochardiography in many tertiary health care centers in Nigeria, late referral and late diagnosis of ASD still occur leading to the development of ES. We describe a case of a 19-year-old male with ASD who presented late with ES.

Percutaneous revascularization of anomalous left main coronary artery stenosis with a retro-aortic course.

We described the rare case of a 54-year-old male patient with an anomalous left main coronary artery, originating from the right sinus with a retro-aortic course. A significant distal left main coronary artery (LM) atherosclerotic lesion was identified and a successful PCI with direct stenting was performed. Coronary artery abnormalities represent the most technically challenging cases for interventional cardiologists. However, in selected cases, a percutaneous intervention can offer an effective and safe therapeutic option.

Case of fatal toxic epidermal necrolysis due to cardiac catheterization dye.

Cardiac catheterization is a very common procedure carried out for diagnostic as well as therapeutic reasons. Complications are not surprising due to the invasive nature of the procedure. Most of these complications have been seen numerous times by cardiologists who frequent the catheterization laboratory. Unfortunately, this patient experienced an extremely rare complication that ultimately resulted in his death. Here, the authors report a case of toxic epidermal necrolysis (TEN) resulting from the nonionic radiocontrast agent used in cardiac catheterization.

Case report: delirium due to a diltiazem-fentanyl CYP3A4 drug interaction.

INTRODUCTION: Fentanyl and diltiazem are frequently used medications. Diltiazem inhibits cytochrome P450 3A4 isoenzymes. This can suppress fentanyl metabolism. METHOD: We present a case of delirium after coadministration of fentanyl and diltiazem. DISCUSSION: Cautious use is warranted while concomitantly administering fentanyl and diltiazem as this can potentiate fentanyl toxicity. Other 3A4 inhibitors include ketoconazole, erythromycin, nefazodone, ritonavir, delavirdine, aprepitant and imatinib. Psychosomatic medicine psychiatrists, pain and palliative care physicians and cardiologists in …

Fontan operation through a right lateral thoracotomy to treat Cantrell syndrome with severe ectopia cordis.

A median sternotomy would be very difficult for Cantrell syndrome with severe ectopia cordis. For Cantrell syndrome and tricuspid atresia after left modified Blalock-Taussig shunt with severe ectopia cordis, defect in the middle and inferior portion of the sternum, and the closing of ventriculo-peritoneal shunt, we performed extracardiac total cavopulmonary connection through a right lateral thoracotomy after establishing right modified Blalock-Taussig shunt and performing coil embolization of left modified Blalock-Taussig shunt by cardiologists.

[Unilateral angioedema of the tongue and the floor of the mouth as a side-effect of ACE-inhibitors].

An 80-year-old woman who was medically compromised had recurrent diffuse unilateral swelling of the tongue and the floor of the mouth. The clinical working diagnosis of angioedema as a result of the use of ACE-inhibitors (lisinopril) was made. In consultation with the cardiologist in charge of her case and the general practitioner, the ACE-inhibitor was discontinued. The patient's complaints subsequently ceased.

Unguided temporary pacing via jugular/subclavian vein in an emergency department of a high-volume tertiary care hospital of India: its safety, efficacy, and practicability.

BACKGROUND: Temporary pacing is usually performed by cardiologists under fluoroscopic, echocardiographic, or ECG guidance. However, in the developing world, there are inadequate number of cardiologists, and C-arm, catheterization laboratories, or echocardiography are not available at primary or secondary healthcare facilities. In addition, in emergencies option of fluoroscopy and echocardiography is limited. So these patients are transferred to a facility where cardiologists and these facilities are available. Crucial time is lost in transit, which leads to increased mortality. In this study, …

Nonsustained ventricular tachycardia.

The patient with nonsustained ventricular tachycardia represents a common management problem for the cardiologist. The challenges posed by this type of arrhythmia differs from those posed by other arrhythmias, because most instances of nonsustained ventricular tachycardia do not cause symptoms. This article reviews common situations in which nonsustained ventricular tachycardia occurs and their appropriate management.

Percutaneous closure of patent foramen ovale in a patient with situs viscerum inversus.

We describe the case of a patient with situs viscerum inversus totalis in whom we performed percutaneous closure of a patent foramen ovale. This case report may represent a further contribution to illustrate instrumental and interventional issues to consider in patients with situs viscerum inversus; it is also an example in which a background in embryology and congenital heart disease may aid cardiologists for the well-tolerated and effective diagnosis and treatment of adult patients with cardiac anomalies.

[Congenital heart disease in adults. Part I. Non-cyanotic heart diseases].

Over the last decades major advances have been made in the diagnosis and treatment of congenital heart disease. Therefore, many children with simple and complex congenital heart defects can now survive to adulthood and represent an expanding population. Most of them are not "cured" and need specialized follow-up. This two-part review presents the more common acyanotic and cyanotic congenital heart diseases that adult cardiologists are likely to face in their daily practice and discusses recent treatment advances.

Patent foramen ovale closure for secondary prevention of cryptogenic stroke.

Introduction: A patent foramen ovale (PFO) is highly prevalent among the adult population. It allows shunting of blood through the inter-atrial septum and has been associated with cryptogenic stroke, transient ischemic attack, platypnea-orthodeoxia syndrome, decompression sickness (e.g. deep-water divers), and migraines.Areas Covered: In this manuscript, we will review the anatomy of PFO with particular emphasis on the factors associated with increased risk of paradoxical embolization, as well as the different modalities for the diagnosis of PFO. We will discuss medical, surgical, and transcat

Redo aortic valve replacement for prosthesis endocarditis in patients previously classified as high or prohibitive risk: a narrative review.

Transcatheter aortic valve implantation (TAVI) and sutureless aortic valve replacement (Su-AVR) enabled in the last years many patients at high or prohibitive risk to be treated for their severe symptomatic aortic valve stenosis. As often happens in medicine, new techniques bring not only new hopes, but also new problems. In recent years, alongside the lengthening of the life of these patients treated with TAVI or Su-AVR, cardiologists and cardiac surgeons have had to face the long-term complications associated with the implantation of these devices, such as the prosthetic infective endocardit…

Hybrid approaches to complex congenital cardiac surgery.

OBJECTIVES: A hybrid operation is a joint procedure involving the interventional cardiologist and the cardiac surgeon concomitantly to optimise surgical management. The aim of our study was to demonstrate the conceptual development and the feasibility of a hybrid approach to complex congenital cardiac surgery. METHODS: Descriptive study of two different indications for concomitant intervention by the cardiologist and the cardiac surgeon. Seven patients with complex congenital heart defects requiring high risk operative interventions were included in the study. The indications were: (1) intrao

Late results of surgery for congenital heart defects.

Surgery for congenital heart defects started 50 years ago with "closed" procedures, and open heart surgery is in its forth decade. Thus, long-term results are now available. Although a majority of patients lead normal lives, problems do exist. Hemodynamic anomalies can be related to residual lesions or to persistent systemic hypertension (after coarctation repair) or pulmonary hypertension (after late repair of left to right shunt lesions). Right or left ventricular dysfunction may be observed, due to longstanding overload, hypoxia or to the open heart procedure itself. Rhythm disturbances hav
