Neurotropic malignant melanoma of right temple with orbital metastasis: a clinicopathological case report.

A case is reported of neurotropic melanoma developed from a superficial spreading melanoma with minimal cytological deviation, situated in the right temple. The nine-year course was clinically characterised by local recurrences, involvement of the orbit and the parotid region via neurogenic invasion, and systemic metastases to lung, seventh rib, and the brain. The histopathology was characterised by fascicles of dysplastic spindle cells, neuroid arrangement in loose fibrillary matrix, and peri- and intraneural permeation of the nerve trunks. Despite minimal atypism the neoplasm metastasised an…

Extramedullary plasmacytoma of the head and neck, parotid and submandibular salivary glands.

Twenty-two cases of extramedullary plasmacytomas (EMP) were diagnosed in the Birmingham region during 1962-1973. Five cases of EMP of the head and neck are discussed, including cases involving the parotid and submandibular salivary glands. Comparative data on incidence, age, sex, and survival is presented. A multidisciplinary approach is important for diagnosis, treatment and follow-up. It is mandatory to carry out investigations to exclude multiple myeloma. Radiotherapy is the treatment of choice; clinical, radiological and pathological evidence are cited. However, a residuum may be left afte

Carotid Body Tumor: A Case Report and Review of the Literature.

Carotid body tumors or carotid body paragangliomas, are rare, highly vascular neuroendocrine neoplasms originating from the paraganglionic c … gnosis. We present the case of an 84-year-old male with a progressively enlarging mass in the right parotid region, present for one month, without associated neurological symptoms. Carotid body tumors should be considered in the differential diagnosis of neck masses. Correlating clinical findings with imaging studies enables a timely diagnosis and appropriate tr …

Complete Response to Pembrolizumab in Radiation-Induced Undifferentiated Pleomorphic Sarcoma of the Face: A Case Report and Review of the Literature.

INTRODUCTION: Undifferentiated pleomorphic sarcoma (UPS) as a secondary malignancy following adjuvant radiation therapy is rare and typically associated with poor prognosis. PD-1 inhibition has been previously investigated as a possible treatment in this space. CASE PRESENTATION: This case presents a 91-year-old male with UPS of the right parotid region, presumed secondary to radiation therapy for cutaneous squamous cell carcinoma. The patient was treated with pembrolizumab immunotherapy and a complete clinical response was achieved, with significant improvement noted on restaging scans. Trea…

Recurrent Acinic Cell Carcinoma of Parotid Gland Diagnosed by Fine Needle Aspiration Cytology: A Rare Case Report.

Acinic cell carcinoma (ACC) is a rare malignant tumor of the head and neck region. It accounts for 12.4%-17.4% of parotid gland malignancies and approximately 3.4% of all salivary gland tumors. ACC is a low-grade salivary gland malignancy that predominantly affects females. In some cases, ACC can undergo high-grade transformation (HGT), resulting in more aggressive behavior. Here, we present a rare case of recurrent Acinic cell carcinoma with aggressive features and high-grade cytomorphology-including increased cellularity, papillary and singly scattered cells, high nuclear-to-cytoplasmic (N:C

Giant cell tumor of temporomandibular joint presenting as a parotid tumor: Challenges in the accurate subclassification of giant cell tumors in an unusual location.

Fine needle aspiration is frequently used as the initial diagnostic procedure in the work-up of head and neck lesions, including soft tissue masses and salivary gland neoplasms. Giant cell tumors (GCTs), both osseous and extraosseous, are benign tumors that occur, albeit rarely, in the head and neck region. Extraosseous GCTs may be further classified based on their tissue of origin and specific anatomic location. Regardless of location, giant cell tumors are morphologically similar and share cytologic and histologic diagnostic criteria. Evaluation of imaging is therefore essential to the corre

Pleomorphic Adenoma of the Upper Lip: A Case Report.

Pleomorphic adenoma is the most frequently encountered benign mixed tumor of the minor salivary gland and usually presents in the parotid; however, in the minor salivary gland, it is more common in the palate. Tumors of the minor salivary glands are uncommon, with the most common intraoral site reported being the hard and soft palate owing to the preponderance of minor salivary glands in this region followed by the lips. Pleomorphic adenoma arising from minor salivary glands of the lips tends to occur at an earlier age than it does at other sites. Pleomorphic adenoma of the lip is a rare neopl

Malignant pilomatrixoma arising on the previously irradiated face: case report and literature review.

BACKGROUND: Malignant pilomatrixoma (also known as pilomatrix carcinoma) is a very rare malignant neoplasm derived from the hair matrix. These slow-growing malignancies of the skin are found in the lower dermis and subcutaneous fat layer. They can exhibit local aggressive behavior or distant metastasis. This malignancy can arise as a solitary lesion de novo or through transformation of a pilomatrixoma over a long period. There is no known distinct etiologic factor or predisposing condition for this kind of malignancy. OBJECTIVE AND CONCLUSION: We present an interesting case of a 65-year-old m

Primary extra-cranial meningioma of head and neck: clinical, histopathological and immunohistochemical study of three cases.

