OBJECTIVE: The past few years have seen a rapid advancement in the management of keratoconus (KC). However, there is no prescribed standard of care for the management of KC. This study evaluated the prevailing practice patterns among Indian ophthalmologists in the diagnosis and treatment of KC via an online survey.
METHODS: This was a survey-based cross-sectional study in which a questionnaire (Supplement 1) was created. Questions pertaining to the practicing experience, setting of practice, and training background were asked in addition to the investigations done and decision making in KC ma…

BACKGROUND: Keratoconus is a corneal ectatic disease caused by stromal thinning leading to astigmatism and progressive loss of vision. Loss of the keratocytes and excessive degradation of collagen fibres by matrix metalloproteinases are the molecular signatures of the disease. Despite several limitations, corneal collagen cross-linking and keratoplasty are the most widely used treatment options for keratoconus. In the pursuit of alternative treatment modalities, clinician scientists have explored cell therapy paradigms for treating the condition.
METHODS: Articles pertaining to keratoconus ce…

A 24-year-old male was treated with transepithelial photorefractive keratectomy for myopia in both eyes 5 years ago. He had allergic conjunctivitis for 3 years. The night vision of both eyes was decreased for 2 years. The clinical diagnosis was keratoconus binoculus. Therefore, corneal collagen cross-linking was performed in both eyes, and the follow-up was 6 months. Confocal microscopy showed that the cross-linking of riboflavin and fiber amino was successful, and the keratoconus did not progress during the follow-up. Although there have been few case reports of keratoconus after transepithel…

PURPOSE: To evaluate the visual, refractive, and topographic outcomes of accelerated corneal collagen cross-linking (CXL) in the pediatric age group.
METHODS: In this retrospective case series study, 89 eyes of 56 patients with progressive keratoconus (KCN) who were under or equal to 18 years old at the time of surgery were included. All patients underwent accelerated corneal CXL. A thorough baseline and follow-up ophthalmic examination including uncorrected distance visual acuity, best corrected visual acuity (BCVA), slit-lamp and fundus examination, and corneal tomography by Scheimpflg came…

PURPOSE: To describe the risk factors, management, and outcome of delayed Descemet membrane (DM) detachment after penetrating keratoplasty (PK) for keratoconus.
METHODS: We report 7 eyes from 6 cases and combine these data with 7 previous case reports identified by a search of PubMed.
RESULTS: DM detachment occurred at a median of 25 years (range, 7-33 years) after PK. One individual had bilateral detachments. There was typically a mild ocular discomfort accompanied in some cases by a rapid onset of visual blur. Cases were often treated for allograft rejection before a DM detachment was susp…

PURPOSE: To evaluate the pattern of retinal thickness distribution in patients with keratoconus (KCN) and its correlation with disease severity.
METHODS: For this cross-sectional cohort study, the study subjects with documented keratoconus and normal eyes were prospectively enrolled. All subjects had anterior segment (Pentacam HR) and posterior segment (Spectralis) imaging. Posterior segment imaging by optical coherence tomography included the posterior pole asymmetry analysis map. Data were analyzed with multiple linear regression models and correlation tests to examine the mean and variance…

PURPOSE: To investigate chronological changes in patients with very asymmetric keratoconus (KC) by Scheimpflug-based corneal tomographic evaluation.
METHODS: We retrospectively reviewed the clinical records of 382 patients with KC attending a university hospital. Patients who were diagnosed with KC in one eye and forme fruste keratoconus in the other eye at the last visit and followed up with Scheimpflug-based tomographic corneal imaging were included. The anterior radius of curvature (ARC) and posterior radius of curvature (PRC) obtained at a 3.0-mm optical zone and the thinnest pachymetry r…

SIGNIFICANCE: Reliable intraocular pressure (IOP) measurement after intrastromal corneal ring segments (ICRS) implantation is a challenge because of altered corneal morphology. In this study, IOP is measured with four tonometers, compared with Goldmann applanation tonometry (GAT) values and the influence of corneal parameters is established.
PURPOSE: This study compares IOP measurements made using different tonometers in patients implanted with ICRS and assesses the effects of central corneal thickness (CCT), corneal curvature, and corneal astigmatism on the IOP measurements obtained.

Keratoconus is a common corneal ectasia that leads to progressive visual impairment. Numerous studies have shown abnormal protein expression patterns in keratoconic corneas. However, the specific mechanisms causing this disease remain ambiguous. This review aims to provide an update on morphological studies of the keratoconic cornea, relate these early studies with current findings from proteomic, biochemical and cell culture studies and to postulate possible pathogenic pathways.

: Collagen cross-linking (CXL) is a procedure that primarily aims to increase corneal stiffness. Although used for a variety of conditions, it is most commonly applied to the treatment of keratoconus. Collagen cross-linking involves irradiation of the cornea with ultraviolet A (UVA) irradiation after it has been soaked with riboflavin (vitamin B), a photosensitizer. In conventional treatment, based on the Dresden protocol, a minimum corneal thickness threshold of 400 μm is recommended and UVA (370 nm) irradiation of 3 mW/cm irradiance is applied for 30 min, resulting in a cumulative dose of 5.…

PURPOSE: The aim of this study was to report 2 cases with topographic keratoconus progression despite stability for a long-term period after corneal collagen crosslinking (CXL).
METHODS: This is a case report.
RESULTS: Two patients with progressive keratoconus underwent CXL treatment. Four and 5 years postoperatively, a topographic examination revealed an increase in the keratometric values indicating keratoconus progression.
CONCLUSIONS: Post-CXL, topographic keratoconus progression may occur several years after, despite stability for a long-term period.

BACKGROUND: Urrets-Zavalia syndrome (UZS) consists of a fixed dilated pupil associated with iris atrophy. It is a poorly understood complication following penetrating keratoplasty (PKP) for keratoconus (KC). In this work, we aim to establish the incidence, visual outcomes, and an understanding of UZS.
METHODS: This was a retrospective single-center study in a tertiary eye service in the United Kingdom of consecutive patients with UZS following PKP for KC in a 10-year period. Post-operative complications, including raised intraocular pressure (IOP), were recorded. UZS patients and age-matched …

PURPOSE: To ascertain the level and speed of visual recovery after penetrating keratoplasty for keratoconus.
METHOD: A retrospective review was performed of 100 consecutive cases of penetrating keratoplasty for keratoconus, performed between 1999 and 2005. Review assessed visual function and the speed at which patients achieved a functional best corrected visual acuity (BCVA) of > or = 6/12 either with glasses or phoropter. Analysis of visual, refractive and keratometric results were made on 76 eyes that had reached 6 months after suture removal. Intraoperative and postoperative complications…

PURPOSE: To report 2 cases of corneal hydrops occurring in eyes with previously healthy corneal grafts.
METHODS: Retrospective case series. A report of 2 presenting cases. Both patients underwent a full ophthalmic clinical examination and anterior segment optical coherence tomography (OCT).
RESULTS: Two patients with longstanding corneal grafts for keratoconus presented with an acute painful red eye and reduced vision. They were found to have acute hydrops with breaks in and detachments of Descemet membrane.
CONCLUSIONS: Keratoconic patients with longstanding corneal grafts who present acut…
