Development of a Preliminary Question Prompt List as a Communication Tool for Adults With Achalasia: A Modified Delphi Study.

BACKGROUND: Question prompt lists (QPLs) are structured sets of disease-specific questions that enhance patient-physician communication by encouraging patients to ask questions during consultations. AIM: The aim of this study was to develop a preliminary achalasia-specific QPL created by esophageal experts. METHODS: The QPL content was derived through a modified Delphi method consisting of 2 rounds. In round 1, experts provided 5 answers to the prompts "What general questions should patients ask when given a new diagnosis of achalasia" and "What questions do I not hear patients asking, but g

Exploring the Engagement of Racial and Ethnic Minorities in HIV Treatment and Vaccine Clinical Trials: A Scoping Review of Literature and Implications for Future Research.

HIV disproportionately impacts US racial and ethnic minorities but they participate in treatment and vaccine clinical trials at a lower rate than whites. To summarize barriers and facilitators to this participation we conducted a scoping review of the literature guided by the Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) guidelines. Studies published from January 2007 and September 2019 were reviewed. Thirty-one articles were identified from an initial pool of 325 records using three coders. All records were then assessed for barriers and facilitators and summarize

Can the Routine Use of Patient-Reported Outcome Measures Improve the Delivery of Person-Centered Diabetes Care? A Review of Recent Developments and a Case Study.

PURPOSE OF REVIEW: In recent years, the recommendation for and use of patient-reported outcome measures (PROMs) in routine diabetes care has … dividualization of self-management support and goal-setting, and broader benefits related to active patient participation and person-centred diabetes care. We identify multiple intervention, individual, and care setting characteristics, which influence acceptability, feasibility, implementation, and effectiveness of PROMs in routine care. Recent clinical PROM studies h …

Self-management programmes for COPD: moving forward.

Self-management is of increasing importance in chronic obstructive pulmonary disease (COPD) management. However, there is confusion over what processes are involved, how the value of self-management should be determined, and about the research priorities. To gain more insight into and agreement about the content of programmes, outcomes, and future directions of COPD self-management, a group of interested researchers and physicians, all of whom had previously published on this subject and who had previously collaborated on other projects, convened a workshop. This article summarises their initi

Which patients should be offered caesarean section?

Caesarean section (CS) rates have steadily risen from 10% of all deliveries in the 1980s to a current figure of around 23.8%. Approximately 75% of CS are emergency procedures and only 25% are elective planned deliveries. When deciding whether to offer CS, it is important to consider the psychological implications for the patient as well as the physical and mental sequelae in future pregnancies. Clinicians should provide pregnant women with evidence-based information and support. Information should include details about the true indication(s) for the CS and what it implies, including its risks

Health literacy and patient empowerment in health communication: the importance of separating conjoined twins.

OBJECTIVE: Patient empowerment and health literacy have both been studied empirically, but they have hardly ever been explicitly linked. METHODS: Pertinent literature from the development of both concepts was studied, drawing not only on health care literature, but also on management research. RESULTS: This article argues that it is important to recognize that the concepts are distinct, both conceptually and empirically. At the same time, the impacts of health literacy and patient empowerment are deeply intertwined. High literacy does not necessarily entail empowerment and vice versa, and mi

Recruitment of asthmatic ethnic minorities into a methacholine research study: factors influencing participation.

BACKGROUND: How to recruit minority participants into research studies has been an issue since 1993, when NIH funding guidelines required minorities to be included as research participants. OBJECTIVE: The purpose of this analysis was to determine what factors affected recruitment of asthmatic minorities into a large bronchoconstrictor study involving African-Americans, Hispanics/Mexican Americans, Asian/Pacific Islanders, and whites with mild asthma (forced expiratory volume in the first second of expiration > or = 70%). METHODS: Ethnic minorities were recruited for 3 years. Recruitment stra

Patient resources in the therapeutic education of haemophiliacs in France: their skills and roles as defined by consensus of a working group.

The activities of 'expert patients' or 'patient tutors', who help educate their peers, are gaining recognition in the health care system. This study investigates the role played by such patients in therapeutic education programmes organized by caregivers to validate the role of patients in implementing the therapeutic education of haemophilic patients and to define the skills required for such activities. This study employs the consensus methodology recommended by France's National Authority for Health. The working group includes seven caregivers from Hemophiliac Treatment Centers (HTCs) and t

Decision-Making And Selection Bias in Four Observational Studies on Duchenne and Becker Muscular Dystrophy.

