Endoscopic intranasal reduction of the orbit in isolated blowout fractures.

CONCLUSION: Endoscopic intranasal reduction of the orbital floor with a detachable temporary balloon, or of the medial orbital wall with a silastic sheet and Merocel packing, provided good functional results and definite advantages. We suggest that these techniques are another surgical alternative for isolated orbital floor or medial wall blowout fractures that do not accompany any associated fractures of the orbital rim. OBJECTIVES: Extended applications of endoscopic sinus surgery have reported endoscopic intranasal reduction or reconstruction of the orbital wall with good functional and co…

Expansion of the socket and orbit for congenital clinical anophthalmia.

BACKGROUND: Congenital clinical anophthalmia requires fitting an artificial eye, achieving a pleasing aspect of the palpebral apparatus, and stimulating growth of the orbit, as well as of the entire midface. METHODS: Out of 25 patients with clinical anophthalmia, 22 were treated according to a new protocol (first described by Wiese et al. in 1999) and followed for up to 6 years. In 10 patients, anophthalmia was bilateral, in nine patients, it was on the right side only and in three it was on the left side only. Treatment consisted of implantation of an osmotic expander to first stimulate grow…

Typical genomic imbalances in primary MALT lymphoma of the orbit.

Primary orbital non-Hodgkin lymphoma is a mucosa-associated lymphoid tissue (MALT)-type extranodal marginal zone lymphoma. Little information is available on its genome as conventional cytogenetics is limited by scarce biopsy material, while fluorescence in situ hybridization (FISH) explores only selected regions. Comparative genomic hybridization (CGH) performs full genomic analysis and is applicable to different sources of DNA, such as fresh and frozen cells, as well as paraffin-embedded tissues. In this study, CGH was used to analyse primary MALT lymphoma of the orbit. Aneuploidy was identi…

[Lymphoproliferative diseases of the orbit and appendages of the eye].

The results are available of clinical, morphological, cytological and immunological investigations of orbital lymphomas and lymphomas of appendages of the eye. Malignant lymphoma was detected in 17 patients and reactive lymphoid hyperplasia in 3 patients. All the malignant lymphomas had B-cell phenotype. By cell composition, MALT-lymphoma is more polymorphic than lymphoma from mantle zone cells and lymphoma from small lymphocytes and centro follicular lymphoma. The conjunctive is affected primarily with lymphoma from marginal zone cells or mantle zone cells. As a rule, this is a primary local …

Use of the preseptal transconjunctival approach in orbit reconstruction surgery.

PURPOSE: This report evaluated the advantages of the preseptal transconjunctival approach in reconstruction of the orbit. PATIENTS AND METHODS: Ninety-nine preseptal incisions were used in 80 patients for different indications (blowout fracture, complex zygoma fracture, enophthalmos correction, midface hypoplasia, secondary incision). All operative procedures were performed without an additional lateral canthotomy. The infraorbital rim was stabilized with miniplates or microplates. RESULTS: There was no ectropion or entropion in any patients. Complications included 1 laceration of the tarsal…

Magnetic resonance imaging of the orbit: Basic principles and anatomy.

This review article gives a basic introduction to the technical principles of magnetic resonance imaging for ophthalmologists and orbital surgeons and describes the anatomical structures that can be identified on high-resolution magnetic resonance images of the orbit. The meaning of general imaging parameters, specific weighting of the images, and the use of contrast agents, fat suppression techniques and surface coils is explained. Possible artifacts are also described. The advantages and disadvantages of MRI in comparison with computed tomography (CT) are discussed in order to give recommend…

Peroxynitrite formation in the orbit of diabetics with rhinocerebral mucormycosis.

OBJECTIVE: To evaluate whether an intact respiratory burst exists within the orbit of diabetics with rhinocerebral mucormycosis. METHODS: Immunohistochemical detection of nitrotyrosine in the orbital tissue of diabetics requiring exenteration due to rhinocerebral mucormycosis. Nitrotyrosine is the stable product of the nitration of tyrosine residues by peroxynitrite. Peroxynitrite is a potent oxidant produced by the combination of superoxide and nitric oxide during the respiratory burst. RESULTS: Four specimens were analyzed. All showed focal areas of specific staining against nitrotyrosine …

Rhabdomyosarcoma of the orbit. Evaluation with MR imaging and CT.

