Double disruptions of the superior shoulder suspensory complex.

The superior shoulder suspensory complex (S.S.S.C.), a bony/soft tissue structure, is important both for its role as an intact unit and for the individual components that make up this unit. Traumatic "double disruptions" of the S.S.S.C. frequently create an unstable anatomic situation with adverse long-term healing and functional consequences. This "double disruption" principle underlies, unites, and allows one to understand several well-described but difficult-to-treat shoulder injuries that have previously been described in isolation. Injuries to the S.S.S.C. require careful radiologic evalu…

Inferior glenohumeral ligament (IGHL) complex: anatomy, injuries, imaging features, and treatment options.

The inferior glenohumeral ligament (IGHL) complex is comprised of three components supporting the inferior aspect of the shoulder. It consists of an anterior band, a posterior band, and an interposed axillary pouch. Injuries to the IGHL complex have a unifying clinical history of traumatic shoulder injury, which are often sports or fall-related, with the biomechanical mechanism, positioning of the arm, and individual patient factors determining the specific component of the ligamentous complex that is injured, the location of the injury of those components, and the degree of bone involvement.

Sonography and MRI of latissimus dorsi strain injury in four elite athletes.

OBJECTIVE: The objective of this study was to describe the MR and sonographic findings in latissimus dorsi (LD) muscle strain in athletes, and to review the most common injuries described in the literature, most of which are humeral avulsions. MATERIAL AND METHODS: Four injuries and two reinjuries of the myotendinous junction of the LD were followed from the day of injury until the return to play. Sonography (US) and MR imaging were performed in each case to confirm the diagnosis and to monitor the healing process. RESULTS: All cases had acute and isolated pain in the back of the shoulder wh

Sonography of affected and unaffected shoulders in hemiplegic patients: analysis of the relationship between sonographic imaging data and clinical variables.

OBJECTIVE: To explore the relationship between a number of clinically relevant variables and sonographic imaging data in respect to the level of impairment experienced in the affected and unaffected shoulders of hemiplegic stroke patients. METHOD: Fifty-one hemiplegic stroke patients (32 males, 19 females; 29 right-sided hemiplegics, 22 left-sided hemiplegics) participated in this study. A musculoskeletal radiologist conducted a sonographic exam on both the affected and unaffected shoulders of all patients and two physicians classified the severity of the injury on a six-point rating scale. Concomitant intra-articular glenohumeral injuries in displaced fractures of the lateral clavicle.

PURPOSE: To detect concomitant intra-articular glenohumeral injuries, in acute displaced fractures of the lateral clavicle, initially missed due to unfeasible clinical evaluation of the acutely injured shoulder. METHODS: All patients suffering from an acute displaced lateral clavicle fracture with indication to surgical treatment underwent diagnostic shoulder arthroscopy prior to open reduction and internal fixation. In case of therapy-relevant intra-articular glenohumeral injuries, subsequent surgical treatment was performed. RESULTS: Intra-articular injuries were found in 13 of 28 patients

Normal anatomy, variants and pitfalls on shoulder MRI.

MR imaging of the shoulder is widely used for assessment of impingement and instability-related clinical conditions. The following review article demonstrates the normal anatomy, variations and classical pitfalls. In addition to classical pitfalls (sublabral hole, sublabral recess, Buford complex) the authors focus on a number of normal, bony, cartilaginous, ligamentous and tendinous structures that can simulate disease at the shoulder. In addition, ways to distinguish these pitfalls from true shoulder abnormalities are shown.

"Fighter's shoulder" part I: impact on shoulder motion and scapular movement in judo and jiu-jitsu athletes.

BACKGROUND: Martial arts athletes, such as judo and jiu-jitsu, often develop musculature asymmetry due to repetitive movements over the years of practice. Common injuries include strains, sprains, and fractures, with shoulder and upper-arm injuries being particularly prevalent. The objective of this study was to identify potential changes in the shoulder range of motion (ROM) of martial arts athletes and to associate these changes with years of practice, belt graduation, and scapular dyskinesis. The scapular dyskinesis was evaluated and classified according to the Kibler classification. METHO

[Chronic recurrent instability of the shoulder joint].

The article gives an analysis of results of conservative treatment of 503 patients with a primary traumatic dislocation of the shoulder. High frequency of the development of chronic instability of the shoulder joint was shown to be 31.2%. The using of arthroscopy, noninvasive methods of diagnosis (rentgenography, USI) allowed to diagnose the typical complex of morphological injuries following dislocations of the shoulder. General and local factors responsible for the development of chronic instability of the shoulder joint were established. Arthroscopy and early surgical stabilization of the j

Diagnostic accuracy of Fresno-Quebec Rules and risk factors for an associated fracture in patients presenting to the emergency department with anterior shoulder dislocation: A retrospective study.

BACKGROUND: Anterior shoulder dislocation is the most common shoulder injury in patients presenting to the emergency department (ED). Up to 25% of these injuries are fracture-dislocations. In general, the standard approach is to obtain plain radiographs before and after reduction. Fresno-Quebec Rules (FQR) are described to identify the patients who require an x-ray before reduction to reduce radiation exposure and delays in treatment. We aimed to evaluate the efficacy of clinical predictors used in the Fresno-Quebec algorithm for detecting a shoulder fracture-dislocation. METHODS: Records of

Arthroscopic rotator cuff repairs using suture anchors.

Arthroscopic rotator cuff repairs using titanium-alloy suture anchors are a new treatment option for active patients with shoulder injuries. Shoulder arthroscopy and arthroscopic repair procedures are alternative treatments to traditional open surgical procedures for Bankart lesions and rotator cuff tears. Distinct advantages of arthroscopic repair techniques include decreased patient tissue trauma and morbidity rates and shortened recovery and rehabilitation periods.

Functional Outcome following Phrenic Nerve Transfer in Brachial Plexus Injury.

BACKGROUND: Brachial plexus injuries are debilitating injuries resulting in paralyzed shoulder to global paralysis of the upper extremity. Treatment strategies have evolved over the years with nerve transfer forming the mainstay of surgical management. Phrenic nerve provides certain advantages as donor over other options but has been less preferred due to fear of pulmonary complications. In this study, we assess the functional outcomes of phrenic nerve transfer in brachial plexus injuries. MATERIALS AND METHODS: A retrospective study was performed on 18 patients operated between 2012 and 2017

Shoulder dystocia: risk factors, predictability, and preventability.

Shoulder dystocia remains an unpredictable obstetric emergency, striking fear in the hearts of obstetricians both novice and experienced. While outcomes that lead to permanent injury are rare, almost all obstetricians with enough years of practice have participated in a birth with a severe shoulder dystocia and are at least aware of cases that have resulted in significant neurologic injury or even neonatal death. This is despite many years of research trying to understand the risk factors associated with it, all in an attempt primarily to characterize when the risk is high enough to avoid vagi
