Histopathology of sudden hearing loss.

Eleven temporal bones from eight patients who had clinical histories of sudden hearing loss (SHL) were studied to assess the possible etiopa … ogic findings causing SHL were observed even in cases with the same etiology. A case of SHL showing endolymphatic hydrops as the main histopathologic finding is described.

Untargeted Metabolomic Analysis in Endolymphatic Sac Luminal Fluid from Patients with Meniere's Disease.

Dysfunction of the endolymphatic sac (ES) is one of the etiologies of Meniere's disease (MD), the mechanism of which remains unclear. The ai … he ELF of the patients with MD, suggesting the elevated endolymphatic Ca2+ in the ears with chronic endolymphatic hydrops is likely to be associated with the reduction of these two chelators of Ca2+ in ES. The results in the present study indicate metabolomic analysis in the ELF of the patients with MD can potentially improve our understanding on the molecular pathophy …

Comparative clinical features of unilateral and bilateral Ménière's disease.

BACKGROUND: Ménière's disease (MD) is an inner ear disorder, primarily unilateral but sometimes bilateral. Their comparative clinical profiles are not fully defined. OBJECTIVE: To compare the clinical characteristics of unilateral Ménière's disease (UMD) and bilateral Ménière's disease (BMD). MATERIAL AND METHODS: A retrospective analysis of UMD and BMD patients was conducted. Statistical analyses included the Wilcoxon rank-sum test, Spearman's correlation, and Fisher's exact test to compare clinical parameters and their associations with endolymphatic hydrops (EH). RESULTS: In UMD, vertigo…

Longitudinal endolymph movements induced by perilymphatic injections.

Endolymph movements and endocochlear potential (EP) changes were measured during disturbances of perilymphatic pressure. induced by injecting artificial perilymph into scala tympani (ST) or scala vestibuli (SV) of the guinea pig cochlea. Injections were performed either with or without an outlet made in the opposite perilymphatic scala. Injections into ST without an outlet induced large pressure changes but virtually no endolymph movement or EP change. Injection at the same rate into ST with an outlet in SV produced smaller pressure changes which were accompanied by a basally-directed displace

Possible involvement of free radicals in lipopolysaccharide-induced labyrinthitis in the guinea pig: a morphological and functional investigation.

Intratympanic injection of bacterial lipopolysaccharide impaired caloric responses and caused severe and widespread morphological damage to vestibular end organs and the endolymphatic sac in the guinea pig. These effects could be blocked with N-nitro-L-arginine methylester, a competitive inhibitor of nitric oxide synthase, with superoxide dismutase, an O2 scavenger, with dexamethasone, and with ebselen, a scavenger of peroxynitrite. These observations indicate that enhanced nitric oxide and superoxide production, resulting in formation of peroxynitrite, is probably an important factor responsi

Dietary Restriction for The Treatment of Meniere's Disease.

Meniere's disease (MD) is an idiopathic inner ear disorder characterized by spontaneous recurrent vertigo, fluctuating sensorineural hearing loss (SNHL), aural fullness and tinnitus. Endolymphatic hydrops (EH) of the inner ear is currently considered the pathophysiological mechanisms that underlies typical symptoms of MD. MD diagnosis is based on the criteria of the Baràny Society. There are many therapeutic options for MD, but none is considered effective by the scientific community. The first-line treatment commonly includes dietary modification, as low salt diet and reduction of alcohol and…

In Vivo Morphometric Analysis of Human Cranial Nerves Using Magnetic Resonance Imaging in Menière's Disease Ears and Normal Hearing Ears.

Analysis of neural structures in Menière's Disease (MD) is of importance, since a loss of such structures has previously been proposed for t … quence (3D-CISS). In the patient group, MD was additionally confirmed using MRI-based assessment of endolymphatic hydrops. Morphometric analysis was performed using a freeware DICOM viewer. Evaluation of cranial nerves included measurements of cross-sectional areas (CSAs) of the nerves at different levels as well as orthogonal diametric measurements.

