Surgical management of large-angle incomitant strabismus in patients with oculomotor nerve palsy.

OBJECTIVE: To evaluate the surgical options in treating strabismus caused by different degrees of oculomotor nerve palsy. METHODS: Surgical procedures for 13 patients with unilateral oculomotor nerve palsy were retrospectively studied. Eight patients had partial paralysis with isolated or multiple muscle involvement. A greater amount of lateral rectus recession and medial rectus resection than is usual was performed in six cases; transposition combined with resection of the medial rectus was performed in two cases with limited hypotropia. Of five patients with total oculomotor nerve paralysis…

[Is congenital superior oblique strabismus a paretic disorder?--A magnetic resonance tomographic study].

BACKGROUND: Recent reports postulate that the concomitant vertical deviation found in congenital superior oblique palsy is due to mechanical abnormalities rather than a congenitally paretic muscle, and is overcome in most patients by fusion. On the basis of the clinical characteristics alone a primary paresis is indeed unlikely. Although intraoperatively a different elasticity of the superior oblique tendon exists in congenital versus acquired cases of superior oblique palsy, preoperatively performed MR imaging shows that the clinical findings in congenital superior oblique muscle malfunction

[Familial sick sinus syndrome associated to strabismus and persistent left superior vena cava].

We report a family in which the mother and her three sons suffered sick sinus syndrome and strabismus. Two members had a persistent left superior vena cava with drainage into coronary sinus. The illness in all members of this family was oligosymptomatic, and well tolerated with mild symptoms like dizziness, fatigue and exercise dyspnea associated with nodal rhythm. Three of them, had episodes of paroxysmal atrial fibrillation. All patients remain asymptomatic after pacemaker implantation.

Postoperative emesis after pediatric strabismus surgery: the effect of dixyrazine compared to droperidol.

Sixty-one children, ASA physical status I, aged 2-14 years, admitted for strabismus surgery were studied. All were premedicated with diazepam and atropin rectally. Anesthesia was induced with thiopental or with halothane on a facemask, and succinylcholine was given to facilitate tracheal intubation. Anesthesia was maintained with halothane and nitrous oxide. Each child was randomly assigned to receive either no antiemetic prophylaxis (control), droperidol 0.075 mg/kg, or dixyrazine 0.25 mg/kg. The drugs were injected intravenously at the end of surgery. The incidence of vomiting during the fol…

Pulled-in-two syndrome in a young woman with restrictive strabismus and rare genetic mutation.

Pulled-in-two syndrome (PITS) is a rare but serious complication of strabismus surgery, with a reported incidence of once every decade among strabismus surgeons. It occurs more commonly in patients of advanced age and those with cranial nerve palsies or who have undergone prior surgeries. We present a case of PITS in a young female, diagnosed following surgery with a hereditary myosin myopathy.

Altered Spontaneous Brain Activity in Adolescents and Adults with Comitant Strabismus and Strabismic Amblyopia.

PURPOSE: Neuroimaging studies have established that strabismus and amblyopia are associated with structural and functional brain alterations. However, differences in spontaneous brain activity between strabismus patients with and without amblyopia remain unclear. This study employed fractional amplitude of low-frequency fluctuation (fALFF), a normalized version of the amplitude of low-frequency fluctuation that reduces physiological noise and improves detection of spontaneous brain activity, to examine spontaneous brain activity patterns in comitant strabismus (CS) and strabismic amblyopia (SA…

Pilot comparison of monocular and binocular modes of virtual reality field testing in strabismus.

Virtual reality field (VRF) perimetry is a promising new tool for visual field assessments, offering both monocular and binocular testing modes. We aimed to examine whether ocular misalignment, known as strabismus, influences VRF outcomes between monocular and binocular testing modes, which is currently unknown. Children with non-amblyopic horizontal strabismus were enrolled and completed VRF testing (Olleyes VisuALL) in either the game-based or standard Humphrey visual field (HVF)-equivalent algorithm. Within their respective algorithms, participants completed tests in both monocular and bino…

Factors associated with the use of botulinum toxin injections for adult strabismus in the IRIS Registry.

This cross-sectional study used data from a large nationwide registry to describe the factors associated with use of botulinum toxin injections for adults with strabismus in the United States. Botulinum toxin injections were performed on 3.1% of adults undergoing an intervention for strabismus between 2013 and 2020. Adults treated with botulinum toxin injections were more likely to be older and female. Compared to non-Hispanic White patients, non-Hispanic Black patients were three times less likely to receive treatment with botulinum toxin after adjusting for age, sex, geographic region, and t…

Combined recession-resection in the management of paralytic strabismus: simplification of the complex (a case report).

Oculomotor nerve palsy presents the greatest challenge in clinical treatment among the other cranial nerves that innervate extraocular muscl … section on his right eye, with a promising result after a 3-week follow-up. When treating paralytic strabismus resulting from isolated oculomotor nerve palsy, a combination of medial rectus resection and lateral rectus recession yields good results and long-term stability.

Modified Target Angle as a Predictor of Success in Strabismus Management after Orbital Fracture.

Surgery for strabismus secondary to orbital fracture reconstruction surgery has had low success rates and high reoperation rates due to its incomitant nature and complex underlying mechanisms. There has been no consensus as to which of the various methods for improving the surgical results are best. We proposed a modified target angle criteria that combined the regular target angle and a favorable Hess area ratio percentage (HAR%) threshold to evaluate surgical results within the first postoperative week and conducted a retrospective chart review. According to the criteria of the modified targ…

Evolving trends in strabismus following retinal surgery: is there still a role for botulinum toxin?

Purpose Ocular motility disturbances following retinal detachment surgery are well documented, resulting in ocular misalignment and disabling diplopia. Interestingly, there has been a downward trend over the last decade across the United Kingdom with the evolution of refined vitrectomy techniques and instrumentation. We aim to report our updated experience on factors influencing botulinum toxin outcomes in view of the trend toward vitrectomy. Methods The Moorfields strabismus service carried out a follow-up retrospective study of all subjects that received botulinum toxin for retinal surgery-r…
