Association between oxidative stress and atrial fibrillation.

BACKGROUND: Oxidative stress (OS) may be a key mechanism underlying the development of atrial fibrillation (AF) in experimental studies, but data in humans remain limited. OBJECTIVE: Systemic OS can be estimated by measurements of circulating levels of the aminothiols including glutathione, cysteine, and their oxidized products. We tested the hypothesis that the redox potentials of glutathione (EhGSH) and cysteine will be associated with prevalent and incident AF. METHODS: Plasma levels of aminothiols were measured in 1439 patients undergoing coronary angiography, of whom 148 (10.3%) had a d…

Ablation of long-standing persistent atrial fibrillation.

Atrial fibrillation (AF) is the most commonly encountered arrhythmia in the clinical setting affecting nearly 6 million people in United States and the numbers are only expected to rise as the population continues to age. Broadly it is classified into paroxysmal, persistent and longstanding persistent AF. Electrical, structural and autonomic remodeling are some of the diverse pathophysiological mechanisms that contribute to the persistence of AF. Our review article emphasizes particularly on long standing persistent atrial fibrillation (LSPAF) aspect of the disease which poses a great challeng…

Secondary Stroke Prevention in Nonvalvular Atrial Fibrillation.

Approximately 750,000 US adults per year experience a stroke. On average, the annual risk for future ischemic stroke (secondary stroke) after an initial ischemic stroke or transient ischemic attack is approximately 3% to 4%. Cardioembolic strokes account for 20% to 25% of all strokes, with nonvalvular atrial fibrillation (NVAF) considered one of the main sources of embolism; this explains up to half of all cardioembolic strokes. We present the risk factors for stroke in NVAF, risk stratification, a diagnosis of NVAF, and treatment and prevention of stroke in NVAF. We reviewed the literature by…

How I use anticoagulation in atrial fibrillation.

Atrial fibrillation is the most common cardiac arrhythmia and conveys a significant risk of morbidity and mortality due to related stroke and systemic embolism. Oral anticoagulation (OAC) is the mainstay of thromboembolism prevention, and management of anticoagulation can be challenging. For patients without significant valvular disease, decisions around anticoagulation therapy are first based on the presence of additional stroke risk factors, as measured by the CHA2DS2-VASc (congestive heart failure, hypertension, age ≥75, diabetes, prior stroke or transient ischemic attack, vascular disease,…

Atrial Fibrillation Triggered By Drug-Induced Bradycardia.

Atrial fibrillation (AF) is the most frequently observed arrhythmia in clinical practice. Many causative factors have been identified from well-known structural heart disease to less understood triggers. Both sympathetic and parasympathetic (vagal) stimuli are able to trigger paroxysms of AF. Vagally mediated AF is especially observed in young healthy subjects and especially during nights when the heart rate is considerably slow. Tachycardia induced AF is demonstrated and the possible mechanisms are explained. However, a case of bradycardia induced AF, thus far, hasn't been reported. Here we p…

Nonfluoroscopic Ablation of Atrial Fibrillation Using Cryoballoon.

BACKGROUND: The conventional method of cryoballoon ablation of atrial fibrillation involves the use of fluoroscopy for visual guidance. The use of fluoroscopy is accompanied by significant radiation risks to the patient and the medical staff. Herein, we report our experience in performing successful nonfluoroscopic pulmonary vein isolation using cryoballoon ablation in 5 consecutive patients with paroxysmal atrial fibrillation. METHODS AND RESULTS: Five consecutive patients with paroxysmal atrial fibrillation underwent cryoballoon ablation for pulmonary vein isolation using a nonfluoroscopic …

Stand alone surgical ablation for atrial fibrillation.

BACKGROUND: Trials to maintain sinus rhythm in patients with atrial fibrillation (AF) and refractory symptoms have been complicated by lack of success or intolerance of medications. Experience with minimally invasive AF surgery is relatively new, and early results have been promising. However, the study populations and techniques were heterogeneous, and the follow-up periods were short in many series. METHODS: We present a single center experience through a retrospective review of medical records of patients who had minimally invasive AF surgery. RESULTS: The surgical techniques addressed se…

Idiopathic dilated cardiomyopathy and chronic atrial fibrillation.

BACKGROUND: Atrial fibrillation (AF) is common in idiopathic dilated cardiomyopathy (IDC). We explored the clinical characteristics of IDC patients with chronic AF compared with those with sinus rhythm (SR). METHODS: A group of patients with IDC underwent extensive non-invasive and invasive evaluation during a hospitalization period. The patients were further divided into two groups with AF (n = 19) and SR (n = 68). RESULTS: Left atrial diameter was greater (P<0·001), left ventricular end-diastolic diameter smaller (P<0·05), left ventricular end-diastolic and end-systolic volumes smaller (P<…

How can we best detect atrial fibrillation?

Atrial fibrillation (AF) is an arrhythmia of increasing prevalence associated with a reducible risk of stroke. We conducted a systematic review to address five questions relating to how we can best detect AF: 1. Are there useful screening tests to determine who should have a 12-lead electrocardiogram (ECG)? Potential screening tests, all with acceptable sensitivity, include pulse palpation, single-lead ECG and newer technologies such as modified sphygmomanometers or a finger probe device. Pulse palpation has a high number of false positives, but is the cheapest method. 2. Is it more effective …

Atrial fibrillation in ambulatory consultant cardiology clinics.

BACKGROUND: Information on the prevalence and real-life management of atrial fibrillation (AF) in a general practice setting is sparse. OBJECTIVES: To investigate the prevalence, clinical features and real-life management of AF in ambulatory consultant cardiology clinics. METHODS: We studied consecutive new patients with a diagnosis of AF. Recorded data included demographics, comorbidities and prescribed drugs. Antithrombotic therapy was chosen according to CHADS2 score in non-rheumatic valve AF patients; for AF patients with rheumatic valve disease warfarin treatment was prescribed. The dat…

[Metabolic syndrome and chronic persistent atrial fibrillation].

The aim of the study was to elucidate specific features of chronic recurrent atrial fibrillation (AF) in patients with metabolic syndrome (MS) and disturbed carbohydrate metabolism compared with AF patients without MS. It enrolled 145 patients aged 44-83 years: 117 with abdominal obesity (BMI >30 kg/m2, waist circumference >80 and 94 cm in women and men respectively) including 30 without metabolic disturbances; 35 with impaired glucose tolerance (IGT), 52 with type 2 DM, and 28 controls without MS. Parameters measured included frequency and severity of AF, carbohydrate and lipid metabolism, al…

[Paroxysmal atrial fibrillation: which anti-arrhythmic treatment?].

Only symptomatic patients with paroxysmal AF will be prescribed anti-arrhythmic drugs in order to prevent recurrences. Anti-arrhythmic drugs' efficacy in maintaining sinus rhythm is around 50 to 70 % at one year. Anti-arrhythmic drug prescription is associated with potentially life-threatening pro-arrhythmic effects. Anti-arrhythmic drug prescription should be tailored to each patient after carefully evaluating the risk/benefit ratio. Pulmonary veins isolation might be proposed in case of symptomatic paroxysmal AF after failure of anti-arrhythmic drug therapy.
