Nomogram for predicting selective renal arterial embolization in post-PCNL hemorrhage among patients with chronic kidney disease.

PURPOSE: Percutaneous nephrolithotomy (PCNL) is the standard treatment for large or complex renal calculi; however, postoperative hemorrhage requiring selective renal arterial embolization (SRAE) remains a rare but serious complication. Chronic kidney disease (CKD) may further increase bleeding risk, but predictive models for SRAE in this population are lacking. To develop and validate a nomogram for predicting the need for SRAE in patients with CKD who experience hemorrhage after PCNL. METHODS: This retrospective cohort study included 3153 patients who underwent PCNL in two tertiary hospital

Single session removal of encrusted double J stent using a simultaneous endourological approach: A case series.

Double J (DJ) stent encrustation is a serious urological complication that often requires complex surgical management. This study evaluated the feasibility, safety, and efficacy of a novel combined antegrade and retrograde endourological approach to remove encrusted DJ stents. A retrospective analysis was conducted involving 12 patients treated between May 2020 and April 2025 at a tertiary university hospital. All patients underwent simultaneous antegrade and retrograde procedures. The inclusion criterion was encrustation of both the renal and vesical coils, with some cases involving ureteral

Robotic pyelolithotomy for treating large renal stone disease: a systematic review and single-arm meta-analysis.

Robotic pyelolithotomy continues to gain attention as an alternative to percutaneous nephrolithotomy (PCNL) for managing complex renal stones. We performed a single-arm meta-analysis and systematically searched the English-language literature published in PubMed, Web of Science, Scopus, and Google Scholar databases up to June 2024. The risk of non-randomized bias was assessed using ROBINS-I, and the quality of the literature was assessed using MINORS (Methodological Index for Non-Randomized Studies). Merger parameters were calculated using Stata16/SE under a random-effects model. Five non-comp

[Quantitative phase imaging (QPI) of peripheral blood platelets for evaluation of thrombotic and hemorrhagic complications in patients with staghorn kidney stones after PCNL].

INTRODUCTION: An evaluation and prognosis of complications of different treatment options in patients with staghorn stones are necessary to choose optimal surgical strategy and perioperative prophylaxis on individualized basis. Intra- and postoperative thrombotic and hemorrhagic complications are not still well-studied in modern operative urology. AIM: To explore the influence of morpho-densitometry changes of blood platelets on perioperative thrombotic and hemorrhagic complications in patients with staghorn nephrolithiasis after percutaneous nephrolithotomy (PCNL). Management of Pediatric Urolithiasis Using a Combination of Laparoscopic Lithotomy and Pyeloscopy.

BACKGROUND: Minimally invasive procedures have become the standard option for treatment of urinary stones; the use of more invasive techniques is not a common practice currently. This emergent technology is not always available and laparoscopy is the alternative option. There are few reports of laparoscopic management for urinary lithiasis in pediatric patients. This study is about the use of laparoscopic surgery combined with pyeloscopy as a feasible first-line treatment for pediatric urolithiasis in renal and proximal ureteral locations. PATIENTS AND METHODS: We reviewed the records of 14 p

Nephrolithiasis in autosomal dominant polycystic kidney disease.

BACKGROUND AND PURPOSE: Autosomal dominant polycystic kidney disease (ADPKD) manifests with renal and extrarenal abnormalities and is inherited in an autosomal dominant fashion. In addition to multiple renal cysts, abnormalities such as liver cysts (80%), pancreatic cysts, splenic cysts, pulmonary cysts, berry aneurysms in the distribution of arterial circle of Willis (8%), colonic diverticula, mitral valve prolapse, etc., can be present. The condition will develop in half of the offspring of affected persons because of its 100% penetrance. Nephrolithiasis in patients with ADPKD is not infrequ

Diagnosis and management of postpercutaneous nephrolithotomy residual stone fragments.

Residual stone fragments can occur in up to 8% of patients who are treated with percutaneous nephrolithotomy (PCNL). When left untreated, approximately half of these patients will experience a stone-related event, for which more than half will need a secondary surgical intervention. Predictors of adverse events are a residual fragment larger than 2 mm that is located in the pelvis or ureter. Preventive measures for the creation of residual fragments include a carefully selected access giving exposure to the bulk of the stone, the creation of multiple tracts, the use of single pulse pneumatic l

[Urolithiasis-Therapy and recurrence prevention taking into account gender-specific aspects].

The prevalence of urolithiasis is steadily increasing worldwide in both genders. Diet and lifestyle, such as the rising prevalence of obesity and other metabolic syndrome traits, are considered key factors in this trend. Gender differences as a result of interventional therapy for urolithiasis have not been observed. However, iatrogenic injury to the male urethra is considered the most common reason for urethral strictures after endourologic (stone) therapy. In contrast, sepsis, as the major cause of urinary stone-related mortality, is more frequently reported in women after ureterorenoscopy a

Management of staghorn renal stones.

Staghorn stones are large branching stones that fill part of all of the renal pelvis and renal calyces and they can be complete or partial depending on the level of occupancy of the collecting system. Although kidney stones are commoner in men, staghorn stones are less often reported in men compared to women and they are usually unilateral. Due to the significant morbidity and potential mortality attributed to staghorn stones, prompt assessment and treatment is mandatory. Conversely, conservative treatment has been shown to carry a mortality rate of 28% in 10-year period and 36% risk of develo

Wandering Double-J Stent in the Retroperitoneum: A Case Report.

Background: Double-J (DJ) stent is an integral part of urologic practice. DJ stents can have numerous complications such as persistent irritative symptoms, encrustations, and up and down migrations within the pelvicaliceal system (PCS), but displacement outside the urinary tract is rare. We are presenting a unique case of DJ stent lying outside the PCS for more than a decade. Case Presentation: A 46-year-old female presented with left flank pain and dysuria. She had undergone percutaneous nephrolithotomy in the left side 11 years ago. Imaging studies revealed a forgotten DJ stent with proximal

Mortality and flexible ureteroscopy: analysis of six cases.

PURPOSE: Advancements in the endourological equipment have made retrograde intrarenal surgery (RIRS) an attractive, widespread technique, capable of competing with traditional shock wave lithotripsy and percutaneous nephrolithotomy. Since the complication rate is generally low, even less is known about dramatic and fatal complications after RIRS. METHODS: We performed a survey asking 11 experienced endourologists to review their RIRS series and report the cases of mortality to their best knowledge. RESULTS: Six urologists reported on six fatal cases. In three cases, a history of urinary trac

The impact of chronic kidney disease stages and CROES AND GSS scores on stone free rate in kidney stones.

INTRODUCTION: This study evaluates the impact of chronic kidney disease (CKD) stages on stone-free rates (SFR) and renal function outcomes after percutaneous nephrolithotomy (PCNL). Additionally, it examines the predictive role of the CROES and Guy's Stone Score (GSS) systems. METHODS: Data from 2994 patients who underwent PCNL between 2007 and 2024 were retrospectively analyzed. Patients were classified into four CKD groups based on preoperative estimated glomerular filtration rate (eGFR). SFR, complication rates, and postoperative renal function changes were assessed. RESULTS: SFR was sign
