Potential of extracellular vesicle-derived microRNAs as a platform for biomarker discovery in acute lymphoblastic leukemia.

BACKGROUND: Extracellular vesicle (EV)-derived microRNAs (miRNAs) represent a promising platform for biomarker discovery in acute lymphoblastic leukemia (ALL). This study evaluated the biomarker potential of EV-derived miRNAs isolated from five ALL cell lines. METHODS: Human ALL cell lines were cultured in EV-depleted fetal bovine serum. These included two parental cell lines, CCL-119 and CRL-3273, and three related cell lines previously characterized as exhibiting chemoresistance-associated phenotypes: CRL-2264 and CRL-2265, derived from CCL-119, and CRL-3274, derived from CRL-3273. EVs were

B-cell acute lymphoblastic leukemia following myelodysplastic syndromes: a case report and literature review.

Myelodysplastic syndromes (MDS) comprise clonal hematopoietic stem cell disorders characterized by heterogeneous clinical manifestations. Approximately 20-30% of MDS cases progress to acute myeloid leukemia, whereas transformation to acute lymphoblastic leukemia (ALL) is extremely rare. In this report, we present the case of a Chinese male patient who presented with MDS-refractory anemia with ringed sideroblasts, which developed into B-cell ALL. During disease transformation, the patient acquired novel gene mutations. By comparing the gene mutations identified at the initial MDS diagnosis with

[Indications for allogeneic stem cell transplantation in Philadelphia chromosome-positive acute lymphoblastic leukemia].

Allogeneic hematopoietic stem cell transplantation (allo-SCT) in first complete remission (CR1) has been the standard treatment for Philadelphia chromosome-positive acute lymphoblastic leukemia. However, the introduction of imatinib, a first-generation tyrosine kinase inhibitor (TKI), dramatically improved outcomes. The subsequent development of next-generation TKIs like dasatinib and ponatinib, as well as the antibody therapy blinatumomab, has further diversified treatment strategies. There is an increasing shift toward avoiding allo-SCT in pediatric patients, both in Japan and internationall

The molecular biology of acute lymphoblastic leukemia (review).

The emergence of molecular and cytogenetic correlates has opened exciting avenues to a better understanding of acute lymphocytic leukemia (ALL) and in turn has sharpened the clinician's ability for classification and treatment. While the pathophysiology and etiology of ALL remains vague, the clinical significance of the chromosomal abnormalities in the pathogenesis of malignant transformation is becoming clearer. This review highlights recent achievements in the genetic research of ALL and discusses the epidemiology, clinical features, and therapeutic determinants of this hematopoietic maligna

H3K18 Lactylation Promotes Cell Proliferation in T-Cell Acute Lymphoblastic Leukemia.

The Warburg effect-induced lactate production in T-cell acute lymphoblastic leukemia (T-ALL) cell proliferation is well established, however, the role of lactate-mediated protein lactylation in this process remains poorly understood. In this study, we demonstrated that inhibiting lactate levels through the lactate dehydrogenase A (LDHA) inhibitor significantly reduced both cell proliferation and protein lactylation levels in T-ALL cells. Lactate deficiency led to a marked decrease in protein lactylation, accompanied by impaired cell proliferation. Meanwhile, lactate deficiency also induced the

Lymphomatoid Papulosis Development in Acute Lymphoblastic Leukemia.

Lymphomatoid papulosis (LyP) is a chronic, recurrent benign skin disease characterized by histological features of a CD 30-positive cutaneous T-cell lymphoproliferative disorder. It is rare, with an annual, worldwide incidence of 1.2 - 1.9 per million, and accounts for 16-47% of pediatric cutaneous lymphoproliferative disorders. It often occurs on the extremities or the trunk and rarely affects the face or genitals. Its onset may be triggered by irradiation therapy, immunomodulating agents, infection or atopic dermatitis. It has a benign course but is associated with certain hematological mali

Outcomes of relapsed B-cell acute lymphoblastic leukemia after sequential treatment with blinatumomab and inotuzumab.

Novel monoclonal antibody (mAb)-based therapies targeting CD19 and CD22 (blinatumomab and inotuzumab) have shown high rates of complete remission (CR) and been used as a bridging treatment to potentially curative allogeneic hematopoietic stem cell transplantation (alloHSCT) in adults with relapsed or refractory (R/R) B-cell acute lymphoblastic leukemia (B-ALL). However, limited data exist on the outcome of patients resistant to both mAbs as well as responses to each agent when progressed after the alternate antigen-targeted mAb. Herein, we report outcomes of 29 patients with R/R B-ALL previous

Cutaneous Presentation of Pre-B Cell Acute Lymphoblastic Leukemia in a 3-Month-Old Boy.

We present a rare case of leukemia cutis as the sole presenting sign of pre-B cell acute lymphoblastic leukemia in a 3-month-old boy. The patient exhibited a solitary, erythematous scalp nodule without systemic symptoms or abnormal peripheral laboratory findings, ultimately diagnosed via biopsy and treated successfully. This case highlights the importance of early dermatologic evaluation and biopsy in persistent or atypical infant scalp lesions.

Updates in Childhood Acute Lymphoblastic Leukemia.

Recent molecular studies have identified subgroups of acute lymphoblastic leukemia (ALL) driven by genomic aberrations rather than structural chromosome rearrangements or aneuploidy. Recent years have brought the advent of immunotherapy to cancer treatment regimens, and several effective agents have been developed for B-ALL. With increasing knowledge of molecular drivers of pediatric ALL and the identification of potential targets widely expressed on leukemic blasts, targeted agents and immunotherapies have been developed for pediatric patients with R/R-ALL.

