A study on entrainment in various forms of tachycardias.

In order to study the entrainment phenomenon in various forms of clinical and experimental tachycardia, overdrive pacing was performed in clinical paroxysmal supraventricular tachycardia (PSVT) of Wolff-Parkinson-White syndrome (6 cases), atrial flutter (AF-6 cases), ventricular tachycardia (VT-2 cases), experimental AF induced by Rosenblueth's method (5 dogs), and in VT induced by aconitine (5 dogs). Progressive fusion was demonstrated in all but aconitine-induced VT. After cessation of pacing, the first tachycardia complex showed no fusion at all, but the timing of the complex varied dependi

Bipolar ablation of refractory atrial and ventricular arrhythmias: Importance of temperature values of intracardiac return electrodes.

INTRODUCTION: Bipolar radiofrequency catheter ablation (Bi-RFCA) emerged as an option for treatment of arrhythmias resistant to the conventional approach. Data on safety issues of Bi-RFCA, including temperature values of intracardiac return electrode (IRE) are lacking. OBJECTIVE: To determine the safety profile of Bi-RFCA regarding temperature measurements obtained from nonirrigated IRE of different sizes. METHODS: The study group consisted of consecutive patients after failed conventional RFCA who underwent Bi-RFCA. RESULTS: Out of 1510 RFCA performed in our center, 19 patients underwent B

Determinants of Left Atrial Thrombus in Patients Undergoing Catheter Ablation for Atrial Fibrillation and Atrial Flutter.

BACKGROUND: Catheter ablation (CA) is a first-line therapy for rhythm control in patients with atrial fibrillation (AF) and atrial flutter (AFL). Left atrial thrombus (LAT) is an absolute contraindication to CA due to the increased risk of periprocedural ischemic complications. A recent consensus statement from the European Heart Rhythm Association (EHRA) recommended the exclusion of LAT before CA in patients receiving oral anticoagulant therapy (OAT) for less than three weeks or in patients at high risk, despite the substantial reduction in the rate of periprocedural stroke achieved by antico

A new and simple method for distinguishing complete from incomplete block through the cavotricuspid isthmus.

BACKGROUND: A complete line of block (CLOB) in the cavotricuspid isthmus (CTI) is the endpoint of typical atrial flutter ablation. Before CTI block is obtained, a progressive CTI conduction delay due to an incomplete line of block (InLOB) can be difficult to distinguish from CLOB. The purpose of this study was to assess a new simple approach based on the changes in atrio-ventricular (AV) conduction delays during septal and lateral right atrial pacing, to distinguish a CLOB from an InLOB during typical atrial flutter (AFL) ablation. METHODS AND RESULTS: Forty patients who presented an InLOB be

Effectiveness of Implanted Cardiac Rhythm Recorders With Electrocardiographic Monitoring for Detecting Arrhythmias in Pregnant Women With Symptomatic Arrhythmia and/or Structural Heart Disease: A Randomized Clinical Trial.

IMPORTANCE: Arrhythmias are an important cause of maternal morbidity and mortality but remain difficult to diagnose. OBJECTIVE: To compare implantable loop recorder (ILR) plus 24-hour Holter electrocardiographic (ECG) monitoring with standard 24-hour Holter ECG monitoring alone in terms of acceptability, ability to identify significant arrythmias, and effect on management and pregnancy outcome in women who were symptomatic or at high risk of arrythmia because of underlying structural heart disease. DESIGN, SETTING, AND PARTICIPANTS: This single-center, prospective randomized clinical trial r

Severe sepsis and cardiac arrhythmias.

BACKGROUND: Although sepsis is a known risk factor for cardiac arrhythmias, data on incidence and outcomes of atrial and ventricular arrhythmias in severe sepsis is limited. The purpose of this study is to examine the association of severe sepsis with cardiac arrhythmias and their impact on outcome in this patient population. METHODS: We used hospital discharge data from the Nationwide Inpatient Sample (NIS) during the year 2012. All patients 18 years or older were included in the study. The International Classification of Diseases-Clinical Modification, 9th revision (ICD-9-CM) code was used

The epidemiology of arrhythmia in infants: a population-based study.

AIM: Cardiac arrhythmias are an important cause of morbidity in infants. Although the spectrum of types of arrhythmia has been reported, there has been no previous population-based study of the incidence of arrhythmias in infancy. Our aim was to define the population incidence of arrhythmia in infants. METHODS: We based this study on the Northern Region of England with a resident population of 3.1 million and an annual live birth rate of 33,000. We identified all clinically significant arrhythmias in infants in 1991-2010 from the regional cardiac database. All diagnoses were based on analysis

[The best of congenital heart disease in 2006].

As in previous years, the end of 2005 and the year 2006 were very fruitful in publications on congenital heart disease in children and adults. Interventional cardiology is still the object of randomised and non-randomised trials in adults and children. The closure of the foramen ovale is still popular in the context of a cerebrovascular accident whether or not associated with migraine. Several articles have studied the percutaneous closure of atrial septal defects: different devices, different indications, complications, comparison with surgical closure. Some ventricular septal defects can be

[Subarachnoid hemorrhage with pulmonary edema and electrocardiographic changes. The differential diagnosis of myocardial infarct].

