Prone versus Supine Position for Lung Ultrasound in Neonates with Respiratory Distress.

OBJECTIVE: To study the feasibility of lung ultrasound (LUS) in prone position and to compare it with supine position in neonates with respiratory distress. STUDY DESIGN: Neonates ≥ 29 weeks of gestational age with respiratory distress requiring respiratory support within first 12 hours of life were enrolled prospectively. First LUS (fLUS) was done in the position infant was nursed (supine or prone), infant's position changed, a second LUS (sLUS) was performed immediately and a third LUS (tLUS) was done 1 to 2 hours later. Primary outcome was the comparison of LUS scores (LUSsc) between fLUS

An hour-specific nomogram for transcutaneous bilirubin values in term and late preterm Hispanic neonates.

We sought to determine percentile values for hour-specific transcutaneous bilirubin (TcB) measurements in Hispanic neonates during the first 72 hours of age. Neonates with gestational age >or= 35 weeks and body weight >or= 2100 g were included. All neonates were screened with JM-103 TcB measurements at a minimum of every 24 hours by nursing personnel, and only TcB values obtained in Hispanic neonates with postnatal ages of 10 to 74 hours were analyzed. The 5th, 25th, 50th, 75th, and 95th percentile curves were determined. These data were compared with a previously published TcB nomogram predom

Elimination of bleeding associated with urinary catheterization in neonates.

Three cases of hematuria associated with urinary catheterization in infant males in this NICU over a year prompted an investigation by the health care team. The team tracked catheterizations in the unit, conducted a national survey, and did a literature review. Based on their findings, the team revised the unit policy and procedure and evaluated new products. After changes were implemented, the incidence of hematuria--originally at 20.7 percent--dropped to 5.5 percent during the first evaluation period and to zero during a second follow-up period.

The National Birth Center Study. Part II--Intrapartum and immediate postpartum and neonatal care.

Part II of a three-part report of the National Birth Center Study describes care provided to 11,814 women and their newborns during and after labor and delivery until they were transferred or discharged from the birth centers. There were few low birth weight or preterm or postterm births, but more macrosomic babies than among all U.S. births during the same time period. Certified nurse-midwives provided most of the intrapartum care, which is described in the context of medically recommended standards and data that describe care provided to low-risk women giving birth in U.S. hospitals. Birth c

Novel Specturi urine collector device demonstrating high measurement accuracy and proven clinical usability.

OBJECTIVES: Urine collection from neonates and infants remains challenging. Non-invasive urine collection methods (UCMs) are preferred, but they are often associated with drawbacks, such as feces contamination, discomfort, and/or inaccurate urinary analysis. This study aimed to demonstrate the measurement accuracy and clinical usability of the Specturi urine collector device. METHODS: 25 different urines were incubated in the Specturi urine collector device and three non-invasive UCMs, namely gauzes, urine collection bag, and PeeSpot, for 3 h at 37 °C. Albumin, calcium, chloride, creatinine,

The completion of partograms: knowledge, attitudes and practices of midwives in a public health obstetric unit in Bloemfontein, South Africa.

INTRODUCTION: most maternal and 24.3% of infant deaths occur during childbirth. Interventions during childbirth may reduce maternal and neonatal deaths. The Guidelines for maternity care in South Africa (2015) stipulates that all observations during labour should be recorded on a partogram. The objective of this study was to assess the knowledge and attitudes of nursing personnel and to evaluate their practices of completing partograms at National District Hospital, South Africa. METHODS: a two-phase, quantitative, cross-sectional, descriptive study design was used. In phase 1, the knowledge

[New parenting education in maternal child nursing].

Taiwan society is today typified by low birth rates amongst Taiwanese and a rising number of children born to immigrant and trans-cultural marriage families. Unhealthy behavior and anxiety on the part of pregnant women increase postpartum depression and complications and negatively affect neonatal heath. Such may further negatively impact upon romantic feelings between the new parents and the nascent parent-child relationship. New parenting education is a proactive and innovative strategy that may be used to improve maternal and child health. Therefore, it is worthy to explore how best to achi

Positioning after feedings: what is the evidence to reduce feeding intolerances?

Nursing’s goal is to be the least invasive as the patient's condition allows. As a NICU nurse, each feeding for our patients is a priority for them to grow and develop with the idea to speed discharge to home. When infants develop reflux or have increased gastric aspirates, we are concerned that their weight will taper and their length of stay will increase. Positioning provides nurses with a noninvasive option to care for these patients. The that the right-side position is best for infants to enhance digestion stems from theories of anatomy and physics. Research strengthens this supposition f

Periviability: A Review of Key Concepts and Management for Perinatal Nursing.

One of the most complex clinical problems in obstetrics and neonatology is caring for pregnant women at the threshold of viability. Births near viability boundaries are grave events that carry a high prevalence of neonatal death or an increased potential for severe lifelong complications and disabilities among those who survive. Compared with several decades ago, premature infants receiving neonatal care by today's standards have better outcomes than those born in other eras. However, preterm labor at periviability represents a more complex counseling and management challenge. Although preterm

SIADH: a case review.

Because the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) can cause neurologic sequelae with the potential to affect long-term outcomes, its prompt recognition and treatment are essential. Normally, antidiuretic hormone (ADH) is secreted when effective circulating blood volume is decreased. SIADH is marked by secretion of ADH in the presence of effective or normal circulating blood volume. This causes plasma hyponatremia simultaneously with plasma hypo-osmolality and inappropriate hyperosmolality of the urine. This article explains the pathophysiology of the syndrome; des

Adrenal insufficiency in the term and preterm neonate.

The adrenal gland is part of the endocrine system and produces hormones that are essential for survival. The adrenal cortex, the largest part of the adrenal gland, produces three major hormones. Glucocorticoids regulate metabolism of glucose, protein, and fat and allow the body to respond to stress by increasing blood glucose levels and cardiac output. Mineralocorticoids regulate fluid and electrolyte balance and help maintain blood pressure. Androgens are responsible for sexual differentiation in the fetus. Insufficient production of these hormones can occur for many reasons and can have a pr

Transient neonatal diabetes mellitus or an exaggerated steroid response? A case study.

Hyperglycemia is a common finding in the extremely premature neonate, especially during the first several days of life. Premature neonates have difficulty maintaining normal blood glucose levels in spite of normal or below-normal glucose infusion rates. Some infants experience intermittent episodes of hyperglycemia throughout the course of hospitalization that typically can be related to infection, medications, overinfusion of glucose, or episodes of stress. Transient neonatal diabetes mellitus (TNDM) is an uncommon diagnosis that is utilized when other reasons for the hyperglycemia cannot be