Extracranial meningiomas of the head and neck region are rare neoplasms, the majority being a secondary location of a primary intracranial tumour. We herewith report three rare cases of extracranial meningiomas, located in the temporal muscle, parotid gland and nasal cavity, together with complete pathological, immunohistochemical and ultrastructural studies. Prognosis of this tumour is generally excellent. Surgical excision is the treatment of choice, with no need for further treatment; nevertheless, differential diagnosis must consider other more common tumours of the head and neck and be ba

The soft tissue myxoma of the head and neck region--report of a case and literature review.

A case of soft tissue myxoma in scalp is reported in a patient with a history of congenital cleft lip and palate. Review of the literature shows head and neck soft tissue myxoma occurs almost in every decade of life with peak occurrence in the fourth decade. Men are affected more than women. The most common location in the head and neck region is the palate, followed by the parotid area. The prognosis of this soft tissue tumor is good. Conservative surgical excision with adequate margins is the treatment of choice. Several theories concerning the pathogenesis of this tumor are discussed, with

Do you know your PSMA-tracer? Variability in the biodistribution of different PSMA ligands and its potential impact on defining PSMA-positivity prior to PSMA-targeted therapy.

BACKGROUND: In clinical practice, several radiopharmaceuticals are used for PSMA-PET imaging, each with distinct biodistribution patterns. This may impact treatment decisions and outcomes, as eligibility for PSMA-directed radioligand therapy is usually assessed by comparing tumoral uptake to normal liver uptake as a reference. In this study, we aimed to compare tracer uptake intraindividually in various reference regions including liver, parotid gland and spleen as well as the respective tumor-to-background ratios (TBR) of different 18F-labeled PSMA ligands to today's standard radiopharmaceuti

The role of ultrasound texture analysis in the discrimination of pleomorphic adenoma and Warthin tumor in subjects with well-defined tumor borders.

BACKGROUND: Pleomorphic adenoma (PA) and Warthin tumor (WT) are the two most common benign parotid tumors, and their distinct management strategies necessitate accurate preoperative discrimination. Therefore, this study aimed to investigate the value of ultrasound texture analysis in differentiating between them. METHODS: This retrospective study analyzed ultrasound images from patients with pathologically confirmed PA or WT. Following lesion segmentation, texture features were extracted from the regions of interest (ROI). Feature selection was subsequently performed to identify the most disc

Enhanced data usability in dynamic 68 Ga-PSMA-11 PET of intracranial tumors using a dynamic head motion correction algorithm.

BACKGROUND: Head motion during dynamic positron emission tomography (dPET) compromises pharmacokinetic modeling, especially in prolonged acquisitions where motion artifacts invalidate quantitative analysis. Although motion correction methods are well-established for static PET, their application in dynamic imaging remains limited. This study evaluates the efficacy of a data-driven dynamic head motion correction (dHMC) algorithm in preserving the reliability of kinetic parameter, using dynamic 68 Ga-PSMA-11 PET of intracranial tumors as a challenging model due to the tracer’s inability to cross

Facial Reanimation and Reconstruction of the Radical Parotidectomy.

Radical parotidectomy may result from treating advanced parotid malignancies invading the facial nerve. Survival is often enhanced with multimodality treatment protocols, including postoperative radiation therapy. In addition to the reconstructive challenge of restoring facial nerve function, patients may be left with a significant cervicofacial concavity and inadequate skin coverage. This should be addressed with stable vascularized tissue that is resistant to radiation-induced atrophy. This article describes a comprehensive strategy, includes the use of the anterolateral thigh free flap, the

Multicompartmental Epidermoid Cyst Causing Chronic Parotid Gland and Masticator Space Muscle Atrophy.

Epidermoid cysts are rare benign intracranial tumors of congenital origin. They are slow-growing and are seen to insinuate between brain structures. These are commonly located in cerebello-pontine angle and parasellar regions. The symptoms produced are primarily due to mass effect. Hearing loss, facial nerve palsy, and trigeminal neuralgia are reported when cranial nerves are involved; motor palsy of trigeminal nerve is uncommon. Here, we present an interesting case of an extensive multicompartmental epidermoid cyst causing atrophy of trigeminal nerve with radiologic evidence of chronic motor

Primary extramedullary plasmacytoma of the salivary glands.

Primary extramedullary plasmacytomas (PEMP) are uncommon plasma cell neoplasms that generally occur in the submucosal tissue of the upper airway. Salivary gland PEMP is an extremely rare condition. This report describes a unique case of PEMP in the parotid gland associated with pulmonary amyloidosis. A review of all salivary gland PEMP's suggests that they behave similarly to PEMP's in other locations. The treatment of choice for localized disease is radiotherapy which should include regional lymphatics. Local control can be achieved in the majority of cases. The minority of patients develop s
