Value of Doppler sonography in assessing the progression of chronic viral hepatitis and in the diagnosis and grading of cirrhosis.

OBJECTIVE: The purpose of this study was to evaluate the value of Doppler sonography in assessing the progression of chronic viral hepatitis and in the diagnosis and grading of cirrhosis. METHODS: Abdominal sonographic and liver Doppler studies were performed in 3 groups: 36 patients with chronic viral hepatitis, 63 patients with cirrhosis, and 30 control subjects with no evidence of liver disease. A series of Doppler indices of hepatic vascularity, including portal vein velocity, portal vein pulsatility score, flow volume of the portal vein, resistive and pulsatility indices of the hepatic a

Ultrasound-guided thrombin injection for treatment of iatrogenic femoral pseudo-aneurysm: a nursing protocol.

Femoral pseudo-aneurysm (FPA) is a pulsatile hematoma (false aneurysm) of the femoral artery that communicates with an artery through a disruption in the arterial wall. Most often, FPA develops after an injury or angiographic access to the femoral artery with bleeding into the adjacent tissue. FPA is an uncommon complication, but a very significant one, that can have serious consequences. FPA can initially present as a new thrill or bruit, a pulsatile hematoma, or marked pain and tenderness near the site of arterial puncture. Doppler flow imaging has been the mainstay of diagnosis. Ultrasound

Influence of moderate vasoconstriction on the wave reflection properties of the pulmonary arterial bed.

Increased transmural pressure in the pulmonary arterial bed may reduce vascular input impedance and reduce hydraulic power linked to pulsatile blood flow. Vascular impedance and pulsatile hydraulic power (Wp) levels of isolated perfused rabbit lungs were compared after similar rises of pulmonary arterial pressure (PAp), induced either by vasoconstriction or by left atrial pressure (LAp) elevation. Resulting Wp levels were significantly smaller after vasoconstriction than LAp elevation. Wp showed a minimum level at physiologic PAp (about 20 cm H2O) irrespective of the cause of PAp elevation. Numerical analysis of flow through a severely stenotic carotid artery bifurcation.

The results of computational simulations may supplement MR and other in vivo diagnostic techniques to provide an accurate picture of the hemodynamics in particular vessels, which may help demonstrate the risks of embolism or plaque rupture posed by particular plaque deposits. In this study, a model based on an endarterectomy specimen of the plaque in a carotid bifurcation was examined. The flow conditions include steady flow at Reynolds numbers of 300, 600, and 900 as well as unsteady pulsatile flow. Both dynamic pressure and wall shear stress are very high, with shear values up to 70 N/m2, pr…

Longitudinal Analysis of Location- and Growth-Associated Hemodynamics in Matched Paraclinoid Aneurysms.

BACKGROUND AND PURPOSE: Predicting intracranial aneurysm growth remains difficult, with relatively few widely accepted predictors beyond aneurysm size, location, and patient age. This study investigated the relationship of vascular morphology and hemodynamics to aneurysm location and growth using a matched pairs study design that limited the influence of other predictors. MATERIALS AND METHODS: Longitudinally followed ICA aneurysms identified as growing or stable were matched by size and location, based on their proximity to either the ophthalmic arteries, superior hypophyseal arteries, or po

Doppler assessment of fetal blood flow velocity waveforms during acute maternal oxygen administration as predictor of fetal outcome in post-term pregnancy.

The changes in fetal blood flow velocity waveforms during maternal administration of 60% humidified oxygen were assessed by Doppler ultrasonography in 45 post-term fetuses. During oxygen treatment, nine fetuses exhibited temporary increases (24.3 +/- 2.0% [1 standard deviation] above pretreatment values) in the pulsatility index at the level of internal carotid artery. Although no significant changes (2.9 +/- 5.1%) were found in the remaining 36 fetuses. In this former group a higher incidence of emergency cesarean delivery due to fetal distress and more neonatal complications were observed. A

Carotid geometry effects on blood flow and on risk for vascular disease.

