Paradigm of Sudden Death Prevention in Hypertrophic Cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is a worldwide genetic heart disease and a common cause of sudden death in the young. Penetration of the implantable cardioverter-defibrillator (ICD) into this patient population over the past 20 years has made accurate selection of patients for primary prevention ICDs a priority. Consequently, a new paradigm has emerged in the management of this complex disease with ICD therapy responsible for a substantial decrease in overall HCM-related mortality (to 0.5%/y) and independent of patient age. Selection of candidates for ICDs has matured substantially with the

Surgical options in benign parotid tumors: a proposal for classification.

Different surgical options are currently available for treating benign tumors of the parotid gland, and the discussion on optimal treatment continues despite several meta-analyses. These options include more limited resections (extracapsular dissection, partial lateral parotidectomy) versus more extensive and traditional options (lateral parotid lobectomy, total parotidectomy). Different schools favor one option or another based on their experience, skills and tradition. This review provides a critical analysis of the literature regarding these options. The main limitation of all the studies i

Moving forward from Cockcroft-Gault creatinine clearance to race-free estimated glomerular filtration rate to improve medication-related decision-making in adults across healthcare settings: A consensus of the National Kidney Foundation Workgroup for Implementation of Race-Free eGFR-Based Medication-Related Decisions.

PURPOSE: The goals of this paper are to (1) provide evidence and expert consensus to support a unified approach to estimating kidney filtration in adults with stable kidney function using race-free estimated glomerular filtration rate (eGFR) in place of Cockcroft-Gault estimated creatinine clearance (C-G eCrCL) for medical and medication-related decisions, and (2) demonstrate how adjusting eGFR results for an individual's body surface area (BSA) when it is higher or lower than 1.73 m2 will improve results for medication-related decisions. SUMMARY: C-G eCrCL is predominantly used by US pharmac

The Development and Usability of the AMPREDICT Decision Support Tool: A Mixed Methods Study.

OBJECTIVE: Amputation level decision making in patients with chronic limb threatening ischaemia is challenging. Currently, evidence relies on published average population risks rather than individual patient risks. The result is significant variation in the distribution of amputation levels across health systems, geographical regions, and time. Clinical decision support has been shown to enhance decision making, especially complex decision making. The goal of this study was to translate the previously validated AMPREDICT prediction models by developing and testing the usability of the AMPREDIC

Pancreatic neuroendocrine tumors - Bridging knowledge gaps for better outcomes.

Clinical decision-making in patients with pancreatic neuroendocrine tumors is complex. The absence of effective biomarkers and the reliance on morphological criteria underlying current predictive models expose patients to a substantial risk of overdiagnosis and overtreatment. The implementation of non-invasive tools that improve the precision of prognostic estimates remains an unmet clinical need.

How variants of uncertain significance affect clinical decisions: A systematic review.

PURPOSE: To assess the extent to which variants of uncertain significance (VUS) affect clinical decisions. METHODS: We conducted a systematic review of studies reporting the impact of VUS on clinical decisions. We extracted quantitative data on VUS' impact for grouped analysis. We calculated and compared odds ratios of the specified decisions for patients found to have benign results, VUS, and pathogenic results. RESULTS: We included 45 articles in our overall review and 21 in our quantitative analysis. The overall review (n = 45) showed that VUS affect at least some care decision in 67% of

[Exploring the Entrustment Decision Experiences of Preceptors Regarding New Nurses' Independent Patient Care].

BACKGROUND: Within the framework of competency-based medical education, determining whether new nurses are capable of making patient care decisions independently is a critical issue in clinical nursing education. However, there is a lack of in-depth exploration of clinical preceptor experiences related to entrustment decision-making in this context. PURPOSE: This study was designed to explore the experiences of nursing preceptors in terms of developing the confidence necessary in new nurses to make independent clinical decisions during the preceptorship process. METHODS: A qualitative resear

Decision-Making and Management of Acute Isolated Syndesmosis Injuries in Athletes.

