Open surgical treatment for chronic midportion Achilles tendinopathy: faster recovery with the soleus fibres transfer technique.

PURPOSE: The study aimed to compare two methods of open surgical treatment for midportion Achilles tendinopathy in sportsmen. A novel technique consisting in transferring some soleus fibres into the degenerated tendon to improve its vascularization and longitudinal tenotomies are evaluated and compared. METHODS: From 2006 to 2011, fifty-two competitive and noncompetitive athletes affected by midportion Achilles tendinopathy were surgically treated and prospectively evaluated at 6 months and at a final 4-year mean follow-up. Twenty patients had longitudinal tenotomies, and thirty-two had soleu

Iliopsoas transfer in the management of established dislocation and refractory progressive subluxation of the hip in cerebral palsy.

The authors have reviewed 25 hips in 23 patients with cerebral palsy in which iliopsoas transfer had been performed for established dislocation or refractory progressive subluxation of the hip. The iliopsoas tendon was transferred either postero-laterally or antero-laterally, depending upon the degree of fixed flexion of the hip. An adductor release was performed in all cases and an open reduction when necessary. A painfree stable joint was produced except for one hip in which the iliopsoas tendon had become detached because of sepsis. The loss of flexor power at the hip due to the transfer is

Neurotized lateral gastrocnemius muscle transfer for persistent traumatic peroneal nerve palsy: Surgical technique.

INTRODUCTION: Persistent traumatic peroneal nerve palsy, following nerve surgery failure, is usually treated by tendon transfer or more recently by tibial nerve transfer. However, when there is destruction of the tibial anterior muscle, an isolated nerve transfer is not possible. In this article, we present the key steps and surgical tips for the Ninkovic procedure including transposition of the neurotized lateral gastrocnemius muscle with the aim of restoring active voluntary dorsiflexion. SURGICAL TECHNIQUE: The transposition of the lateral head of the gastrocnemius muscle to the tendons of…

One-stage repair of skin and tendon digital defects using the arterialized venous flap with palmaris longus tendon: an additional four cases.

The use of the arterialized venous flap with a palmaris longus tendon transfer has previously been reported. Further trials of this technique were conducted in four patients to reconstruct complicated finger injuries involving loss of skin and extensor tendon. In contrast to the results of previous cases, those of recent cases are more encouraging. This technique may be the procedure of choice in patients with digital skin defects, where there are associated extensor tendon defects with exposed bone.

Chronic rupture of tendo Achillis.

The Achilles tendon is the most commonly ruptured tendon in the human body. About 20% of complete ruptures of the Achilles tendon are diagnosed late. The management of chronic ruptures of tendo Achillis is usually different from that of acute rupture, as the tendon ends normally will have retracted. As clinical diagnosis of chronic ruptures can be problematic, imaging can be useful. Most investigators counsel operative management, and the possibility of undertaking a tendon transfer should be kept in mind. Local tendons, such as the flexor digitorum longus, the flexor hallucis longus, and the …

Spontaneous ruptures of the extensor tendons of the fingers in Madelung's deformity.

Madelung's deformity is a congenital malformation with a slow clinical and radiological evolution and few complications. However, we have seen six instances of spontaneous rupture of the extensor tendons of the fingers related to the deformity of the inferior radio-ulnar joint. Treatment consisted of resection of the ulnar head, stabilisation of the distal ulna, and repair of the extensor tendons by tendon transfers. We discuss the possibility of avoiding the tendon ruptures by treating the deformity at an early stage.

Chronic Achilles tendon ruptures.

Chronic Achilles ruptures present a significant challenge for the treating physician. These patients often have marked impairment of functional activities and usually require surgical treatment to optimize outcome. This article reviews some of the methods that have been described to treat the chronic or neglected Achilles tendon rupture. The methods that have been used include gastrocnemius advancement, fascial turndown flaps, local tendon transfers, free tissue transfer, and use of synthetic grafts. There are no comparative data to guide evidence-based decision making in regard to choosing be…

Surgical management of the tendon sheath at different repair stages. Biomechanical and morphological evaluations of direct sheath closure, partial sheath excision, and interposing sheath grafting.

