Hospitals prepare plans, drill staff to ensure that potential Ebola patients are identified, isolated, and managed safely.

Hospitals around the country have stepped up their efforts to train staff and implement procedures to ensure the safe identification and management of any patients with signs of Ebola virus disease (EVD). Ronald Reagan UCLA Medical Center in Los Angeles, CA, held an "Ebola preparedness exercise" to give staff an opportunity to walk through the hospital's protocol for handling a simulated patient with EVD. The University of Alabama at Birmingham (UAB) Medical Center has held similar exercises, and is now holding twice-weekly meetings of its leadership team to make sure that all new developments

Establishing an accurate and sensitive in vitro drug screening system for human adenovirus infection with human corneal cells.

Epidemic keratoconjunctivitis (EKC) is a hazardous and highly contagious disease, with the potential to cause epidemic outbreaks in hospitals and other community settings. There are currently no approved drugs for human adenovirus (HAdV), the causative agent of EKC. To establish a novel drug screening system for ocular HAdV infections, we employed CRL11516, a non-cancerous but immortalized human corneal epithelial cell line. Brincidoforvir and 3'-deoxy-3'-fluorothymidine inhibit replication of HAdV species C type 1 (C1), C2, E4, and C6 to the same extent. This alternative assay system may allo

Investing in health information management: The right people, in the right place, at the right time.

AIM: To describe the process adopted to review the academic curriculum for training health information management professionals in Nigeria. CONTEXT: Health information management professionals are responsible for managing patients' health service records and hospital information systems across health facilities in Nigeria. An assessment found many are inadequately skilled in information and communications technology (ICT) skills believed to be needed for them to play leadership roles in hospital information systems and function effectively. This was traced to a dearth of relevant ICT courses

Application of Comprehensive Artificial intelligence Retinal Expert (CARE) system: a national real-world evidence study.

BACKGROUND: Medical artificial intelligence (AI) has entered the clinical implementation phase, although real-world performance of deep-learning systems (DLSs) for screening fundus disease remains unsatisfactory. Our study aimed to train a clinically applicable DLS for fundus diseases using data derived from the real world, and externally test the model using fundus photographs collected prospectively from the settings in which the model would most likely be adopted. METHODS: In this national real-world evidence study, we trained a DLS, the Comprehensive AI Retinal Expert (CARE) system, to id

Understanding the beginning of a pandemic: China's response to the emergence of COVID-19.

China had suffered the COVID-19 outbreak from the end of 2019, SARS-CoV-2 began to spread secretly within the community and in hospitals, causing numerous citizens to be infected without knowing it. After the official confirmation of COVID-19, hospital diagnosis and treatment systems were under great pressure, and medical supplies were scarce. Then the pandemic experienced an explosive growth and quickly peaked. In order to respond to the challenge, the Chinese government quickly built hospitals, adjusted the diagnosis and treatment systems, deployed medical staff to support areas affected by

Improving performance with clinical decision support.

BACKGROUND: CADU/CIS (Clinical and Administrative Decision-support Utility and Clinical Information System) is a clinical decision-support workstation that allows large volumes of clinical information systems data to be analyzed in a timely and user-friendly fashion. CARE PROCESS MEASUREMENT: For any given disease, subgroups of patients are identified, and automated, customized "clinical pathways" are generated. For each subgroup, the best practice norms for use of test and therapies are identified. Practice style variations are then compared to outcomes to focus inquiry on decisions that sign

Controversies in the Management of Oral and Maxillofacial Infections.

The management and treatment of odontogenic infection, and its frequent extension into the head and neck, remains an important section of oral and maxillofacial surgical practice. This area of maxillofacial expertise is widely recognized by the medical community and an essential component to the hospital referral system. Although the general principles of infection management have not changed, there have been modifications in the timing of treatment sequences and treatment techniques. These modifications are influenced by the development of diagnostic methods and advances in bacterial genetics

Conducting clinical trials in a constantly changing health care environment.

Continuous change and cost containment characterize the current health care system, making conduct of clinical trials and other clinical research difficult. Identification, accrual, and follow-up of patients who move between health care environments such as hospitals, nursing homes, schools and the home is particularly challenging. This article describes a circuit rider approach to patient identification and follow-up that was established by the Communication Sciences and Disorders Clinical Trials Research Group. It also gives suggestions for design of clinical trials in a constantly changing

Innovation in medical diagnosis--the Scandinavian curiosity.

Fine-needle aspiration biopsy is a major diagnostic tool in Scandinavia, where 8000 such biopsies may be done in a large metropolitan hospital each year. The procedure is rapid, inexpensive, and technologically simple, yet it has found only limited, albeit increasing, acceptance in medical practice outside Scandinavia. Although the differences between Scandinavian and other medical systems may explain why the technique is not used more widely, there seems to be an underlying reluctance among medical communities to accept subjective types of innovation compared with objective innovation.

Prevalence of Sepsis and Possible Severe Bacterial Infection among Neonates in Nepal.

Nepal lacks adequate data on the prevalence of sepsis and Possible Severe Bacterial Infection (PSBI) among neonates. Thus, this systematic review was designed to estimate the prevalence of neonatal sepsis and PSBI status in Nepal. We searched PubMed and Nepal Journal Online for relevant studies on PSBI and neonatal sepsis published from 2006 to 2016. The eligibility criteria included those studies done in Nepal, evaluating the prevalence of PSBI/neonatal sepsis with denominators as the population at risk that is either total live births or total cases evaluated. Altogether, four studies met th

Long-term follow-up of rural involuntary clients.

A cohort of 72 persons who had entered a rural Oregon county's involuntary treatment system in 1979 through 1982 was followed for six years. While schizophrenia was the most frequent diagnosis, several other conditions were represented including adjustment disorders, organic mental disorders, and substance abuse. The majority (55%) of persons with organic mental disorders died as did 12% of the individuals with schizophrenia. The overall mortality rate was three times the expected figure (p = .002). During the follow-up period, only 39% of the initial cohort received treatment from a community

Modelling access to renal transplantation waiting list in a French healthcare network using a Bayesian method.

Evaluation of adult candidates for kidney transplantation diverges from one centre to another. Our purpose was to assess the suitability of Bayesian method for describing the factors associated to registration on the waiting list in a French healthcare network. We have found no published paper using Bayesian method in this domain. Eight hundred and nine patients starting renal replacement therapy were included in the analysis. The data were extracted from the information system of the healthcare network. We performed conventional statistical analysis and data mining analysis using mainly Bayes

Improving Medication Reconciliation at Discharge: A Cross-Sectional Survey.

BACKGROUND: Medication discrepancies remain a persistent patient safety concern despite decades of quality improvement efforts. Research indicates that about half of hospitalized adults experience at least one such discrepancy after discharge, often stemming from unclear clinician roles and poor communication among nurses, physicians, and pharmacists. Understanding nurses' perspectives is critical to designing interventions that will improve medication reconciliation and enhance patient safety at discharge. PURPOSE: This study aimed to explore RNs' perspectives of medication reconciliation du

Extrinsic risk factors for pneumonia in the patient at high risk of infection.

Most nosocomial pathogens cause pneumonia through the following sequence: transit to the patient on the hands of medical personnel or perhaps in food, proliferation in the nasopharynx, and subsequent pulmonary aspiration. There are three exceptional pathogens, each of particular concern as a cause of pneumonia in the immunocompromised patient, which follow atypical routes. Important recent advances in understanding these routes permit more rational preventive measures. This report examines the evidentiary basis for the following pathophysiolgic propositions about these three pathogens: Aspergi