Meniere's disease: update of etiopathogenetic theories and proposal of a possible model of explanation.

Meniere's Disease (MD) is an affection consisting of an association of sensorineural hearing loss, tinnitus and vertigo initially presenting by crises. A review of the most considered possible causative factors and pathophysiologic interpretations allows us to underline the uncertainties which still exist about the genesis of this illness. We propose a mechanistic model based on the effect of a haemodynamic imbalance leading to transient ischaemia which could have an effect on the pH of the inner ear as well as on the work of the inner ear proton pumps. It is hypothesized that under ischaemic

Diagnoses in a dizziness unit.

Data are given on 2,515 patients who attended the Dizziness Unit on 3,113 visits. The sex incidence was 58% female, 42% male, and the sex pr … es. Obviously, they deserve more clinical attention than such relatively rare conditions as delayed endolymphatic hydrops (0.5%) or inner ear fistula (0.4%). For all diagnostic categories, on repeated visits the original diagnosis was changed in about one-third of cases. Diagnostic conversion was most frequent in "undiagnosed", at a level of 58%.

Regional difference in susceptibility to damage of vestibulo-semicircular canals in experimental labyrinthine lesion.

In the study on regional difference in susceptibility to damage of vestibulo-semicircular canals upon causing labyrinthine lesion by four different procedures, namely, by intracranial approach, through the middle ear, through the facial nerve and by means of experimental endolymphatic hydrops, the following results were obtained. 1. The crista of posterior semicircular canal among the semicircular canals and the macula of the saccule within the vestibule were most susceptible to impairment. In other words, the most susceptible region was the so-called pars inferior. In regard to the impairment…

Allergic management of Meniere disease.

Twenty-eight patients with Meniere disease were selected on the basis of a positive allergic history. Factors other than allergy that might produce the signs and symptoms of endolymphatic hydrops were first diagnosed and controlled with appropriate management. Allergic diagnosis and management consisted of testing and treating inhalant allergies according to the methods of Rinkel. Foods were first tested using the Bryant's modification of Black's cytotoxic food test. All positive foods were eliminated except for those considered extremely difficult to avoid. These were managed using the provoc…

[Oxygen therapy in the long term treatment of Meniere's disease].

Endolymphatic hydrops is the hystopathological substrate characteristic of Ménière's disease. Besides the classical treatment with diuretics and/or osmotic drugs for some time, now treatment in a "pressure chamber" (OTI) has also been applied. The oxygen administered in the hyperbaric chamber can reduce the hydrops both by increasing the hydrostatic pressure and by mechanically stimulating the flow of endolymph toward the duct and endolymphatic sac. In addition, an increase is seen in the amount of O2 dissolved in the labyrinthine fluids and this contributes to recovering cell metabolism and r…

Preliminary study on cervical vestibular-evoked myogenic potentials tuning changes in acute low-tone sensorineural hearing loss.

BACKGROUND: Acute low-tone sensorineural hearing loss (ALHL) is frequently associated with endolymphatic hydrops. Cervical vestibular-evoked myogenic potentials (cVEMP) have been used to detect saccular hydrops, especially in Meniere's disease, but their role in ALHL is unclear. OBJECTIVE: To preliminarily investigate whether cVEMP tuning frequency shifts are associated with hearing outcomes in patients with ALHL. METHODS: This retrospective study analyzed medical records from 29 patients diagnosed with unilateral ALHL between 2016 and 2021. cVEMP testing utilized 500 Hz and 1000 Hz tone bur…

DNAJC14 Ameliorates Inner Ear Degeneration in the DFNB4 Mouse Model.

The His723Arg (H723R) mutation in SLC26A4, encoding pendrin, is the most prevalent mutation in East Asia, resulting in DFNB4, an autosomal r … KCNJ10 expression was increased with DNAJC14 overexpression; however, hearing function and enlarged endolymphatic hydrops were not recovered. These results indicate that DNAJC14 overexpression ameliorates the cochlear degeneration caused by misfolded pendrin and might be a potential therapeutic target for DFNB4.