A 32-year-old man (weight 132 kg, height 190 cm) suddenly became unconscious and cyanosed with an unrecordable pulse and ventricular flutter on ECG. After resuscitation, the blood pressure was 200/100 mm Hg; the patient moved his arms and legs at times, but he did not regain consciousness. Focal neurological signs and meningism were not demonstrable. Subsequent ECGs showed a raised ST segment, followed later by terminal T wave inversion; marked pulmonary oedema was present clinically and radiologically. The creatine kinase activity was 344 U/l. As lateral myocardial infarction was suspected, t…

[Electrophysiological mechanisms of atrial tachyarrhythmia].

Both supraventricular and ventricular arrhythmias are generated by one of the following 2 mechanisms: increased automaticity and reentry. In this paper we will refer about the mechanisms of the commonest atrial tachyarrhythmias. Atrial parasystole: it is determined by an automatic focus protected by entrance block. In some cases the focus is only partially protected owing to a partial block or to the electrotonic influence of the sinus rhythm. Atrial tachycardia: it can be due both to increased automaticity and to reentry. Atrial flutter: it is generated by a counterclockwise macroreentry loca

Safety and Effectiveness of Pulsed Field Ablation to Treat Atrial Fibrillation: One-Year Outcomes From the MANIFEST-PF Registry.

BACKGROUND: Pulsed field ablation is a novel nonthermal cardiac ablation modality using ultra-rapid electrical pulses to cause cell death by a mechanism of irreversible electroporation. Unlike the traditional ablation energy sources, pulsed field ablation has demonstrated significant preferentiality to myocardial tissue ablation, and thus avoids certain thermally mediated complications. However, its safety and effectiveness remain unknown in usual clinical care. METHODS: MANIFEST-PF (Multi-National Survey on the Methods, Efficacy, and Safety on the Post-Approval Clinical Use of Pulsed Field A

Transthoracic DC shock may represent a serious hazard in pacemaker dependent patients.

External defibrillation is widely used for the termination of various atrial and ventricular tachyarrhythmias, including pacemaker patients. Our study was intended to evaluate the effects of DC shocks in 36 patients with unipolar pacemakers implanted in the right pectoral region (25 DDD, 10 VVI, 3 AAI). The shocks were delivered with paddles on the anterior surface of the thorax, as far as possible away from the pacemaker. The pacing output was programmed at 0.5 msec and 5 V (25 patients), 4 V (1 patient), and 2.5 V (10 patients). Transient loss of capture occurred in 18 patients (50%). These

Automated external cardioversion defibrillation monitoring in cardiac arrest: a randomized trial.

BACKGROUND: In-hospital cardiac arrest has a poor prognosis despite active electrocardiography monitoring. The initial rhythm of approximately 25% of in-hospital cardiopulmonary resuscitation (CPR) events is pulseless ventricular tachycardia/ventricular fibrillation (VT/VF). Early defibrillation is an independent predictor of survival in CPR events caused by VT/VF. The automated external cardioverter defibrillator (AECD) is a device attached by pads to the chest wall that monitors, detects, and within seconds, automatically delivers electric countershock to an appropriate tachyarrhythmia. [Inadequate therapies with implantable cardioverter-defibrillators--incidence, etiology, predictive factors and preventive strategies].

Patients with implantable cardioverter defibrillators (ICD) often suffer inappropriate ICD-therapies. The incidence, causes and risk factors of ICD-therapies for a rhythm other than ventricular tachyarrhythmias (VT) were determined retrospectively in 462 consecutive patients (pts). Inappropriate ICD-therapies were identified based on stored R-R intervals and/or electrograms. Eighty-two pts (18%) had inappropriate ICD-therapies. Actuarial rates for inappropriate ICD-therapies were 13%, 20%, 24% and 29% at 1, 2, 3 and 4 years after ICD-implantation, respectively. Atrial fibrillation with rapid v

Long-term sotalol therapy in patients with arrhythmias.

Sotalol is a pure beta-adrenergic receptor antagonist. The present study was divided into a first and second therapy period. A total of 38 patients with atrial or ventricular arrhythmias were included in the first therapy period. After a drug-free period of approximately two months, 14 of the 38 patients entered the second therapy period and were given oral sotalol. During the two treatment periods, oral sotalol was given in doses ranging from 40 to 480 mg/day for 0.5 to 11 months in the first period and for four to nine months in the second period. Oral sotalol decreased or abolished arrhythm

Catheter ablation in congenital heart diseases: a French nationwide study.

BACKGROUND AND AIMS: Current evidence on catheter ablation for patients with congenital heart disease (CHD) is derived from small, retrospective studies. This study aims to provide insights from a nationwide contemporary registry. METHODS: This prospective study included all CHD patients referred for catheter ablation from 2020 to July 2024 across 28 French centres. The primary outcome was the rate of per-procedural acute success. Secondary outcomes included complications as well as freedom from arrhythmia recurrence. RESULTS: A total of 1135 consecutive catheter ablation procedures were per