Rhabdomyosarcoma is the most common primary orbital malignancy of childhood. It can present insidiously, mimicking other (benign) processes clinically and radiographically. CT and MR imaging are crucial in the diagnostic evaluation, treatment planning, and follow-up monitoring of the disease. Such imaging, especially when contrast is used, can accurately detect and state the extent of tumor involvement.

[Adenocarcinoma of the meibomian gland with invasion of the orbit].

Adenocarcinoma of the meibomian glands is a tumor with atypical glandular features, that appear initially as a chalazion formation and then invades the neighbouring tissues. The patient, a 63 years old woman, presents a chalazion at the level of the left lower eyelid, which, after developing for a year, stars bleeding and infiltrates the ground of the inferior sac and the orbit. The tumoral formation makes a common body with the eyeball and the cornea, which, lacking protection, becomes opaque. Removal of the ulcerous eyelids is performed, with partial exenteration of the orbit. Anatomopatholo…

Inadequately irradiated solitary extramedullary plasmacytoma of the orbit requiring exenteration.

PURPOSE: To emphasize that solitary orbital extramedullary plasmacytoma, a rare tumor, is not invariably radiosensitive. METHOD: We examined a patient who had undergone empirical irradiation that failed to control an orbital tumor. RESULTS: After a second biopsy had secured the diagnosis of a solitary extramedullary plasmacytoma, further irradiation proved ineffective and exenteration was required. The patient was free of recurrence one year after exenteration and had satisfactory cosmesis with a prosthesis. CONCLUSION: A histologic diagnosis is essential before treatment is commenced so th…

Aneurysmal bone cyst of the orbit associated with fibrous dysplasia.

Aneurysmal bone cyst is a rare but recognized osseous lesion of the orbit. We report the presentation, treatment, and postoperative course of an orbital aneurysmal bone cyst in a patient with McCune-Albright syndrome. To our knowledge, this report represents the first description of aneurysmal bone cyst of the orbit in a patient with fibrous dysplasia. Although visual loss in patients with fibrous dysplasia is most commonly due to narrowing of the optic canal, aneurysmal bone cyst should be considered in a patient with fibrous dysplasia who presents with decreasing vision in the face of a rapi…

Endoscopically controlled surgical decompression of the orbit in malignant ophthalmopathy.

From May 1988, an endoscopically controlled endonasal orbital decompression was performed in 17 patients with malignant ophthalmopathy. Indications were exclusively acute loss of visual acuity or visual field defects, when medical and radiation therapy had failed. The new minimal invasive technique proceeds in three steps. First, an endonasal, endoscopically controlled ethmoidectomy with resection of the middle turbinate is done and the medial wall of the maxillary sinus is widely opened. Second, the medial and inferior orbital walls are removed, preserving the infraorbital nerve. In the last …

Involvement of the eye and orbit in neurofibromatosis type 1.

11 individuals were diagnosed to have neurofibromatosis type 1 and were examined for evidence of any ophthalmic lesions. Lisch nodules were the commonest manifestation of the disease and were present in 73% of all the patients (88% of those aged 16 years or more). 55% of the cases showed presence of neurofibroma on the lids. Other findings were optic glioma, unilateral sphenoid dysplasia with enlarged orbit, medullated nerve fibers and prominent corneal nerves with an incidence of 9% each.

Exuberant epibulbar tumor penetrating into the orbit in xeroderma pigmentosum.

The clinical and histopathological findings are described in a 10-year-old girl with xeroderma pigmentosum and multiple ophthalmic complications, including the eyelids, conjunctiva, orbit, cornea and iris; some of these complications took an unusual form. Histological examination following right orbital exenteration revealed an epibulbar squamous cell carcinoma penetrating the orbit and building a noose all around the ocular globe with subsequent strangulation. Moreover, a palpebral squamous cell carcinoma developed-independently from the right lower lid and, unconnected with the epibulbar tum…

Congenital unilateral absence of orbit with anophthalmos and partial arhinia.

Congenital unilateral absence of orbit is a very rare clinical entity. An 18 month old female child with an unilateral absence of orbit with clinical anophthalmos and partial arhinia is presented. X-ray of the skull revealed an absence of the bony orbit on the left side. Unlike previously described cases of absence of the orbit, this maldevelopment is unilateral and present in an otherwise normal thriving child.
