Quantitative evaluation of ocular motility in blow-out fractures for selection of nonsurgically managed cases.

PURPOSE: To clarify in which cases of blow-out fractures ocular motility will naturally recover. DESIGN: A retrospective review of observational cases. METHODS: Involved ocular motility ranges were measured by the Hess screen test at 15 or 30 degrees in 23 nonsurgical blow-out fracture patients without muscle entrapment or strong mechanical ocular restriction and in eight surgical patients with the above. RESULTS: In nonsurgical patients, the eyes moved more than 10 or 20 degrees respectively on a 15- or 30-degree Hess screen test in the early period, whereas in the surgical patients, the e

Familial ptotic lid elevation during ipsilateral abduction.

PURPOSE: To report and discuss the clinical findings of a 17-member family with 2 siblings who exhibit ptosis and abnormal synkinetic lid elevation associated with ipsilateral abduction. SUBJECTS AND METHODS: Sixteen members of the 17-member immediate family underwent ophthalmic examination. RESULTS: Two siblings exhibited ptosis and abnormal synkinetic lid elevation associated with ipsilateral abduction. One was bilaterally affected and the other had unilateral findings. A third sibling had isolated bilateral congenital ptosis. A fourth demonstrated classic Duane syndrome Type I in the righ

[Fisher's one and half syndrome with facial palsy as clinical presentation of giant cell temporal arteritis].

An ischemic stroke in an old patient is commonly due to thrombosis or embolism. A restriction in the differential diagnosis and not considering another etiology could occasion the loss of effective therapy and prevent major complications. We report the case of a patient who suffering from systemic symptoms for a few months, was admitted to the hospital with the one and a half syndrome described by Fisher, together with right facial palsy, both products of an ischemic pontine lesion. Bilateral biopsies of the temporal arteries were diagnostic of temporal arteritis and MRN of the cranium confirm

Paraneoplastic opsoclonus-myoclonus secondary to malignant melanoma.

Opsoclonus-myoclonus is a well-described paraneoplastic syndrome that most often occurs in association with small cell carcinoma of the lung and breast carcinoma. To the best of our knowledge, we describe the first reported case of paraneoplastic opsoclonus-myoclonus occurring is association with malignant melanoma. Antineuronal antibodies were not identified despite repeated testing. No therapeutic benefit was observed following corticosteroid administration.

Neuroophthalmologic manifestations of systemic disease.

Significant contributions on the neuroophthalmologic manifestations of systemic disease have been published in the past year. The first part of this paper is devoted to practical guidelines that may help in the diagnosis and the management of oculomotor disorders, especially in connection with systemic diseases; the second part is focused on specific entities. Including idiopathic intracranial hypertension and ophthalmoplegic migraine in this review might be debatable; however, although the cause of these conditions is still unclear, relevant findings suggest that they may be the consequence o

Carbamazepine responsive epileptic oral motor and ocular motor apraxia.

We evaluated seven patients with oral motor apraxia and ocular motor apraxia. Apraxia in three patients (Group 1) with new-onset partial seizures and epileptiform discharges on EEG improved with carbamazepine. Four patients (Group 2) without seizures and nonepileptiform EEG findings had no change in apraxia after a trial of carbamazepine. Epileptic apraxia may precede clinical seizures and can respond to antiepileptic drugs.

[Hallucination in opsoclonus-polymyoclonus syndrome].

Here we report 2 cases of opsoclonus-polymyoclonus syndrome (OPS) associated with viral encephalitis. They had sleep disturbance, and visual hallucination. Case 1 had auditory hallucination in addition, and case 2 had dreamlike behavior. Those hallucination, which were colorful and vivid, usually appeared at the bed time. Their hallucinations were similar to peduncular hallucinations and you may also call hypnagogic hallucinations, which are often seen in patients with narcolepsy. Dreamlike behavior is observed during REM sleep in patients with brainstem damage or sometimes in the healthy elde

Ocular manifestations of Sotos syndrome.

BACKGROUND: Sotos syndrome (cerebral gigantism) is a congenital syndrome characterized by large body size for patient age, advanced bone age, and unusual facial characteristics with varying levels of cognitive, developmental and perceptual deficits. While more than 150 cases have been reported, only a single case report has been published in the ophthalmic literature. This study briefly reviews the pertinent aspects of this syndrome and reports on the medical, physical, developmental and ocular manifestations of 32 subjects. METHODS: Our patient population was obtained from several schools an

Transient opsoclonus in organophosphate poisoning.

Opsoclonus transiently occurred in the acute stage of combined parathion and methyl-parathion poisoning. Despite persistent cholinesterase inhibition this symptom subsided spontaneously. This observation provides strong evidence for cholinergic pathways being reversibly involved in isolated opsoclonus.

Superior rectus muscle overaction after cataract extraction.

