Rheology of cystic fibrosis sputum after in vitro treatment with hypertonic saline alone and in combination with recombinant human deoxyribonuclease I.

Treatment with recombinant human deoxyribonuclease I (rhDNase) is currently used as therapy for cystic fibrosis (CF) lung disease. Hypertonic saline (HS) acts as an expectorant promoting mucus secretion and augmenting the volume of sputum. We evaluated the individual and combined effects of HS and rhDNase in vitro on the viscoelasticity of CF sputum. Sputum samples were collected from nine CF patients to use for in vitro testing. Aliquots of CF sputum (0.20 to 0.40 g) were subjected to the following protocols: (1) negative control sample without any treatment; (2) positive control sample, addi…

Galactomannan detection in sputum samples of patients with chronic obstructive pulmonary disease: A promising marker for diagnosis of chronic pulmonary aspergillosis?

BACKGROUND AND PURPOSE: Diagnosing chronic pulmonary aspergillosis (CPA) is challenging due to nonspecific symptoms, variable radiological findings, and limited mycological evidences. While galactomannan (GM) testing has been validated in serum and bronchoalveolar lavage fluid (BAL) for invasive pulmonary aspergillosis (IPA), its usefulness in sputum samples for CPA remains unclear. This study aimed to determine an appropriate GM cut-off level in sputum samples and its performance in diagnosis of CPA. MATERIALS AND METHODS: The diagnostic performance of sputum GM was evaluated in patients wit…

Diagnostic value of nonpurulent sputum in nontuberculous mycobacterial lung disease: A cross-sectional study based on the Miller and Jones classification.

BACKGROUND: Sputum quality is widely considered a determinant of microbiological diagnostic yield in respiratory infections. However, its significance in the context of nontuberculous mycobacterial (NTM) lung disease remains unclear. This study aimed to investigate the association between sputum quality, as assessed using the Miller and Jones classification, and the detection rates of NTM via smear microscopy, polymerase chain reaction (PCR), and culture. METHODS: We conducted a cross-sectional study of 54 patients diagnosed with NTM lung disease at a single institution in Japan between Septe…

Assessing the impact of individual exposure to air pollution via biomarkers in sputum of children in Ho Chi Minh city, Vietnam.

Understanding the impact of exposure to air pollution on children's health is critical for developing effective child health protection policies. Alveolar macrophage black carbon (AMBC) provides an indicator of personal exposure to air pollution. Hence, we aimed to examine air pollution exposure and its effects on children by measuring AMBC area and inflammatory cytokines in sputum. Sputum induction was attempted in 120 children aged 13-14 years, but samples of sufficient volume and quality for analysis were only obtained from 47 (39.2%). Alveolar macrophages (AMs) were visualized, and black c…

Acetic Acid Enables Molecular Enumeration of Mycobacterium tuberculosis from Sputum and Eliminates the Need for a Biosafety Level 3 Laboratory.

BACKGROUND: Improved monitoring of Mycobacterium tuberculosis response to treatment is urgently required. We previously developed the molecu … icidal to M. tuberculosis with no significant loss in 16S rRNA or an unprotected mRNA fragment when sputum was stored in acetic acid at 25°C for 2 weeks or -20°C for 1 year. This novel use of acetic acid allows processing of sputum for READ-TB at biosafety level 2 (BSL-2) on sample receipt. READ-TB is semiautomated and rapid. READ-TB correlated with the …

Non-sputum-based samples and biomarkers for detection of Mycobacterium tuberculosis: the hope to improve childhood and HIV-associated tuberculosis diagnosis.

In 2014, the World Health Organisation (WHO) published target product profiles (TPP) for development of novel tuberculosis (TB) diagnostics. One of the key highlights is the need for point-of-care non-sputum-based tests capable of detecting all forms of TB through identification of characteristic biomarkers or biosignatures. Compared to the limitations associated with sputum-based TB tests, non-sputum samples are easy to collect, non-invasive, with potential to improve TB diagnosis among children and among people living with HIV/AIDS (PLHIV). This review gives an overview of the existing evide…

Usefulness of Combining Sputum and Nasopharyngeal Samples for Viral Detection by Reverse Transcriptase PCR in Adults Hospitalized with Acute Respiratory Illness.

Nasopharyngeal swabs (NPS) or washings have traditionally been used to diagnose respiratory tract infections. Reverse transcriptase PCR (RT-PCR) is widely used for rapid viral detection using samples from the upper respiratory tract. However, RT-PCR is rarely applied to sputum samples, mainly due to the viscosity of sputum. Thus, we assessed the detection rates of respiratory viruses from NPS, sputum samples, and combined NPS and sputum samples using multiplex RT-PCR (Allplex respiratory panels I, II, and III; Seegene, Seoul, South Korea). Paired NPS and sputum samples were collected from 219 …
