[Complete urethral duplication with dorsal epispadiac urethra: case report].

Urethral duplication is a rare congenital anomaly with variable clinical presentation. A number of theories have been proposed to describe the embryology of the condition; the actual mechanism of the disorder is still unclear. We present a 2-year-old boy with complete urethral duplication and two functional sphincters. Functional and surgical results were satisfying with good continence.

Choanal atresia, CHARGE association, and congenital nasal stenosis.

Congenital bilateral narrowing or obstruction of the nasal airway can result in significant respiratory distress in the neonate, requiring emergency intervention. Some of these children have associated congenital malformations that may also impact on the management of the nasal anomaly. The embryology, presentation, and management of this spectrum of disorders are discussed, and the major surgical controversies are reviewed.

Congenital absence of incus--a rare abnormality.

A case is described of a nine-year-old girl with a bilateral and symmetrical congenital absence of incus. This very rare abnormality has been described only once previously in the literature. Although the pathology may be rare, treatment is straightforward and prognosis good. A discussion of middle ear embryology and a review of the literature is presented.

Ectomesenchymal mandibular symphysis bone graft: an improvement in alveolar cleft grafting?

Bone grafting the alveolar cleft in cleft lip and palate (CLP) patients is widely accepted. A traditional graft is the iliac crest. Other bone graft donor sites are briefly discussed. The ratio for an ectomesenchymal bone graft in alveolar cleft repair is explained. Aspects of the embryology, bone graft physiology, and reports on mandibular symphysis bone grafting are discussed.

Eagle's syndrome: a reappraisal.

W.W. Eagle in 1937 described two patients with elongated styloid processes, cervico-facial pain and a history of pharyngeal trauma. Selected case reports are used to illustrate the spectrum of Eagle's Syndrome. An analysis of the prevailing theories of etiology and causation is undertaken correlating anatomy, embryology, and physiology to derive a clearer understanding of Eagle's Syndrome.

[Supernumerary testis and homolateral varicocele. Apropos of a case, study of spermatogenesis and review of the literature].

Polyorchidism is a rare abnormality. A case is reported of a small scrotal supernumerary testis associated with a large varicocele. A torsion of this testis probably occurred and the vascular pedicle seemed atrophic. Despite abnormal fertility with oligozoospermy the accessary testis was removed. A review of relevant literature is made giving details of embryology of this anomaly.

[Ectopic ureter. Description of a clinical case with appearance of stress urinary incontinence].

A case of ectopic ureter in a 45 year old woman arising after the onset of stress incontinence is reported. The patient presented complete pyeloureteral duplication with an ectopic ureter whose orifice drained into the proximal urethra. The embryology, diagnostic and therapeutic problems of the case are discussed with emphasis on the tardive appearance of the symptoms.

Unusual variant of left superior vena cava.

The first computed tomography-documented case of a persistent left superior vena cava that drains directly into the right superior vena cava is presented. The venous embryology is reviewed and the differential diagnosis of other pertinent venous anomalies is discussed. The patient also had an incidental finding of a congenitally absent left lobe of the thyroid gland.

Primary cholesteatoma in the congenitally atretic ear.

A developmental aberration significant enough to cause a congenital auricular atresia may well entrap ectodermal keratinous elements medial to an atresia plate, resulting in a primary cholesteatoma. We report such a case and discuss the proposed pathogenesis and pertinent embryology. A two-stage surgical procedure is offered as a reasonable management alternative when the cholesteatoma is well encapsulated.

Congenital malformations of the mitral valve.

Congenital malformations of the mitral valve may be encountered in isolation or in association with other congenital heart defects. Each level of the mitral valve complex may be affected, according to the embryological development, explaining the fact that these lesions are sometimes associated with each other. As a perfect preoperative assessment is of importance, good knowledge of both normal and abnormal anatomy is required in order to guide the surgeon accurately. This review presents the different embryological, anatomical and echocardiographic aspects of the congenital mitral anomalies.

Tracheal occlusion for fetal diaphragmatic hernia.

Despite advances in neonatal care, diaphragmatic hernia still inflicts significant morbidity and mortality on affected neonates. Abnormal embryologic events disrupt the formation of the diaphragm allowing the abdominal viscera to occupy the intrathoracic space. This interrupts normal pulmonary development with resulting pulmonary hypoplasia and pulmonary hypertension in neonatal survivors. This review will outline the relevant embryology, etiologies, and pertinent historical aspects of diaphragmatic hernia treatments to better understand the current antenatal approach to therapy for this disea…

Median raphe cysts of the perineum in children.

Median raphe cysts of the perineum are uncommon congenital lesions of the male genitalia. They can be found all the way from the distal penis and scrotum toward the perineum in a midline position. They are considered as congenital alterations in embryologic development. A case of a 6 year-old boy is presented. Review of the literature relevant to children was made regarding the embryologic, diagnostic, and treatment aspects. We believe it is important that adult and pediatric urologists recognize these lesions and their management to provide the appropriate information to the parents.

Recruiting and cultivating future leaders in laboratory science.

PURPOSE OF REVIEW: Technological in-vitro fertilization (IVF) advancements originate in the embryology laboratory, and are accompanied by increased regulatory oversight and risk management. Stakes have never been higher or the need greater for the recruitment and cultivation of leaders in laboratory science to navigate the direction of IVF. Current thought leaders in state-of-the-art laboratories must prioritize this mission to optimize and preserve the future of IVF. RECENT FINDINGS: Leaders in laboratory science must be able to speak to patients, the lay public, business leaders, scientific…

Valvular heart disease in congenital heart disease: a narrative review.

Patients with congenital heart disease (CHD) are one of the fastest growing populations in cardiology, and valvular pathology is at the center of many congenital lesions. Derangements in valvular embryology lead to several anomalies prone to dysfunction, each with hemodynamic effects that require appropriate surveillance and management. Surgical innovation has provided new treatments that have improved survival in this population, though has also contributed to esotericism in patients who already have unique anatomic and physiologic considerations. Conduit and prosthesis durability are often m…

Does moderate celiac axis stenosis identified on preoperative multidetector computed tomographic angiography predict an increased risk of complications after pancreaticoduodenectomy for malignant pancreatic tumors?

OBJECTIVES: To investigate embryology of complete and incomplete pancreas divisum (PD). METHODS: We analyzed anatomy of the pancreatic duct system in 44 cases of complete PD and 41 cases of incomplete PD. Under the concept of branch fusion between the ventral and dorsal pancreatic ducts, incomplete PD was divided into 3 types: fusion of the extreme end of the upper branch of the ventral pancreatic duct with the dorsal pancreatic duct (type 1); fusion of the lower branch of the dorsal pancreatic duct with the lower branch of the ventral pancreatic duct (type 2); and fusion of the lower branch …