BACKGROUND: Natural history data are essential for trial design in Duchenne (DMD) and Becker muscular dystrophy (BMD), but recruitment for observational studies can be challenging. OBJECTIVE: We reviewed reasons why patients or caregivers declined participation, and compared characteristics of participants and non-participants to assess possible selection bias in four observational studies, three on DMD and one on BMD. METHODS: Three pediatric DMD studies focused on cross-sectional cognitive function and brain MRI (DMDbrain, n = 35 and DMDperfusion, n = 12), and on longitudinal upper extremi

Embedding Equity, Transparency, and Impact Through Patient and Public Involvement: Insights From the HANDCLAP Study.

INTRODUCTION: Patient and public involvement and engagement (PPIE) in research refers to studies conducted with or by members of the public, rather than for them. The Health Inequalities Assessment Toolkit (HIAT) aims to integrate an intersectional equity perspective into research and considers how people with real-life experience can contribute to this process. The Guidance for Reporting Involvement of Patients and the Public (GRIPP2) is an international tool that standardises the reporting of PPIE in healthcare projects. There is growing interest in assessing the impact of public involvement

Patient and Public Involvement in Research Evaluating Integrated Care for People Experiencing Homelessness: Findings From the PHOENIx Community Pharmacy Pilot Randomised-Controlled Trial.

INTRODUCTION: There is a paucity of research on and a limited understanding of patient and public involvement (PPI) in the context of research in homelessness and, in particular, direct involvement of people with lived and living experience of homelessness (PEH) as expert advisors. We aim to report on outcomes and reflections from lived experience advisory panel (LEAP) meetings and PPI activities, held throughout the study lifecycle of a pilot randomised-controlled trial (RCT) focused on evaluating integrated health and practical support for PEH. METHODS: Community Pharmacy Homeless Outreach

[Unexpected good news after major surgery: a lesson in communication].

When dealing with choices concerning life-threatening or major surgery shared decision making, continuous tuning in with colleagues and monitoring direction is a necessity. The gastroenterologist detected a pancreatic tumour in a 53-year-old man. The patient was told that this tumour was almost surely malignant and that he would die within a year. A pylorus-preserving pancreatoduodenectomy was advised and performed. However, the final outcome was an IgG4-mediated autoimmune pancreatitis. The patient felt betrayed by the doctors. Crucial episodes of communication were analysed and we concluded

Patient and public involvement (PPI) reporting in maternal and neonatal clinical trials: an exploratory review.

BACKGROUND: Patient and public involvement (PPI) is increasingly a research priority encouraged by health research funders. It is difficult to know, however, how prevalent PPI is within research. The aim of this review was to gain a contemporary understanding of the frequency and types of PPI being reported in maternal and neonatal clinical trial reports, and if an increase in PPI reporting was evident over time. METHODS: An exploratory review of maternal and neonatal trial reports published in nine healthcare journals between 2017 and 2022 was undertaken. A search was conducted for eligible

Development of a core outcome set for the evaluation of shared decision-making interventions in healthcare: a study protocol.

INTRODUCTION: Shared decision-making (SDM) is a process whereby patients are supported to reach decisions about their healthcare in collaboration with healthcare professionals. Despite decades of SDM research, the impact of using SDM interventions (eg, patient decision aids, decision coaching, question prompt lists, training and feedback or service changes) on clinical and health service outcomes remains uncertain. This is partly because synthesis of the existing literature is hindered by substantial heterogeneity in the evaluation of interventions to facilitate SDM. A core outcome set (COS) i

Engaging with patients with diabetes: the role of social media in low-income healthcare organisations.

BACKGROUND: Type 2 diabetes is the fastest-growing global health concern, and its global prevalence is projected to affect 643 million individuals by 2030. Social media platforms, like Facebook, have become crucial channels for healthcare organisations to engage with the public to promote prevention and disease management, especially in low-resource settings like Honduras. This study aims to perform a retrospective analysis of Honduran healthcare organisations' Facebook posts to understand how effectively they engage diabetes-related content with their followers. METHODS: The top 10 followed
