BACKGROUND: Surgery for obstructive sleep apnea (OSA) has changed in concept and technique that transformed from radical excision to functio … sillar fossa (pharyngoplasty), omni-suspension of the soft palate (palatoplasty) and advancement of uvula (uvuloplasty).
RESULTS: No patient experienced airway compromise, voice change or persistent nasal regurgitation following palatal hybrid surgery. One patient existed postoperative tonsillar fossa bleeding received conservative treatment. Postopera …

We present a case of difficult intubation in a patient with a laryngeal web. A 33-year-old male patient presented for open thoracotomy and had a previously undiagnosed laryngeal web, which complicated the placement of a double-lumen tube. A single-lumen tube was placed with the use of a bougie through the narrowed airway. With the subsequent use of an airway exchange catheter a double-lumen tube was positioned. Techniques for managing narrowing of the supraglottic airway are presented and the literature dealing with laryngeal webs is reviewed. In the setting of an unusual airway and thoracic s

In April 1852 in Cape Town, South Africa, Dr David Livingstone, then 39 years of age and yet to commence the explorations of Africa which would bring him to public attention, underwent the surgical procedure of uvulectomy. This article reviews the history of Livingstone's uvulectomy and its indications and outcome, as documented by his own writings. The practice of uvulectomy in indigenous African societies and some contemporary ideas about uvulectomy are also considered in order to try to ascertain why Livingstone elected to undergo this procedure.

Upper airway surgical treatments for obstructive sleep apnea syndrome (OSAS) attempt to modify dysfunctional pharyngeal anatomy or by-pass the pharynx. Modifications of the pharynx diminish the bulk of soft tissue structures which abut the air column, place them under tension, others alter their spatial inter-relationships. Surgical procedures are designed to modify the retropalatal pharynx, the retrolingual pharynx, or both. There is no single surgical procedure, short of tracheostomy, which consistently results in complete elimination of OSAS. However, appropriate application of current surg

Finkelstein et al have attempted to provide objective data regarding laser-assisted uvulopalatoplasty (LAUP) and uvulopalatopharyngoplasty (UPPP). Their study represents an important and ambitious undertaking because there is a paucity of anatomical studies in the literature. There are several significant concerns in the design of the protocol, however, that require caution in the interpretation and application of the results.

OBJECTIVE: Most papers on LAUP lack patient evaluation of success rate with any statistical significance. We describe a method that we use to measure the improvement in snoring as well as changes related to quality of life.
METHODS: We use a specially designed questionnaire, answered by the patient immediately prior to and immediately after treatment, and 1 week, 1 month, and 6 months postoperatively. Thirty-five patients were included in the study.
RESULTS: Significant improvements were observed in a decrease in frequency of sleeping in a separate room due to snoring, a decrease in frequenc

Uvulo-palato-pharyngo-plasty (UPPP) was introduced as a method of treatment of obstructive sleep apnoea in 1980. The pathogenesis of obstructive sleep apnoea is described. The operative technique is also described and illustrative case histories are quoted. The effect obtained varies and, for this reason, the role of UPPP in the treatment of obstructive sleep apnoea cannot be finally established.

Two 12-year-old boys with severe sleep apnea syndrome but normal-sized tonsils were satisfactorily corrected by tonsillectomy and uvulopalatopharyngoplasty. One of the boys had muscular hypotony as contributing cause of the condition. The other had mandibular hypoplasia in combination with a long soft palate. Thorough preoperative anamnesis and examination in a sleep laboratory are necessary to determine which cases will benefit from surgery in spite of normal-sized tonsils and which surgical procedure will be most helpful.

Treatment of deficiencies of the soft palate is dependent upon its condition, and the plastic surgeon is guided in his therapy by its length and suppleness. This was also the case when, in the past, a prosthesis had to be applied. If it is sufficiently long and supple a simple velopharyngoplasty, followed by phonetic reeducation, has every chance of producing a good result, at least in cooperative children and adolescents. When the soft palate is very shortened, however, a simple velopharyngoplasty does not ensure effective rhinopharyngeal occlusion and lengthening of the soft palate and the p

Over 10,000 school children were screened for palatal abnormalities. Nine submucous clefts of the palate were detected, an incidence of 1 : 1,200. Speech defects, ear problems and hearing loss were found as complications in 55% of 50 submucous cleft palate patients studied in detail. Complications were less common (45%) if the palatal abnormality was an isolated defect. Proper management of submucous cleft palate depends upon knowledge of its incidence and natural history.