BACKGROUND: The ankle joint is one of the most frequently injured joints in the athletic population. Some articles estimate that it accounts for approximately 40% of all sports injuries, with lateral ankle sprains accounting for approximately 85% of injuries whereas medial ankle and syndesmosis injuries account for approximately 4-5% and 1-11%, respectively.1-4 Incidence of ankle sprains has been reported to be as high as 7 per 1,000 exposures for court sports and 1 per 1,000 exposures for field sports with lateral ankle sprains being more common than medial or syndesmotic ankle inju-ries.5 Wh

Beyond "futility thresholds": An ethical framework for massive transfusion decisions.

BACKGROUND: Massive transfusion can be lifesaving and resource-intensive, undertaken under time pressure, prognostic uncertainty, and variable system capability. Recent literature suggesting unit-based "futility thresholds" risks translating population-level mortality correlations into bedside stopping rules. STUDY DESIGN AND METHODS: This essay distinguishes three ethically distinct rationales commonly conflated under "futility": (1) physiologic futility-inability to reach the intended clinical goal; (2) potentially inappropriate treatment-contestable judgments about benefits, burdens, and v

AI-assisted decision-making in mild traumatic brain injury.

OBJECTIVE: This study evaluates the potential use of ChatGPT in aiding clinical decision-making for patients with mild traumatic brain injury (TBI) by assessing the quality of responses it generates for clinical care. METHODS: Seventeen mild TBI case scenarios were selected from PubMed Central, and each case was analyzed by GPT-4 (March 21, 2024, version) between April 11 and April 20, 2024. Responses were evaluated by four emergency medicine specialists, who rated the ease of understanding, scientific adequacy, and satisfaction with each response using a 7-point Likert scale. Evaluators were…

Approach to the Patient Considering Long-term Antithyroid Drug Therapy for Graves' Disease.

Antithyroid drugs (ATD) are the treatment of choice for the majority of patients with Graves' hyperthyroidism worldwide. However, relapse of hyperthyroidism after withdrawal of arbitrarily chosen conventional 12 to 18 months of therapy is very common. In the last 2 decades, many studies have shown that treatment with long-term ATD (LT-ATD) is effective and safe in the maintenance of euthyroidism. In addition, it has been reported that serum TSH receptor antibody may not decrease permanently before 5 to 6 years of ATD treatment, and clinical trials have shown that ≥5 years of ATD treatment is a

Ethical Challenges in the Care of the Trauma Patient.

A patient with trauma presents a unique and/or complex challenge to the ethical foundation that guides nursing care. Patients with trauma, by the very nature of the suddenness of their injury, are unable to predetermine or express their wishes in the event of a catastrophic injury. The providers who care for patients with trauma do not have an established patient relationship to aid them in decision-making based on what they think the patient would wish or based on past conversations. Yet, they provide expert care and use ethical principles to direct their professional responsibility to these

Experimental evidence for structured information-sharing networks reducing medical errors.

Errors in clinical decision-making are disturbingly common. Recent studies have found that 10 to 15% of all clinical decisions regarding diagnoses and treatment are inaccurate. Here, we experimentally study the ability of structured information-sharing networks among clinicians to improve clinicians' diagnostic accuracy and treatment decisions. We use a pool of 2,941 practicing clinicians recruited from around the United States to conduct 84 independent group-level trials, ranging across seven different clinical vignettes for topics known to exhibit high rates of diagnostic or treatment error …

Clinical controversies in abdominal sepsis. Insights for critical care settings.

Sepsis is a deadly condition in which the outcome is associated with prompt and adequate recognition, intensive supportive care, antibiotic administration and source control. This last item makes abdominal sepsis a unique treatment challenge. Although pneumonia constitutes the leading cause of sepsis, abdominal sepsis has unique features that merit discussion. The abdomen may be implicated as the primary occult, secondary dependent or secondary independent source of infection. The major factors determining whether a patient will develop an uncomplicated infection or septic shock are: (1) patie
