pH-Activated Dissolvable Polymeric Coatings to Reduce Biofouling on Electrochemical Sensors.

Implantable electrochemical sensors that enable the real-time detection of significant biomarkers offer huge potential for the enhancement and personalisation of therapies; however, biofouling is a key challenge encountered by any implantable system. This is particularly an issue immediately after implantation, when the foreign body response and associated biofouling processes are at their most active in passivating a foreign object. Here, we present the development of a sensor protection and activation strategy against biofouling, based on coatings consisting of a pH-triggered, dissolvable po

Treatment of spastic dysphonia without nerve section.

Spastic dysphonia is a disorder characterized by strained, constricted phonation with excessively adducted vocal cords. Despite initial success with recurrent laryngeal nerve section, the search for other treatment continues. Our clinical study involved inserting a needle electrode percutaneously into the region of the recurrent laryngeal nerve in five patients with spastic dysphonia. Electrical stimulation resulted in dramatic improvement in three patients and minimal improvement in two. Our experimental study was designed to create an animal model for an implantable nerve stimulator to be us

Pacemaker upgrade for recurrent pacemaker syndrome using a redundant contralateral electrode in a patient with bilateral venous stenoses.

Following His-bundle ablation and VVIR pacemaker implantation, severe pacemaker syndrome developed and was treated with DDDR pacing, in a 70-year-old woman. Due to bilateral subclavian vein stenoses, DDDR pacing could not be maintained and an unusual method of restoring atrioventricular synchrony is described using the contralateral redundant atrial electrode connected to the ipsilateral dual chamber pacemaker and ventricular electrode.

[Pacemaker implantation in a patient with persistent left superior vena cava and atresia of the right superior vena cava].

Persistent left superior vena cava with absence or atresia of right superior vena cava is a congenital abnormality of systemic venous return to the heart which may complicate the insertion of a pacemaker electrode. We present a case report of a patient with conduction system disturbances in whom this abnormality was diagnosed during pacemaker implantation, with the possibility of using a transvenous electrode without active fixation system.

Permanent right ventricular pacing through an anomalous left superior vena cava.

A persistent left superior vena cava can complicate the implantation of a transvenous pacemaker. In a patient who required a permanent pacemaker, this venous anomaly was discovered during the insertion of the electrode but it did not prevent long-term right ventricular pacing. This was achieved after the electrode had been manipulated through the coronary sinus and right atrium. A plan of management is proposed for dealing with this unexpected problem.

Does cochleostomy location influence electrode trajectory and intracochlear trauma?

OBJECTIVES/HYPOTHESIS: Trauma to intracochlear structures during cochlear implant insertion is associated with poorer hearing outcomes. One way surgeons can influence insertion trauma is by choosing the surgical approach. We seek to compare cochleostomy (CO), peri-round window (PRW), and round window (RW) approaches using a fresh frozen temporal bone model. STUDY DESIGN: Experiments using fresh frozen temporal bones. METHODS: Cochlear implant insertions using the three aforementioned approaches were performed on 15 fresh frozen human temporal bones using a Cochlear 422 electrode. Insertions

Cognitive effects of deep brain stimulation for essential tremor: evaluation at 1 and 6 years.

Only a few studies have explored cognitive changes with deep brain stimulation (DBS) in patients with essential tremor (ET). Furthermore, the cognitive effects after years of electrical stimulation are unknown. Assessing the impact of stereotactic electrode implantation and the actual electrical stimulation on cognition in patients with ET in the short and long term is of interest, because DBS is increasingly applied and can offer deeper insight into human brain functions. We examined nine ET patients before surgery (PRE-SURGERY), and 1 and 6 years thereafter with DBS switched on (DBS-ON) and

Progressively Recovering Auditory Brainstem Response in a Cochlear-implanted Child After Meningitis: A Case Report.

OBJECTIVE: I describe the case of an 8-year-old boy who developed meningitis several years after implantation and how electrically evoked auditory brainstem response (EABR) is used in the assessment of his hearing performance after recovery. PATIENT: An 8-year-old boy stopped using both his cochlear implants because of streptococcal meningitis accompanied with acute otitis media on the left ear. After recovery his hearing was quickly restored on the right ear, but he complained of not hearing well on the left ear. INTERVENTION: Diagnostic. MAIN OUTCOME MEASURE: EABRs were measured on both e

Measuring implanted patient response to tone pips.

