Displacement of the incisive foramen in conjunction with implant placement in the anterior maxilla without jeopardizing vitality of nasopalatine nerve and vessels: a novel surgical approach.

Accurate implant placement in the anterior maxilla is essential in achieving optimal prosthetic rehabilitation with proper function and acceptable esthetic and phonetic demands. One of the preferable prosthetic solutions to restore a missing maxillary incisor in young adults is by an implant-supported crown. Bone resorption together with an enlarged incisive foramen, challenge proper implant placement. A simultaneous procedure where the implant osteotomy site penetrated the incisive canal is presented. A configurated cortico-cancellous block graft core was adjusted to fit the foramen while its

Clinical and Surgical Predictors of Velopharyngeal Insufficiency After Le Fort I in Patients With Cleft Palate.

Velopharyngeal insufficiency (VPI) is a recognized complication following palatoplasty in patients with cleft palate and may also occur after orthognathic surgery. Although risk factors for VPI after palatoplasty are well established, those contributing to VPI following orthognathic procedures remain unclear. This retrospective cohort study aimed to identify clinical and surgical predictors of VPI in patients with repaired cleft palates undergoing orthognathic surgery. Conducted at a single tertiary pediatric hospital, the study included nonsyndromic individuals who underwent Le Fort I osteoto

Perforation of the sinus membrane during sinus floor elevation: a retrospective study of frequency and possible risk factors.

PURPOSE: To analyze the frequency of perforation of the sinus membrane during maxillary sinus floor elevation (SFE) and to assess possible risk factors. MATERIALS AND METHODS: Seventy-seven cases of SFE performed with a lateral window approach were evaluated retrospectively. Clinical and radiographic variables potentially influencing the risk of sinus membrane perforation were evaluated and divided into patient-related factors (age, sex, smoking habit); surgery-related factors (type of surgical approach, side, units, sites, and technique of osteotomy); and maxillary sinus-related factors (pre

Maxillofacial reconstruction with prefabricated osseous free flaps: a 3-year experience with 24 patients.

Between January of 1998 and May of 2002, 25 prefabricated osseous free flaps (23 fibula and two iliac crest flaps) were transferred in 24 patients to repair maxillary (six flaps) or mandibular (eight flaps) defects after tumor resection, severe maxillary (four flaps) or mandibular (one flap) atrophy (Cawood VI), maxillary (one flap) or mandibular (three flaps) defects after gunshot injury, and maxillary (two flaps) defects after traffic accidents. Prefabrication included insertion of dental implants, positioned with a drilling template in a preplanned position, and split-thickness grafting. Dr

Comparison between 2- and 4-plate fixation in Le Fort I osteotomy: a mixed methods systematic review.

OBJECTIVE: Achieving postsurgical skeletal stability is crucial for successful outcomes and patient satisfaction. Precision maxillofacial surgery, which integrates precision techniques with minimally invasive approaches, is increasingly recognized for its potential to enhance long-term stability and reduce surgical risks and complications. This study aimed to comprehensively evaluate the impact of fixation techniques (2 vs 4 plates) during Le Fort I osteotomy in orthognathic surgery on skeletal stability. The main focus was to compare the stability achieved and potential risks of relapse betwe

Drug Release and Pharmacokinetic Evaluation of Novel Implantable Mometasone Furoate Matrices in Rabbit Maxillary Sinuses.

BACKGROUND: Intranasal corticosteroid sprays (INCSs) used to treat chronic rhinosinusitis are suboptimal due to limited penetration into the middle meatus, rapid clearance, and poor patient compliance. A bioresorbable drug matrix, developed with the XTreoTM drug delivery platform, may overcome the limitations of INCS by providing continuous dosing over several months. OBJECTIVE: To evaluate the in vitro drug release and in vivo pharmacokinetics of novel mometasone furoate (MF) matrices in a rabbit dorsal maxillary osteotomy model. METHODS: XTreoTM matrices were formulated to consistently elu…

Management and simultaneous implant placement of maxillary sinus membrane perforation: A report of two cases.

