Severe Corneal Morphological Alterations after Excimer Laser Surface Ablation for a High Astigmatism.

We report long-term alterations of anterior corneal stroma after excimer laser surface ablation for a high astigmatism. The patient claimed progressive visual loss in his right eye (RE) during the last 3 years after bilateral laser-assisted subepithelial keratectomy (LASEK) surgery. His examination comprised visual acuity (UDVA and CDVA), slit-lamp examination, corneal topography and tomography, AS-OCT, and confocal microscopy. The UDVA was 0.1 in his RE and 1.0 in the left eye. The CDVA in the RE was 0.8. The slit-lamp examination showed a stromal lesion in the inferior paracentral corneal zo…

Refractive surgery.

Refractive surgery has evolved beyond laser refractive techniques over the past decade. Laser refractive surgery procedures (such as laser in-situ keratomileusis), surface ablation techniques (such as laser epithelial keratomileusis), and photorefractive keratectomy have now been established as fairly safe procedures that produce excellent visual outcomes for patients with low-to-moderate amounts of ametropia. Additionally, a broader selection of options are now available to treat a wider range of refractive errors. Small incision lenticule extraction uses a femtosecond laser to shape a refrac…

Cultivated limbal epithelial transplantation and penetrating keratoplasty postchemical injury: a 14-year follow-up.

An 11-year-old girl presented to our tertiary eye care centre with a 9-month-old history of lime injury in the left eye in 1999 with vision of counting fingers close to the face. She initially underwent superficial keratectomy with amniotic membrane graft in 1999. Subsequently, cultivated limbal epithelial transplantation was performed in 2001 which improved her vision to 20/400. Following development of pannus and symblepharon in the left eye, she underwent pannus resection and conjunctival limbal autograft in 2002 and, a month later, optical penetrating keratoplasty (PK) following which her …

Conjunctival intraepithelial neoplasia with corneal furrow degeneration.

A 68-year-old man presented with redness of left eye since six months. Examination revealed bilateral corneal furrow degeneration. Left eye lesion was suggestive of conjunctival squamous cell carcinoma, encroaching on to cornea. Anterior segment optical coherence tomography (AS-OCT) confirmed peripheral corneal thinning. Fluorescein angiography confirmed intrinsic vascularity of lesion. Patient was managed with "no touch" surgical excision, dry keratectomy without alcohol, cryotherapy, and primary closure. Pathologic examination of removed tissue confirmed clinical diagnosis. Management of thi…

Mouse strain variation in SMA(+) myofibroblast development after corneal injury.

The purpose of present study was to investigate differences in myofibroblast development after haze-generating injury in different commonly used strains of mice. The inbred mouse strains used in this study were Balb/c, C57BL/6, C3H/HeJ and DBA/1J. All mice had uniform irregular phototherapeutic keratectomy with an excimer laser according to a previously published method to generate stromal haze. DBA/1J mice generated significantly greater density of alpha smooth muscle actin (SMA)-positive myofibroblasts in the anterior stroma compared to Balb/c (p < 0.05), C57BL/6 (p < 0.05) and C3H/HeJ (p < …

Compensatory epithelial hyperplasia in human corneal disease.

Compensatory hyperplasia of the corneal epithelium (CEH) has been observed histopathologically in animal and human eyes after excimer laser photoablative keratectomy, and has been implicated as a cause of variable refractive results and refractive regression after this procedure. Retrospective histopathologic analysis of routine keratoplasty specimens revealed CEH in 85 of 130 (65%) corneas with keratoconus, 18 of 36 (50%) corneas with chronic herpes simplex virus (HSV) keratitis, and 14 of 25 (56%) corneas coded as nonspecific scars. Mild CEH occurred apically and/or peripherally in keratocon…

[Correction of refractive errors by subtractive corneal surgery: History and perspectives].

The goal of this review is to trace the history of refractive correction, from the earliest optical devices to modern refractive surgical techniques. Following the invention of spectacles in the Middle Ages, the first corneal procedures, such as Sato's and Fyodorov's keratotomies and Barraquer's keratomileusis, highlighted the limitations of early mechanical approaches. In the 1980s, the advent of the excimer laser revolutionized refractive surgery with the introduction of PRK (photorefractive keratectomy) and subsequently LASIK (Laser-Assisted In Situ Keratomileusis). The development of the f…

Topical Losartan in Ophthalmology: Rationale, Experimental and Clinical Evidence, and Emerging Clinical Applications.

Corneal fibrosis remains a major cause of visual morbidity, with transforming growth factor-β (TGF-β) signaling playing a central role in th … neal fibrosis following diverse injuries such as descemetorhexis, alkali burns, and photorefractive keratectomy-related injury. Topical losartan represents a promising, non-cytotoxic antifibrotic strategy in ophthalmology, although human evidence is limited and further randomized controlled clinical trials are required to define its clinical efficacy, optimal …

Bilateral corneal perforation following post-PRK fungal keratitis managed with tenons patch graft.

A woman in her early 20s underwent bilateral photorefractive keratectomy (PRK) for correction of myopia. She developed bilateral central corneal melt 3 weeks post-operatively. There was no response to oral antivirals (for presumed necrotising stromal keratitis) and oral steroids (for presumed immune-mediated melt). She gradually developed pigmented corneal infiltrates and microbiology revealed a fungal infection. Despite intensive antifungal therapy, she developed corneal perforations. After applying tissue adhesive as a temporary measure, she was referred for further management. Bilateral Ten…

Comparison of clinical outcomes of LASIK, Trans-PRK, and SMILE for correction of myopia.

Transepithelial photorefractive keratectomy (Trans-PRK), laser-assisted in situ keratomileusis (LASIK), and small incision lenticule extraction (SMILE) are three mainstay refractive surgeries worldwide. The applicability, efficacy, safety, and predictability of these different techniques are quite similar. Trans-PRK has the strongest biostability, earliest return to normal corneal sensitivity but the longest recovery time, most uncomfortable postoperative experience, and possibility of corneal haze. LASIK possesses the fastest visual rehabilitation but the slowest corneal nerve reinnervation, …