Four patients with an ipsilateral hypertropia after cataract extraction consistent with superior rectus muscle overaction were identified between March 1990 and April 1992. Operative trauma was the most likely causative factor, as other likely conditions were excluded. The proposed pathogenesis for all cases is similar to that of botulinum type-A toxin therapy: a transient postoperative weakness of the ipsilateral inferior rectur muscle leads to a contracture or strengthening of the ipsilateral antagonist (the superior rectus muscle). Possible mechanisms of injury that would result in a transi

Nuclear and infranuclear disorders.

Lesions of the brain stem can either affect the nuclei or the fascicles of the third, fourth or sixth cranial nerves and thus produce ocular motor disorders. Lesions of the oculomotor nuclear complex differ from lesions of the third nerve, since the motoneurones in the nucleus are specifically grouped. Similarly, a lesion of the sixth nerve nucleus results in a conjugate gaze palsy and not in an abducens palsy, because of 'interneurones' being intermingled with the abducens motoneurons. Isolated lesions of a nerve fascicle, which is the part of the cranial nerve running through the brain stem,

Ataxia telangiectasia presenting as an extrapyramidal movement disorder and ocular motor apraxia without overt telangiectasia.

Ataxia telangiectasia may present with few, if any, of its typical extraneurological manifestations, but the combination of an extrapyramidal movement disorder, ocular motor apraxia with head thrusting and cerebellar incoordination is characteristic. In this sporadic case there was no overt immune dysfunction, oculocutaneous telangiectasia were inconspicuous and the neurological presentation was atypical with dystonia predominating over cerebellar incoordination. The uncontrollable and disabling involuntary movements, which have not to our knowledge been described in ataxia telangiectasia befo

Binocular VEP summation in infants and adults with abnormal binocular histories.

Visual evoked potentials (VEPs) were recorded under binocular and monocular viewing conditions in 19 2- to 10-month-old infants, 19 stereodeficient adults, and 12 normal adults. VEPs were elicited by medium contrast, phase-alternated checkerboards with check sizes ranging from 10-52 min of arc. Binocular VEP summation was defined as the percentage by which the binocular VEP amplitude exceeded the mean of the two monocular VEP amplitudes. In the stereonormal adults, binocular VEP summation was significantly greater than zero and significantly less than 100%. In the stereodeficient adults, binoc

Convergence insufficiency and Parkinson's disease progression.

BACKGROUND: Parkinson's disease (PD) is a chronic, neurodegenerative disorder that significantly affects motor, sensory, and vision functions. Convergence insufficiency (CI) may be an indicator of PD progression. This study aimed to explore the relationship between CI and the stages of PD progression, as measured by the Hoehn and Yahr scale, and to compare the prevalence of CI and vision parameters between healthy individuals and PD patients. METHODS: Age-matched healthy controls (n = 97) and PD patients (n = 71) underwent medical history evaluations and vision assessments. Statistical analys

An unexpected turn: a case of torsional anomalous retinal correspondence after myectomy of previously anteriorized inferior oblique muscles in a child.

We report a case of torsional diplopia caused by presumed torsional anomalous retinal correspondence after myectomy of previously asymmetrically anteriorized inferior oblique muscles for inferior oblique overaction. Given this patient's experience, it may be prudent to operate with caution on previously anteriorized inferior oblique muscles, especially when anteriorization is performed at a very young age.

Horizontal gaze palsy with progressive scoliosis.

Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare autosomal recessive condition characterized by absence of abduction and adduction movements with intact vertical eye movements and progressive scoliosis. Patients usually present by mid-childhood with complaints of progressive scoliosis. The clinical diagnosis of HGPPS can be further confirmed by the ROBO3 gene mutation on chromosome number 11. We present 2 Indian siblings who were incidentally diagnosed with HGPPS with synergistic convergence on regular eye examination; diagnosis was confirmed by radiological and genetic testi

Internuclear Ophthalmoplegia Following COVID-19 with Abnormal Findings on Neuroimaging; A Case Report.

COVID-19 had been reported to be associated with neuro-ophthalmic complications. However, there is a shortage of evidence regarding internuclear ophthalmoplegia (INO) presentation after COVID-19 infection with related brain magnetic resonance imaging (MRI). Herein, we report a case of INO in a 69-year-old woman with presentation of diplopia following COVID-19 infection, along with her neuroimaging. We concluded that it is important to recognize the possibility of INO in patients with diplopia after COVID-19 and to consider brain MRI as an informative tool in these patients.

Internal carotid artery aneurysm with incomplete isolated oculomotor nerve palsy: a case report.

BACKGROUND: Oculomotor nerve palsy is a common and well-described disease with diverse etiologies. Clinicians should quickly and correctly diagnose and treat oculomotor nerve palsy according to its characteristics and the accompanying symptoms and signs. Intracranial aneurysm is an important and frequent cause of oculomotor nerve palsy. Considering the catastrophic consequences of rupture, the possibility of an urgent, life-threatening disease should always be considered. CASE PRESENTATION: A 63-year-old Chinese woman presented with intermittent left ptosis and diplopia and painless incomplet
