Complete heart block in COVID-19 without prior cardiac disease.

Cardiac involvement in COVID-19 is not rare but underdiagnosed. We report a 78-year-old man with COVID-19 and complete heart block, timely managed by teamwork involving internist, cardiologist, and intensivist. This case highlights the importance of involvement of the conducting system of the heart in COVID-19 that needs immediate life-saving intervention, especially in community.

Fetal goiter mimicking anomalous pulmonary venous connection.

Fetal thyroid dysfunction is a well-recognized cause of secondary cardiac disease, including arrhythmias and hydrops fetalis, but has not previously been reported to mimic structural heart disease. We describe a case of fetal goiter presenting as suspected anomalous pulmonary venous connection and highlight lessons for the obstetrician and pediatric cardiologist with regard to imaging as well as communication.

Acute neurological events in a patient with secondary polycythaemia and arteriovenous malformations.

General internists and adult cardiologist are seeing adult congenital heart disease more frequently in their clinical practices. We report the case of a polycythaemic patient with the cerebral and pulmonary arteriovenous malformations (AVMs) who presents with a transient ischaemic attack (TIA). Treatment strategies such as antiplatelet drugs and vensection may at best be ineffective and at worst detrimental to the patient.

A Multimodular AI Algorithm for Automated Assessment of Left Ventricular Function in Ischemic Heart Disease: Ejection Fraction, Wall Motion, and Regional Myocardial Segmentation.

BACKGROUND: Ischemic heart damage reduces the pumping efficiency of the heart by affecting the left ventricular ejection fraction (LVEF) and causing wall motion abnormality (WMA). In daily clinical practice, these parameters are interpreted by physicians using two dimensional transthoracic echocardiography (2D-TTE). Because 2D-TTE reports rely on visual evaluations, they are subject to experience-based limitations and exhibit low reproducibility. AIMS: To develop an artificial intelligence algorithm composed of two modules that enable automatic LVEF calculation and WMA detection for analyzing

Lung Ultrasound Versus Chest Radiography for Acute Heart Failure: Impact of Heart Failure History and Pleural Effusion.

Background/Objectives: This is the first prospective, same-day, multi-modality comparison of lung ultrasound (LUS) and chest radiography (CXR) for detecting acute heart failure (AHF) in non-critical patients with dyspnoea, examining the impact of chronic heart failure and pleural effusion, using low-dose chest CT (LDCT) as an objective comparator, and cardiologists-adjudicated AHF as reference standard. Methods: An observational study of 240 consecutive non-critical patients ≥50 years admitted with dyspnoea was conducted. Unstable AHF cases were deemed ineligible. Each modality was evaluated a…

A Deep Learning Approach for Assessment of Regional Wall Motion Abnormality From Echocardiographic Images.

OBJECTIVES: This study investigated whether a deep convolutional neural network (DCNN) could provide improved detection of regional wall motion abnormalities (RWMAs) and differentiate among groups of coronary infarction territories from conventional 2-dimensional echocardiographic images compared with that of cardiologists, sonographers, and resident readers. BACKGROUND: An effective intervention for reduction of misreading of RWMAs is needed. The hypothesis was that a DCNN trained using echocardiographic images would provide improved detection of RWMAs in the clinical setting. METHODS: A to…

Improved outcomes with early collaborative care of ambulatory heart failure patients discharged from the emergency department.

BACKGROUND: The type of outpatient physician care after an emergency department visit for heart failure may affect patients' outcomes. METHODS AND RESULTS: Using the National Ambulatory Care Reporting System, we examined the care and outcomes of heart failure patients who visited and were discharged from the emergency department in Ontario, Canada (April 2004 to March 2007). Early collaborative care by a cardiologist and primary care (PC) physician within 30 days after discharge was compared with PC alone. Care for 10 599 patients (age, 74.9±11.9 years; 50.2% male) was provided by PC alone (n

Opening Pandora's box: parental anxiety and the assessment of childhood murmurs.

BACKGROUND: Most family physicians and pediatricians recognize the induction of parental anxiety when a newly diagnosed childhood murmur is discussed with a child's parent. OBJECTIVES: To assess parental anxiety before and after consultation with a pediatric cardiologist for assessment of a childhood murmur. The study investigated the relationship between anxiety and parental understanding of murmurs, and assessed the possible role of the primary physician's initial diagnosis of the child's murmur on the anxiety level of parents. Current treatment options for cluster headache: limitations and the unmet need for better and specific treatments-a consensus article.

AIM: Treatment for cluster headache is currently based on a trial-and-error approach. The available preventive treatment is unspecific and based on few and small studies not adhering to modern standards. Therefore, the authors collaborated to discuss acute and preventive treatment in cluster headache, addressing the unmet need of safe and tolerable preventive medication from the perspectives of people with cluster headache and society, headache specialist and cardiologist. FINDINGS: The impact of cluster headache on personal life is substantial. Mean annual direct and indirect costs of cluste

Prevalence of left ventricular systolic dysfunction in a typical outpatient pacemaker cohort.

BACKGROUND: Right ventricular (RV) pacing is the standard treatment for symptomatic bradycardia. RV pacing is known to cause dyssyncrony. New treatment options like His bundle pacing enhance the focus on left ventricular dysfunction in patients with pacemakers. OBJECTIVES: The aim of this cross-sectional study was to obtain a real-life picture of the patients in a representative cohort of outpatients with permanent pacemakers. The prevalence and causes of left ventricular dysfunction (LVD) were explored. METHODS: In total, 1869 patients of a pacemaker outpatient clinic were screened for left

Differences in Cardiac Etiologies for Ischemic Stroke in Young and Middle-Aged Patients: A Single-Center Experience in Taiwan.

BACKGROUND: The cardiac etiology of acute ischemic stroke (AIS) plays an important role in young adults; therefore, complete cardiac workups and subsequent prevention methods are important for treating young AIS patients. However, the definition of a young age for AIS patients could be below 45 years old, while in some previous studies, it has been below 55 years old. It remains unclear whether cardiac workups are equally important for AIS patients in the young (the age of 20-45 years old) and middle-aged (46-55 years old) categories. MATERIALS AND METHODS: This prospective study included 103