It has been widely observed that atherosclerotic diseases occur at sites with complex hemodynamics, such as artery bifurcations, junctions, and regions of high curvature. These regions usually have very low or highly oscillatory wall shear stress (WSS). In the present work, 3D pulsatile blood flow through a model of the carotid artery bifurcation was simulated using a finite volume numerical method. The goal was to quantify the risk of atherogenesis associated with different carotid artery geometries. A risk scale based on the average WSS on the sinus wall of the internal carotid artery was pr

Impairment of ocular blood flow during regional orbital anesthesia.

BACKGROUND: Regional anesthesia for ophthalmic surgery has been associated with ischemic complications, such as central retinal vascular occlusion, optic atrophy and ischemic optic neuropathy. Impairment of pulsatile ocular blood flow (POBF) may occur with regional orbital anesthesia. In this study we quantified POBF in patients undergoing regional orbital anesthesia. METHODS: Eleven patients (12 eyes) with a mean age of 76.5 years having regional orbital anesthesia for cataract or retinal surgery at a private refractive surgical centre in Calgary had POBF monitoring before, during and 15 min

Phenotyping hemodynamic response to veno-arterial extracorporeal membrane oxygenation in cardiogenic shock.

Simulation models suggest that veno-arterial extracorporeal membrane oxygenation (VA ECMO) uniformly results in left ventricular (LV) distension in cardiogenic shock (CS). This study characterized the hemodynamic response to VA ECMO in animal models of CS, and in patients with CS. LV end-diastolic pressure (LVEDP) and arterial pulse pressure (PP) were assessed in 58 pigs with CS at 2 levels of VA ECMO flows. Pulmonary artery diastolic pressure (PADP), PA pulse pressure (PAPP), and arterial PP data were collected within 2 h of VA ECMO from 128 patients with CS. The animal study identified 3 phe

Pulmonary Vein Flow Impedance: An Early Predictor of Cardiac Dysfunction in Intrauterine Growth Restriction.

INTRODUCTION: In intrauterine growth restriction (IUGR), increased uteroplacental vascular impedance contributes to preferential flow to left ventricle (LV), with consequent alteration of its compliance and increased left atrial (LA) pressure. Pulmonary vein pulsatility index (PVPI) reflects the increased impedance to LA filling and could be used as a cardiac monitoring parameter in IUGR. MATERIAL AND METHODS: A total of 27 IUGR fetuses (group 1), 28 fetuses with appropriate growth for gestational age from hypertensive mothers (group 2), and 28 controls (group 3) were studied. Pulsatility of an intracavernous aneurysm demonstrated by dynamic 320-detector row CTA at high temporal resolution.

Studying the flow dynamics of intracranial aneurysms and the associated pulsatility of the aneurysm wall may help to risk-stratify unruptured aneurysms. Currently, in-vivo methods that visualize aneurysm wall movement at high temporal resolution are not yet fully established. We describe a patient with an intracavernous aneurysm in whom the area of increased aneurysm pulsatility as demonstrated by high-temporal resolution CTA was identical to the area of aneurysm growth on follow-up. A 35-year-old man presented with headaches and a sixth nerve palsy. CT including dynamic CTA demonstrated a pul

Treatment of post-traumatic direct carotid-cavernous fistulas using flow diverting stents: Is it alone satisfactory?

BACKGROUND: Direct carotid cavernous fistula (CCF) occurs between the internal carotid artery (ICA) and the cavernous sinus. Carotid cavernous fistulas (CCFs) frequently present with chemosis, pulsatile proptosis, ocular bruit, vision loss, and occasionally intracerebral hemorrhage or seizure. In this article, we share our experience in endovascular treatment of six patients having this pathology with intracranial flow diverting stents with review of literatures. CASE DESCRIPTION: All six patients had posttraumatic direct CCF, most of their signs and symptoms were visual disturbance, chemosis

Oscillatory shear stress induced calcium flickers in osteoblast cells.