The effects of direct sheath closure, partial sheath excision, and interposing sheath grafting at different repair stages were evaluated in the chicken model. Primary repair was done after sheath incision and tendon transection, and delayed primary repair was done 4 days after sheath and tendon injuries. The sheath grafting was accomplished by interposing a patch of sheath between the longitudinal incision of the plantar sheath. Six weeks later, biomechanical tests for measuring gliding excursions of the repaired tendons, morphological examinations of the extent of adhesions, and histological

Rupture of the extensor pollicis longus tendon after Colles fracture and by rheumatoid arthritis.

During the period 1973-1986, 116 patients with rupture of the extensor pollicis longus tendon were operated upon. The lesions were categorised into three anatomical levels: proximal, intermediate and distal. Direct suture was performed for acute lesions; later reconstructive procedures were by means of tendon transfers or free tendon grafts.

Closed rupture of the tibialis anterior tendon: a report of 2 cases.

UNLABELLED: Closed rupture of the tibialis anterior tendon is a rare injury, and it usually affects individuals older than 50 years of age. This rare injury tends to occur spontaneously, and this often delays diagnosis and adequate treatment. Although direct surgical repair of the ruptured tibialis anterior tendon is generally considered the treatment of choice, nonanatomic repair, tendon lengthening, or tendon transfer might be necessary in cases where shortening of the muscle-tendon unit has taken place. In this report, we describe 2 cases that involved the surgical repair of closed ruptures…

Investigation of walking speed and plantar pressure after chopart amputation.

BACKGROUND: In foot amputation, Chopart amputation is considered to have a high risk of deformity, and can result in poor function. We experienced a case in which Chopart amputation combined with tendon transfer and tendon lengthening was performed, and the patient was eventually able to walk independently with a foot prosthesis without experiencing deformity of the foot. We investigated walking speed and plantar pressure after Chopart amputation with and without a foot prosthesis. CASE: A 78-year-old man underwent Chopart amputation with tendon transfer and tendon lengthening. As a result, h…

The shoulder in neonatal brachial palsy.

The most common residual weakness in neonatal brachial palsies involve the shoulder muscles. More than 84 tendon transfers were performed for weak external rotators. In addition to the tendon transfers, 10 younger children required closed reduction. Nine glenohumeral fusions were performed in adolescents. These procedures have resulted in improved function with few postoperative problems. Four osteotomies of the dislocated shoulder in older adolescents and two open reductions were performed in the older group of children.

Extreme Thumb Losses: Reconstructive Strategies.

BACKGROUND: Historically, complex amputations of the thumb have been managed in two stages and often require additional soft-tissue and tendon transfer for successful outcomes. This article provides several novel strategies to address these problems in a single stage using existing muscles, immediate free tissue transfer, and toe transfer. METHODS: From a personal experience of 482 toe transfers, 24 cases were performed to reconstruct extreme thumb losses. All thumbs were reconstructed in one stage. In only one case, the thumb was reconstructed with a second toe transfer; the remainder had a …

Biomechanical Analysis of Suture Anchor vs Tenodesis Screw for FHL Transfer.

BACKGROUND: Chronic Achilles injury is often treated with flexor hallucis longus (FHL) tendon transfer to the calcaneus using 1 or 2 incisions. A single incision avoids the risks of extended dissections yet yields smaller grafts, which may limit fixation options. We investigated the required length of FHL autograft and biomechanical profiles for suture anchor and biotenodesis screw fixation. METHODS: Single-incision FHL transfer with suture anchor or biotenodesis screw fixation to the calcaneus was performed on 20 fresh cadaveric specimens. Specimens were cyclically loaded until maximal load …
