Cardiorespiratory Sensors and Their Implications for Out-of-Hospital Cardiac Arrest Detection: A Systematic Review.

Out-of-hospital cardiac arrest (OHCA) is a major health problem, with a poor survival rate of 2-11%. For the roughly 75% of OHCAs that are unwitnessed, survival is approximately 2-4.4%, as there are no bystanders present to provide life-saving interventions and alert Emergency Medical Services. Sensor technologies may reduce the number of unwitnessed OHCAs through automated detection of OHCA-associated physiological changes. However, no technologies are widely available for OHCA detection. This review identifies research and commercial technologies developed for cardiopulmonary monitoring that

A quantitative model to ensure capacity sufficient for timely access to care in a remote patient monitoring program.

INTRODUCTION: Algorithm-enabled remote patient monitoring (RPM) programs pose novel operational challenges. For clinics developing and deploying such programs, no standardized model is available to ensure capacity sufficient for timely access to care. We developed a flexible model and interactive dashboard of capacity planning for whole-population RPM-based care for T1D. METHODS: Data were gathered from a weekly RPM program for 277 paediatric patients with T1D at a paediatric academic medical centre. Through the analysis of 2 years of observational operational data and iterative interviews wi

Phantom Study of Arterial Localization using Tactile Sensor Array and a Normal Vs. Shear Pulse Pressure Propagation Method.

OBJECTIVE: Locating the radial artery reliably is a key challenge in reducing patient risks from complications in Trans-Radial Access, which is an important clinical method for catheterization, cardiac monitoring, and neuroendovascular procedures. New tactile sensing technology is being developed to bridge the skill, cost, and performance gap between ultrasonic needle guidance, and manual palpation, for use in developing countries. This paper further develops tactile artery localization with a novel algorithm for arterial localization based on the properties of a curved tactile sensor array.

The intracranial compartmental syndrome: a proposed model for acute brain injury monitoring and management.

For decades, one of the main targets in the management of severe acute brain injury (ABI) has been intracranial hypertension (IH) control. However, the determination of IH has suffered variations in its thresholds over time without clear evidence for it. Meanwhile, progress in the understanding of intracranial content (brain, blood and cerebrospinal fluid) dynamics and recent development in monitoring techniques suggest that targeting intracranial compliance (ICC) could be a more reliable approach rather than guiding actions by predetermined intracranial pressure values. It is known that ICC i

Patient Safety in Canadian Epilepsy Monitoring Units: A Survey of Current Practices.

BACKGROUND: Guidelines on epilepsy monitoring unit (EMU) standards have been recently published. We aimed to survey Canadian EMUs to describe the landscape of safety practices and compare these to the recommendations from the new guidelines. METHODS: A 34-item survey was created by compiling questions on EMU structure, patient monitoring, equipment, personnel, standardized protocol use, and use of injury prevention tools. The questionnaire was distributed online to 24 Canadian hospital centers performing video-EEG monitoring (VEM) in EMUs. Responses were tabulated and descriptively summarized

Monitoring blood pressure and cardiac function without positioning via a deep learning-assisted strain sensor array.

Continuous and reliable monitoring of blood pressure and cardiac function is of great importance for diagnosing and preventing cardiovascular diseases. However, existing cardiovascular monitoring approaches are bulky and costly, limiting their wide applications for early diagnosis. Here, we developed an intelligent blood pressure and cardiac function monitoring system based on a conformal and flexible strain sensor array and deep learning neural networks. The sensor has a variety of advantages, including high sensitivity, high linearity, fast response and recovery, and high isotropy. Experimen

Integration of a body sensor network of wearable devices for cardio-respiratory monitoring.

Wearable devices represent a non-invasive tool to monitor cardio-respiratory parameters. This paper presents a telemedicine platform constituted of four wireless units. Three wearable inertial measurement units monitor the respiratory-related excursions of the thorax and of the abdomen with respect to a reference unit (positioned on the lower back), through which respiratory rate and normalized tidal volume are extracted. The fourth unit is a reflectance wrist-worn pulse oximeter. To validate the system, 20 healthy volunteers (12 men) participated in a protocol designed to induce desaturation

Precordial Vibrations: A Review of Wearable Systems, Signal Processing Techniques, and Main Applications.

Recently, the ever-growing interest in the continuous monitoring of heart function in out-of-laboratory settings for an early diagnosis of cardiovascular diseases has led to the investigation of innovative methods for cardiac monitoring. Among others, wearables recording seismic waves induced on the chest surface by the mechanical activity of the heart are becoming popular. For what concerns wearable-based methods, cardiac vibrations can be recorded from the thorax in the form of acceleration, angular velocity, and/or displacement by means of accelerometers, gyroscopes, and fiber optic sensors

Continuous wireless pre- and postoperative vital sign monitoring reveal new, severe desaturations after vascular surgery.

OBJECTIVES: Postoperative deviating physiologic values (vital signs) may represent postoperative stress or emerging complications. But they can also reflect chronic preoperative values. Distinguishing between the two circumstances may influence the utility of using vital signs in patient monitoring. Thus, we aimed to describe the occurrence of vital sign deviations before and after major vascular surgery, hypothesising that preoperative vital sign deviations were longer in duration postoperatively. METHODS: In this prospective observational study, arterial vascular patients were continuously

Critical Care Nurses' Perceptions of Clinical Alarm Management on Nursing Practice.

The alarm management of physiological monitoring systems is a key responsibility of critical care nurses. However, the high numbers of false and nonactionable (true but clinically irrelevant) alarms cause distractions to healthcare professionals, interruptions to nursing workflow, and ignoring of crucial tasks. Therefore, understanding how nurses manage large amounts of alarms in their daily work could provide a direction to design interventions to prevent adverse patient care effects. A qualitative design with focus group interviews was conducted with 37 nurses in Taiwan. Content analysis was

Seizure-related differences in biosignal 24-h modulation patterns.

A seizure likelihood biomarker could improve seizure monitoring and facilitate adjustment of treatments based on seizure risk. Here, we tested differences in patient-specific 24-h-modulation patterns of electrodermal activity (EDA), peripheral body temperature (TEMP), and heart rate (HR) between patients with and without seizures. We enrolled patients who underwent continuous video-EEG monitoring at Boston Children's Hospital to wear a biosensor. We divided patients into two groups: those with no seizures and those with at least one seizure during the recording period. We assessed the 24-h mod

As a chronic disease, cervical spondylosis is prone to recurrent attacks as we age if we do not pay attention to protection, which can easily lead to symptoms such as osteophytes and herniated discs. In the early stage of cervical spondylosis, it is possible to alleviate the disease and prevent its aggravation by improving poor cervical posture and increasing cervical activities. This article analyzes the current situation and medical prospect of smart wearable devices with the prevention and treatment of cervical spondylosis in white-collar people as the starting point and smart wearable devi
