Anatomical findings in patients with chronic rhinosinusitis without nasal polyps requiring revision surgery.

OBJECTIVES: For the treatment of chronic rhinosinusitis functional endoscopic sinus surgery is a well-established therapy with high initial success rates. However, a significant proportion of patients have persistent disease requiring revision surgery. To date, studies including data of large patient collectives are missing. In this study, we aimed to identify anatomic factors increasing the need for revision surgery in a large patient collective with chronic rhinosinusitis without nasal polyps. METHODS: Data were collected retrospectively on patients with recurrent or persistent chronic rhin

Correlation of quantitative MR imaging findings with symptoms in patients with incidentally detected inflammatory sinonasal disease.

PURPOSE: To assess the relationships between mucosal thickness, T1-weighted, T2-weighted signals and restricted diffusion on magnetic resonance imaging (MRI) with the degree of symptoms in patients with incidentally detected inflammatory sinonasal disease. MATERIALS AND METHODS: Conventional and diffusion-weighted MRI of 100 patients with incidental sinonasal mucosal thickening were prospectively evaluated. There were 53 men and 47 women, with a mean age of 44.6 years±15.17 (SD) (range: 18-81 years). Correlations between quantitative values (T1-signal, T2-signal and apparent diffusion coeffic

Efficacy of three-dimensional endoscopy in endonasal surgery.

OBJECTIVE: To examine the efficacy of a three-dimensional (3D) endoscope for endoscopic sinus and skull base surgery. METHODS: The study design was a retrospective case series and qualitative research. The clinical efficacy of 3D endoscopes was examined on five cadavers. We performed conventional endoscopic sinus surgery (ESS) in five cases and hypophysectomy in two cases using a 3D endoscope. The educational advantages of the 3D endoscope were assessed using questionnaires given to the participants of cadaver dissection courses. RESULTS: In the posterior portion of the nasal cavity, images

[Clinical, etiologic and therapeutic aspects of chronic sphenoid opacities].

A series of 23 patients with sphenoid disease were seen between 1992 and 1997. The most common symptom was headache. This headache was found to be nonspecific. Cranial nerve abnormalities were found in two patients. The different types of radiolographic images were analyzed, and compared with the surgical diagnosis. Twenty-one patients underwent surgical exploration by intranasal endoscopic sphenoidotomy. This approach allowed drainage and inspection of the sinus.

[Complications of endoscopic sinus surgery: incidence and prevention-analysis of 307 cases].

The complications of endoscopic sinus surgery performed on 307 patients with chronic inflammatory paranasal sinus diseases were analysed. The results showed that: 1. The common complications were excessive bleeding, orbit penetration, intra-orbital haemorrhage, postoperative stenosis of the maxillary ostium and middle turbinate adhesions. 2. The most common perioperative complications were those related to orbital penetration except excessive bleeding. A major complication was a case of cerebrospinal rhinorrhea with secondary purulent meningitis. 3. The annual overall incidences and the rates

Racial, Ethnic, and Gender Variations in Sinonasal Anatomy.

BACKGROUND: Complications during endoscopic sinus surgery often result from unfavorable anatomy. The prevalence rates of such anatomic findings vary tremendously in the literature, in part due to the small, homogenous populations previously studied. OBJECTIVE: To characterize the prevalence of sinonasal anatomic variants among ethnic groups and genders seen at an academic medical center. METHODS: This is a retrospective cross-sectional study of adult subjects who underwent CT imaging of the sinuses from January 2019 to November 2020 at a tertiary academic setting. CT scans were analyzed syst

Trigeminal neuralgia associated with sinusitis.

When a patient presents with trigeminal neuralgia, one usually thinks of a vascular loop at the root entry zone of the nerve and consequently of vascular decompression. An image of sinusitis on the MRI may be considered an incidental finding. We present a case of an elderly woman who experienced severe neuralgic pain in the distribution of the trigeminal nerve on the left side following a mild upper respiratory tract infection. Routine MRI revealed severe sinusitis with no pathology in the brain. Following antibiotic treatment for the sinusitis, the symptoms of the neuralgia resolved completel

Ectopic sphenoid sinus pituitary adenoma (ESSPA) with normal anterior pituitary gland: a clinicopathologic and immunophenotypic study of 32 cases with a comprehensive review of the english literature.

Ectopic sphenoid sinus pituitary adenoma (ESSPA) may arise from a remnant of Rathke's pouch. These tumors are frequently misdiagnosed as other neuroendocrine or epithelial neoplasms which may develop in this site (olfactory neuroblastoma, neuroendocrine carcinoma, sinonasal undifferentiated carcinoma, paraganglioma, melanoma). Thirty-two patients with ESSPA identified in patients with normal pituitary glands (intact sella turcica) were retrospectively retrieved from the consultation files of the authors' institutions. Clinical records were reviewed with follow-up obtained. An immunohistochemic

Endoscopic Management of Sinonasal Ossifying Fibroma: Clinical Experience From a Single Institution.

BACKGROUND: Sinonasal ossifying fibroma (OF) is a rare benign fibro-osseous tumor that can cause locally aggressive disease of the paranasal sinuses and skull base. Data on the outcomes after contemporary endoscopic management remain limited. METHODS: We retrospectively reviewed patients with pathologically confirmed sinonasal OF diagnosed at a tertiary referral center. Seven patients were included in this study. Four patients who underwent definitive endoscopic endonasal surgery with complete resection and postoperative follow-up were included in the outcome analyses. RESULTS: The lesions a

Subtle Manifestation of Lower Cavernous Sinus Syndrome as the Initial and Sole Presentation of Pterygopalatine Squamous Cell Carcinoma Invasion from the Skull Base: A Case Report.

We present a case highlighting the critical role of early recognition of partial cavernous sinus syndrome (CSS), as diagnostic delay can lead to irreversible tumor progression. A 50-year-old woman presented with subtle binocular diplopia and facial tingling sensation for 1 month. Neurological examination revealed isolated right abducens nerve (VI) palsy and paresthesia at territories of right ophthalmic (V1) and maxillary (V2) nerves. Partial CSS was suspected. Brain magnetic resonance image (MRI) revealed a contrast-enhancing mass involving the pterygopalatine fossa, sphenoid, and maxillary s

Meningothelial meningioma of the oculomotor nerve: A case report and review of the literature.

BACKGROUND: The origin of meningioma tumors is known as the meningothelial or arachnoid cap cells. The arachnoid granulations or villi are concentrated along with the dural venous sinuses in the cerebral convexity, parasagittally, and sphenoid wing regions. The majority of meningiomas are found in these locations with dural attachment. Infrequently, meningiomas develop without dural attachment but in dural adjacent. There are numerous reports of patients with cranial nerve involvement as a result of the compressive effect of the sinus cavernous or adjacent structures meningioma tumor on the cr

Bathtub drowning: An 11-year retrospective study in the state of Maryland.

A bathtub drowning is one of the leading causes of death in a bathtub. The purpose of this study was to evaluate how reliable the drowning-related signs could be for identifying a bathtub drowning in the cases of death in the bathtub. Performing a retrospective review of 92 deaths in the bathtub in Maryland, 71.7 percent were the presence of bathtub drowning and 28.3 percent were the absence of bathtub drowning. Three leading contributory causes of death were cardiovascular disease, drug/alcohol-related death, and seizure disorder in both groups. There was a statistically significant differenc