Natural-product polypharmacology in acute lymphoblastic leukemia: a state-of-the-art review.

BACKGROUND: High-risk and relapsed/refractory (R/R) acute lymphoblastic leukemia poses significant therapeutic challenges due to emergent drug resistance and dose-limiting toxicities. Natural products, with their diverse chemical structures and pharmacological activities, provide a promising avenue for multi-target therapies. PURPOSE: This review analyzes key natural product classes, such as phenolic compounds, terpenoids, flavonoids, and alkaloids. It aims to elucidate their mechanisms by modulating critical oncogenic pathways to overcome drug resistance, paving the way for rational therapeu

A Novel Scale Biopsy Method to Collect Zebrafish Malignant and Non-Malignant Lymphocytes.

Zebrafish (Danio rerio) are a powerful model for studying lymphopoiesis and lymphoid cancers, since zebrafish and humans share similar adaptive immune systems, including B and T cells. Several D. rerio T-cell acute lymphoblastic leukemia (T-ALL) models are described, including fish that express lymphoblast-specific human MYC (hMYC). We reported that rag2:hMYC fish also develop B-cell acute lymphoblastic leukemia (B-ALL), making them useful to study both ALL types. A limitation of zebrafish ALL models is that no non-lethal methods exist for serial sampling of ALL. To address this, we pioneered

Treatment-related mutagenic processes in acute lymphoblastic leukemia.

Karyotyping, SNP arrays, fluorescence in situ hybridization and next-generation sequencing techniques have greatly improved our understanding of the genetic drivers of acute lymphoblastic leukemia (ALL). In the past years, another layer of genetic data has been added by the study of mutational signatures, patterns of somatic mutations that represent specific mutational mechanisms. Mutational signatures can give insight into tumor development, but also reveal mutagenic side-effects of treatment in relapse samples. Multiple treatment-related mutational signatures have been detected in relapsed A

[A case of leukemic optic nerve infiltration presenting as asymmetric papilledema in both eyes].

A 11-year-old male patient presented with a 4-month history of decreased vision in the left eye, which gradually worsened to complete loss of vision in the left eye over the past 1 month. The patient had a 6-year history of acute lymphoblastic leukemia and had been clinically cured after chemotherapy for 3 years. The visual acuity of the left eye was hand-motion at near. Fundus examination revealed asymmetric papilledema in both eyes. The right eye showed typical papilledema, while in the left eye, the optic disc margin was blurred, and there were numerous thick patchy yellow-white lesions and

Methemoglobinemia Induced by Food During Chemotherapy in a Boy With Acute Lymphoblastic Leukemia and Literature Review.

Methemoglobinemia, a rare and potentially life-threatening condition in pediatric hematology-oncology patients, requires accurate cause identification for effective treatment. We report a case of a 7-year-old boy with acute lymphoblastic leukemia who developed methemoglobinemia during chemotherapy. Initial vitamin C treatment was ineffective, requiring methylene blue. A detailed dietary history revealed the consumption of nitrate-rich vegetables and rapeseed oil, suggesting these as potential triggers. Our literature review highlights the diverse etiologies of methemoglobinemia in this patient

Navigating B-ALL in the Era of Blinatumomab.

Blinatumomab has rapidly emerged as a cornerstone in the treatment of B-cell ALL (B-ALL) across age and risk groups. Initially approved for minimal residual disease (MRD)-positive adult B-ALL in 2018, its indications have since expanded after pivotal trials demonstrating significant efficacy. The BLAST and E1910 trials highlighted the benefit of blinatumomab in adults in MRD-positive and MRD-negative remission, respectively, with notable improvements in overall survival and relapse-free survival. In pediatric populations, studies such as the AALL1731 and a landmark trial in infants with KMT2A-

Elbow osteomyelitis by Candida tropicalis in acute lymphoblastic leukemia. A case report.

INTRODUCTION: musculoskeletal involvement of the extremities is common in pediatric patients with acute lymphoblastic leukemia. The insidious and oligosymptomatic presentation of several musculoskeletal conditions in these patients often leads to different diagnostic suspicions, which may delay appropriate management of the pathology. Non-albicans Candida osteoarticular infections are a rare entity that may affect immunodeficient patients. We describe a case of osteoarticular infection caused by an opportunistic fungus in a pediatric patient with acute lymphoblastic leukemia during the second

Acute lymphoblastic leukemia first presenting with ascites: A case report.

RATIONALE: Acute lymphoblastic leukemia (ALL) is known for its tendency to present with extramedullary infiltrates, commonly affecting lymph nodes, the spleen, the central nervous system, the orbit, skin, and testes. Less commonly, leukemia can infiltrate unusual sites such as the cervix, gastrointestinal tract, lungs, and thyroid gland. However, it is exceedingly rare for leukemia to present with ascites as the initial symptom. By reporting this rare case, the study seeks to contribute to a better understanding of ALL's varied presentations and improve early detection and management strategie

A Rare Case Study of Granular Acute Lymphoblastic Leukemia Combined with Pleural Infiltration.

BACKGROUND: Acute B-lymphoblastic leukemia (B-ALL) is a common hematologic malignancy characterized by blasts with a variable amount of cytoplasm, typically ranging from minimal to moderate, and containing few cytoplasmic granules. However, granular acute lymphoblastic leukemia, as a rare subtype, is distinguished by the presence of abundant coarse, purplishred granules within the cytoplasm of the blasts, which can be confused with other diseases in clinical diagnosis. METHODS: The patient was examined using bone marrow morphological analysis, flow cytometry, genetic screening, and chromosome
