A comparative study utilizing finite element modeling to analyze the biomechanical differences in various cement distributions within osteoporotic vertebral compression fractures.

This study aims to assess the biomechanical impact of bone cement perfusion on osteoporotic vertebral compression fractures across various distribution sites through finite element simulation. The finite element model of the L1-L3 vertebral body was constructed and its efficacy was validated. Simulated compression fractures of the L2 vertebral body resulted in the distribution of cement in the anterior, middle, and posterior thirds of the L2 vertebral body. Physiological loading and boundary conditions of axial compression experienced by the spine in the upright position were established, and …

Lumbar trabecular bone mineral density distribution in patients with and without vertebral fractures: a case-control study.

PURPOSE: The proportion of load transmitted through the lumbar neural arch increases with aging, spinal degeneration, and lordosis, effectively shielding the lumbar vertebral bodies from load. This stress shielding may contribute to bone loss in the vertebral body, leading to increased fracture risk. To test his hypothesis, we performed a study to determine if vertebral body fractures were associated with a higher neural arch/vertebral body volumetric bone mineral density (vBMD) ratio. METHODS: Trabecular vBMD was calculated by quantitative CT in the L3 vertebral body and neural arch (pars in…

Congenital vertebral anomalies and the possible aetiological relationship with spina bifida.

The frequency and distribution of hemivertebrae, fused vertebral bodies, fused vertebral arches and fused ribs were recorded in 376 children and young adults with either spina bifida cystica or occulta, or solitary vertebral body anomalies, or multiple vertebral body anomalies. An attempt was made to find evidence to support the hypotheses that multiple vertebral anomalies and anencephaly-spina bifida are aetiologically related, and that solitary vertebral body anomalies are sporadic and developmentally distinct. Statistical analysis of the distribution of the vertebral and rib defects partly …

Failure analysis of C-5 after total disc replacement with ProDisc-C at 1 and 2 levels and in combination with a fusion cage: finite-element and biomechanical models.

OBJECT The purpose of this study was to evaluate the failure risk of cervical vertebrae after total disc replacement with a keel-design prosthesis (ProDisc-C), taking into consideration the effects of vertebral body height, multilevel replacement, and the association with an adjacent fusion cage. Although promising clinical results have been reported for the ProDisc-C, some clinical studies have reported vertebral body-splitting fractures at single- and multilevel arthroplasty sites. This implant has central keels to provide solid initial stability, and some authors associate the potential ris…

[Pathology and Acute Phase Treatment of Osteoporotic Vertebral Fractures:Indications and Techniques for Vertebroplasty].

Vertebroplasty has shown excellent analgesic effects in patients with osteoporotic vertebral fractures. In Japan, percutaneous vertebroplasty, balloon kyphoplasty, and vertebral body stenting are commonly performed. All of these techniques require precise transpedicular vertebral puncture and complete cement filling without leakage. Puncture should only be performed after accurately identifying the upper and lower end plates of the vertebral body, lateral borders, and pedicles using combined frontal and lateral fluoroscopic images. In the frontal view, the puncture was made from the lateral cr…

[Assessment of Reduction Abilities of an Expandable Stent in Treating Osteoporosis-Related Fractures of Th/L (Thoracolumbar) Spine].

PURPOSE OF THE STUDY The method of stabilising the osteoporosis-related spinal fractures using an expandable stent (vertebral body stent - VBS) and bone cement is generally accepted for its minimal invasiveness, a low risk of complications and confirmed analgesic effect. The efficiency of reduction of a compressed vertebra is, however, still discussed in the literature. Our hypothesis was that the stent expansion in the vertebral body can achieve a statistically significant reduction in the anterior, middle and posterior part of the vertebra. MATERIAL AND METHODS The patients in whom the VBS t…

Understanding the effect of lumbar lordosis angle on vertebral load distribution during walking.

Atypical sagittal spinopelvic alignment is correlated with exacerbating lower back pain (LBP). This study investigated the effects of simulated sagittal spinopelvic alignment via altered lumbar lordosis (LL) on lumbar vertebral contact forces during walking. A full-body OpenSim model with custom lumbar joints was developed to estimate lumbar vertebral loads for self-selected speed walking gaits of 18 healthy participants. Limited LL during walking augmented the resultant vertebral compressive and shear forces, and vertebral body compression. Excessive LL increased resultant vertebral shear for…

Biomechanics of vertebral compression fractures and clinical application.

Local biomechanical factors in the etiology of vertebral compression fractures are reviewed. The vertebral body is particularly vulnerable to compression fracture when its bone mineral density (BMD) falls with age. However, the risk of fracture, and the type of fracture produced, does not depend simply on BMD. Equally important is the state of degeneration of the adjacent intervertebral discs, which largely determines how compressive forces are distributed over the vertebral body. Disc height also influences load-sharing between the vertebral body and neural arch, and hence by Wolff's Law can …

Vertebroplasty and kyphoplasty: a comparative review of efficacy and adverse events.

Vertebroplasty and kyphoplasty have become common surgical techniques for the treatment of vertebral compression fractures. Vertebroplasty involves the percutaneous injection of bone cement into the cancellous bone of a vertebral body with the goals of pain alleviation and preventing further loss of vertebral body height. Kyphoplasty utilizes an inflatable balloon to create a cavity for the cement with the additional potential goals of restoring height and reducing kyphosis. Vertebroplasty and kyphoplasty are effective treatment options for the reduction of pain associated with vertebral body …