BACKGROUND: An electrical potential not previously reported-electrical cochlear response (ECR)-observed only in implanted patients is described. Its amplitude and growth slope are a measurement of the stimulation achieved by a tone pip on the auditory nerve. The stimulation and recording system constructed for this purpose, the features of this potential obtained in a group of 43 children, and its possible clinical use are described. The ECR is obtained by averaging the EEG epochs acquired each time the cochlear implant (CI) processes a tone pip of known frequency and intensity when the patien

Transnasal Ventral Brainstem Epidural Electrical Stimulation Enhances Consciousness in Anesthetized Sheep: A Feasibility Study.

AIM: Disorders of consciousness (DOC) remain a major clinical challenge, and the efficacy of currently available neuromodulation strategies remains limited. The brainstem reticular formation is central to arousal regulation, but approaches directly targeting it remain technically demanding. This feasibility study aimed to explore whether epidural electrical stimulation applied via a transnasal clival approach could induce arousal and the electrocorticographic (ECoG) changes. METHODS: Three anesthetized sheep were implanted with epidural stimulating electrodes placed over the ventral brainstem

Surgical Approaches for Possible Positions of an Olfactory Implant to Stimulate the Olfactory Bulb.

INTRODUCTION: Current scientific developments seem to allow for an "olfactory implant" in analogy to cochlear implants. However, the position and surgical approaches for electrical stimulation of the olfactory system are unclear. METHODS: In a human anatomic cadaver study, we investigated different endoscopic approaches to electrically stimulate the olfactory bulb (OB) based on the following considerations: (1) the stimulating electrode should be close to the OB. (2) The surgical procedure should be as non-invasive and safe as possible and (3) as easy as possible for an experienced ENT surgeo

Intraparenchymal and Subarachnoid Hemorrhage in Stereotactic Electroencephalography Caused by Indirect Adjacent Arterial Injury: Illustrative Case.

The complication rate of stereotactic electroencephalography (SEEG) is generally low, but various types of postoperative hemorrhage have been reported. We presented an unusual hemorrhagic complication after SEEG placement. A 20-year-old man presented with suspected frontal lobe epilepsy. We implanted 11 SEEG electrodes in the bilateral frontal lobes and the left insula. Computed tomography after implantation showed intraparenchymal hemorrhage in the left temporal lobe and insula and subarachnoid hemorrhage in the left Sylvian cistern. Later, the point of vessel injury was revealed from the ide

Position-Dependent Dysfunction of Deep Brain Stimulation in Tourette Syndrome: Diagnostic Clues.

BACKGROUND: Detection of defective deep brain stimulation (DBS) contacts/electrodes is sometimes challenging. CASE REPORT: We report a patient with Tourette syndrome (TS), who presented with abrupt tic increase and mild generalized headache 9 years after DBS implantation. On the suspicion of a hardware defect, a fracture of the DBS electrode and extension lead was ruled out by radiography and standard implantable pulse generator readouts. Further investigation revealed position-dependent modifiable therapeutic impedances, suggesting an impaired contact of the extension lead/adaptor. After rep

Assessment of responses to cochlear implant stimulation at different levels of the auditory pathway.

This paper reviews characteristics of both the electrically evoked compound action potential (ECAP) and analogous measures of cortically evoked responses (CAEP) to electrical stimulation in cochlear implant users. Specific comparisons are made between the two levels of processing for measures of threshold, growth of responses with increasing stimulus level, changes in stimulation electrode and, finally, in temporal response properties. The results are interpreted in a context that ECAPs primarily reflect the characteristics of the electrode-neural interface for an individual ear. CAEPs clearly

[Epicardial atrial pacing using an active fixation bipolar endocardial lead in children].

Good performance was observed over 10 years after implantation of bipolar epicardial atrial pacing using an active fixation bipolar endocardial lead in 3 pediatric patients with congenital heart block. The bipolar endocardial lead which was supposed to be fixed transvenously was implanted on the atrial surface by first screwing the lead's helix into the myocardium. The catheter was then laid down on the atrial surface, and both electrodes were wrapped by the atrial tissue. The good performance of this pacing lead seemed to depend on stable positioning of the electrode. This lead is superior to

Safety considerations for deep brain stimulation: review and analysis.

Deep brain stimulation has emerged rapidly as an effective therapy for movement disorders. Deep brain stimulation includes an implanted brain electrode and a pacemaker-like implanted pulse generator. The clinical application of deep brain stimulation proceeded in the absence of clear understandings of its mechanisms of action or extensive preclinical studies of safety and efficacy. Post mortem studies suggest that there is a loss of neurons in proximity to the active electrode, but the resulting lesions are not sufficient to treat the disorder and efficacy requires continued stimulation. Overa
