BACKGROUND: Medial meniscus posterior root tear can result in medial meniscus extrusion. However, the severity of medial meniscus extrusion is different in each root tear patient. The purpose of this study was to identify the factors that contribute to the severity of medial meniscus extrusion with medial meniscus posterior root tear, such as duration of disease, the degree of arthritis-chondral wear, subchondral edema, osteophyte size, and Kellgren-Lawrence (K/L) grade-and mechanical alignment for appropriate treatment method.
METHODS: From January 2009 to August 2014, we retrospectively ana…

BACKGROUND: Meniscal lesions represent one of the most common intra-articular knee injuries. Meniscus replacement devices are needed to restore load distribution and knee stability after meniscectomy. Fixation of these devices is crucial to the generation of hoop stresses and the distribution of loads in the joint.
PURPOSE: To evaluate 2 different fixation techniques (suture endobutton and interference screw) for implantation of a novel meniscus device.
STUDY DESIGN: Controlled laboratory study.
METHODS: In 7 human cadaveric knees (aged 17-61 years), 1 anterior and 2 potential posterior tun…

UNLABELLED: Meniscus allograft transplantation (MAT) is used to treat patients with knee pain after total or subtotal meniscectomy. The graft can be inserted during open or arthroscopic surgery. The objectives are anatomic horn positioning and strong fixation to the bone and capsule of an appropriately sized graft.
HYPOTHESIS: Arthroscopic MAT with trans-tibial bone fixation ensures better mid-term functional outcomes and limits allograft extrusion.
PATIENTS AND METHODS: We conducted a retrospective single-centre study of 23 consecutive patients who underwent MAT between 2001 and 2010. Among…

Avulsion fractures of the posterior horn of the medial meniscus are uncommon and must be differentiated from a loose body. The authors present a displaced avulsion fracture of the medial meniscus posterior horn through the intercondylar notch into the anteromedial compartment of the knee, which was treated by arthroscopic reduction and internal fixation using pull-out suture technique.

Osteochondritis dissecans (OCD) of the lateral femoral condyle sometimes occurs with a discoid lateral meniscus. Recently, it was reported that OCD of the lateral femoral condyle occurred after total removal of the lateral meniscus. We report the case of a 12-year-old boy with bilateral OCD of the lateral femoral condyle following bilateral total removal for discoid lateral meniscus. Valgus deviation of the knee after total removal and increased sporting activity might have concentrated excessive stress on the lateral condyles in the standing position. As a result, bilateral OCD might have occ…

CASE: A 21-year-old woman presented with chronic, nontraumatic left knee pain and was diagnosed with a complete-type medial discoid meniscus lacking anterior horn osseous insertion, but connected to the posterior horn of the lateral meniscus by an oblique meniscomeniscal ligament as seen on MRI and confirmed arthroscopically.
CONCLUSION: This represents a previously unreported anatomical variant that broadens the morphological spectrum of medial discoid meniscus. Multiplanar MRI is critical for its accurate identification.

BACKGROUND: Posterior medial meniscus root tears are commonly treated with a transtibial pull-out (TO) repair, but significant postoperative meniscal extrusion has been reported. Recently, knotless adjustable suture anchor fixation has been introduced to treat root tears. This anchor-based fixation demonstrates less cyclic displacement than a TO repair. However, it is unknown how meniscal extrusion and contact mechanics compare between these 2 repair techniques when the meniscus endures cyclic tibiofemoral loading.
HYPOTHESIS: Suture anchor repair will demonstrate reduced meniscal extrusion a…

BACKGROUND: Previous biomechanical studies evaluating medial meniscus posterior root tears (MMPRTs) are limited to low loads applied at specified loading angles, which cannot capture the effects of MMPRTs during the multidirectional forces and moments placed across the knee during physiological activities.
PURPOSE: To quantify the effects of MMPRTs on knee joint contact mechanics during simulated gait.
STUDY DESIGN: Controlled laboratory study.
METHODS: Six human cadaveric knees were mounted on a robotic simulator programmed to apply dynamic forces, moments, and flexion angles to mimic leve…

Varus alignment of the knee joint (varus >5°) is known as a poor prognostic factor for medial meniscus root repair, and alignment correction is recommended in patients with varus deformity and medial meniscus root tears. However, simultaneous medial meniscus pull-out repair and high tibial osteotomy are technically demanding procedures due to the long surgical time, poor visualization, and breaking of the pull-out sutures during high tibial osteotomy procedures. In the present Technical Note, we will introduce a surgical method to perform 2 procedures simultaneously without technical difficult…