Rehabilitation of the long-standing edentulous posterior maxilla with dental implant poses a unique challenge. This is due to mainly two reasons - pneumatization of the maxillary sinus and atrophy of the alveolar bone. The challenge is intensified when the native bone is around 2-3 mm. This requires vertical bone augmentation in the form of direct sinus lift/lateral wall sinus lift procedure. The most common complication associated with this procedure is the sinus membrane perforation resulting in unfavorable stabilization of the graft and associated bone regeneration. Simultaneous implant pla

Effects of Condensation on Peri-implant Bone Density and Remodeling.

Bone condensation is thought to densify interfacial bone and thus improve implant primary stability, but scant data substantiate either claim. We developed a murine oral implant model to test these hypotheses. Osteotomies were created in healed maxillary extraction sites 1) by drilling or 2) by drilling followed by stepwise condensation with tapered osteotomes. Condensation increased interfacial bone density, as measured by a significant change in bone volume/total volume and trabecular spacing, but it simultaneously damaged the bone. On postimplant day 1, the condensed bone interface exhibite

[Surgery of asymmetry].

Endeavoring to present, within a few pages, an exhaustive work of all the surgical proposals, regarding the multitude of ethiopathogeneses, is quite utopic. The author considers the evaluation of the asymmetrical patient and underlines in the complementary examinations the advantage of 3D tomodensitometry. A chapter is devoted to presurgical orthodontic preparation aimed at counteracting the compensations, which affect the maxillary and mandibular alveolar support. The therapeutic strategy calls upon "combined" programs. A "range" of osteotomies is suggested within the framework of the most fr

[Orbito-rhino-cerebral phycomycosis (mucormycosis): report of a case].

The case of a 15 year old white man, diabetic in cetoacidosis, with a orbit-rhino-cerebral phycomycosis is reported. The illness had an acute onset and the treatment was iniciated early with Amphotericin-B and unilateral osteotomy of maxillary and ethmoidal sinus. With this treatment the patient did well with residuals of ophtalmoplegia and amaurosis on the right. Interesting investigation aspects are the occluded internal carotid on the same side of the affected orbit and the CAT-SCAN finding of moderated ventricular dilatation (two months after hospital admission). Mycology, pathophysiology,

Three dimensional accuracy and stability of segmented Le Fort I osteotomy - a systematic review.

BACKGROUND: Segmental Le Fort I osteotomy has traditionally been regarded as less predictable and stable, a perception largely based on two-dimensional evaluation methods. Despite existing systematic reviews on clinical outcomes, a dedicated synthesis of three-dimensional assessments of surgical accuracy and postoperative stability is lacking. PURPOSE: This systematic review evaluated three-dimensional (3D) assessment methods and outcome of planning accuracy and postoperative stability in segmented Le Fort I osteotomies. The aim was to determine the reliability of 3D methods in capturing surg

The surgical outcomes of anterior segmental osteotomy in Asian skeletal class II patients.

PURPOSE: Anterior segmental osteotomy (ASO) following the surgery-first approach is a long-established treatment modality to resolve lip protrusion in patients with skeletal class II patterns. However, the indications and effectiveness of ASO still remain uncertain. The objective of this study is to investigate the effectiveness of ASO in Asian skeletal class II patients by evaluating the skeletal and soft tissue changes and analyzing pre-treatment variables that determine successful outcomes in occlusal as well as esthetic aspects. METHODS: The lateral cephalograms of 44 skeletal class II pa

Two middle-aged cases of deep overbite without molar support treated by orthognathic surgery.

INTRODUCTION: Patients with severe overbite in middle age often undergo prosthetic treatment without a diagnosis of dentofacial deformity, but soft tissue trauma can occur in such patients due to the lack of inter-occlusal space. Comprehensive surgical orthodontic treatment and prosthetic treatment are indicated to correct the overbite and soft tissue injury for such patients. PRESENTATION OF CASE: We report the cases of two middle-aged patients with dentofacial deformity and severe overbite without molar support. In both cases, prosthetic treatment had been performed for many years without a
