Implants are widely used for orthopaedic applications such as fixing fractures, repairing non-unions, obtaining a joint arthrodesis, total joint arthroplasty, spinal reconstruction, and soft tissue anchorage. Previously, orthopaedic implants were designed simply as mechanical devices; the biological aspects of the implant were a byproduct of stable internal/external fixation of the device to the surrounding bone or soft tissue. More recently, biologic coatings have been incorporated into orthopaedic implants in order to modulate the surrounding biological environment. This opinion article revi

An octagenerian woman developed clear cell hidradenocarcinoma, a rare neoplasm of eccrine sweat gland origin, 4 years following pacemaker implantation in her right lateral chest. The tumor immunohistochemically mimicked a metastatic lobular breast carcinoma, for example, strongly positive estrogen, weakly positive progesterone, and weakly reactive mammoglobin. A complete surgical excision of the tumor was complemented with ipsilateral dissection of involved adjacent axillary lymph nodes. Recommended irradiation was refused by the patient. Retrospective 3-year mammogram review, 2-year postsurge

PURPOSE: To describe the sonographic and mammographic features of patients whose breasts have been injected with silicone.
METHODS: Between July 1997 and August 1999, 14 patients with a history of breast injection of liquid silicone underwent physical, mammographic, and sonographic examination. Mammographic findings were classified as macronodular, micronodular, or mixed striated patterns. Sonographic appearances were classified as macronodular, micronodular, mixed, or snowstorm patterns.
RESULTS: Eighty-six percent of the patients had abnormal physical examination. Well-defined nodules were

A 44-year-old female was initially evaluated for a 3-cm umbilical hernia, which developed after a laparoscopic myomectomy performed seven years prior. The umbilical hernia was repaired using a synthetic mesh. Eight months after the umbilical hernia repair, the patient returned with chronic pain in a 3-cm raised mass originating from the umbilical hernia repair incision. The mass and mesh were surgically removed. The umbilical fascial defect was repaired with a primary fascia-to-fascia closure and the umbilicus was reconstructed from adjacent skin. The mass was found histologically to be endome

The authors report on a patient with severe metallosis caused by shaving down of the stem neck due to outer cup impingement. A primary arthroplasty was performed using an Omnifit bipolar endoprosthesis (Stryker) with an acetabular bone graft fixed by ceramic screws. At the time of revision surgery the synovium was stained black. Two ceramic screws were broken, but no scratch was observed on the metal surfaces of the outer cup and the inner head. The ultra-high molecular weight polyethylene (UHMWPE) of the inner-bearing collar was worn and flat. During the operation, a groove was observed at th

To determine the effect of copper in tissue, copper specimens, along with other metals commonly used in firearm projectiles, were implanted into rats. Copper consistently produced a necrotic, abscesslike reaction in all tissues tested. Nickel produced a similar reaction, although of lesser magnitude. Aluminum, zinc, and lead produced no significant tissue reaction.

The clinical records of 69 patients who had Teflon injection of the vocal cord are reviewed. The indications for treatment, methodology and results of therapy are discussed. The histological findings in three serially sectioned autopsy larynges which had been injected with Teflon paste during life are also presented. Special attention is given to the distribution of Teflon and to the local tissue reaction. The findings are discussed in conjunction with the current concepts regarding the use of this therapeutic modality.

Sinonasal mesenchymal neoplasms are challenging to classify due to their rarity. Xenogeneic collagen-based dural substitutes, such as DuraMatrix, are used for dural repair in sinonasal surgery, but their histologic features have been described only briefly in a few animal studies. When encountered in sinonasal specimens in the absence of a relevant surgical history, these materials may be mistaken for incidental mesenchymal neoplasms because of features such as intermixed spindle cells and a minimal foreign-body giant cell reaction. By detailing the histopathologic findings, we aim to facilita

A 70-year-old man underwent right upper lobectomy for lung adenocarcinoma. During the operation, hemostatic matrix (as known Floseal®) was used to prevent pulmonary laceration-associated bleeding. When 18F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography was performed for staging after surgery, intense 18F-FDG uptake was observed in the cicatricial fibrotic tissue in the operation area, and no significant change was observed in that area during the 4-year follow-up. Because it remained stable for several years without treatment, this finding was thought to be due to a

In illicit drug use, "skin popping" refers to the method of administering drugs by injecting it into the subcutaneous tissue. "Skin popping" can lead to acute and chronic skin manifestations caused by the drug itself and/or by the various adulterants and fillers commonly added to drug substances. Chronic skin manifestations include a foreign-body reaction with formation of granulomas at the site of injection. We present the F-FDG PET/CT findings of extensive subcutaneous granulomas in a 58-year-old woman with a heroin addiction undergoing treatment for infective endocarditis of the mitral valv…

Tophaceous deposits in lumbar spine is considered a rare condition. We report the case of a 44-year-old patient with low back pain and radiculopathy. Radiographs revealed lytic spondylolisthesis in L5. Magnetic resonance imaging showed hypointense signal on T1 and a heterogeneous signal on T2 located in the L4-L5 interspinous space and in the left facet joint that invades left neuroforamen. The left knee ultrasound showed «double contour» of the medial femoral condyle. Decompressive laminectomy with arthrodesis at the level of L5-S1 was performed. The histological examination revealed amorphou

Ten cases using decellularized allografts and xenografts for the purpose of resurfacing the first metatarsal head are reviewed in this article. Although most of the cases were performed without any postoperative complications, the focus of this series is on 2 of the 3 cases in which destruction of the first metatarsal head was observed postoperatively due to a foreign body reaction and severe degeneration within the metatarsal head. A salvage procedure using a silicone total joint to replace the damaged surface is shown. The role of cystic changes present preoperatively, and its role in subseq