The dynamic physical microenvironment of bone affects the activity of osteoblast cells, yet little is known about how osteoblast mechanotransduction depends on different features of a dynamic stimulus. Here we investigated the effect of physiologically relevant oscillatory flow shear stress on the calcium mobility in osteoblast cells within a microfluidic platform that mimics the confined environment of bone matrix. We characterized the spatiotemporal evolution of intracellular calcium 'flickers', an important signature of cell activation, in response to steady, pulsatile, and oscillatory shea

Simulation of the three-dimensional hinge flow fields of a bileaflet mechanical heart valve under aortic conditions.

Thromboembolic complications of bileaflet mechanical heart valves (BMHV) are believed to be due to detrimental stresses imposed on blood elements by the hinge flows. Characterization of these flows is thus crucial to identify the underlying causes for complications. In this study, we conduct three-dimensional pulsatile flow simulations through the hinge of a BMHV under aortic conditions. Hinge and leaflet geometries are reconstructed from the Micro-Computed Tomography scans of a BMHV. Simulations are conducted using a Cartesian sharp-interface immersed-boundary methodology combined with a seco…

Transesophageal Doppler measurement of renal arterial blood flow velocities and indices in children.

BACKGROUND: Doppler-derived renal blood flow indices have been used to assess renal pathologies. However, transesophageal ultrasonography (TEE) has not been previously used to assess these renal variables in pediatric patients. In this study, we (a) assessed whether TEE allows adequate visualization of the renal parenchyma and renal artery, and (b) evaluated the concordance of TEE Doppler-derived renal blood flow measurements/indices compared with a standard transabdominal renal ultrasound (TAU) in children. METHODS: This prospective cohort study enrolled 28 healthy children between the ages

Portal vein Doppler in peri-operative and critical care medicine: Physiology, measurement and clinical applications.

Portal vein Doppler ultrasound has emerged as a promising tool for haemodynamic assessment in critical care and peri-operative medicine. This educational narrative review integrates fundamental cardiovascular physiology concepts, including Guyton's venous return model and the hepatic arterial buffer response, with practical clinical applications of portal vein assessment at the bedside. The portal vein's position at the intersection of splanchnic circulation and hepatic perfusion makes it relevant in evaluating venous haemodynamics. We describe the technical aspects of portal vein Doppler meas

Role of transcranial Doppler ultrasound in early assessment of acute post-traumatic brain injury.

Traumatic brain injury (TBI) represents a significant global health and socioeconomic burden. This research aims to assess the accuracy of transcranial Doppler (TCD) ultrasound in the early assessment of cerebral blood flow following acute TBI and its utility as an early predictor of neurological outcomes, in correlation with computed tomography findings. A comparative study was conducted on 55 adult patients with acute TBI admitted to Helwan University Hospital between October 2021 and April 2023. TCD was performed within 8 hours post-trauma, and patients were monitored for secondary neurolog

Cerebral vasoreactivity, a surrogate marker of cerebrovascular disease, is not impaired in subjects with lifetime, untreated, congenital isolated GH deficiency.

OBJECTIVES: Cerebrovascular disease (CeVD) is a major cause of death and disability. The role of the GH/IGF-I axis on CeVD risk is controversial. Patients with GH deficiency (GHD) in the setting of hypopituitarism often exhibit CeVD predisposing factors, like low nitric oxide generation, endothelial dysfunction, increased visceral fat mass, increased levels of LDL cholesterol, and increased intima-media thickness, a surrogate marker of atherosclerosis. However, several confounders such as the primary hypothalamic-pituitary lesion, hormonal replacement therapies, consequences of surgery and rad

Cerebral autoregulation and symptoms of orthostatic hypotension in familial dysautonomia.

Familial dysautonomia is an inherited autonomic disorder with afferent baroreflex failure. We questioned why despite low blood pressure standing, surprisingly few familial dysautonomia patients complain of symptomatic hypotension or have syncope. Using transcranial Doppler ultrasonography of the middle cerebral artery, we measured flow velocity (mean, peak systolic, and diastolic), area under the curve, pulsatility index, and height of the dictrotic notch in 25 patients with familial dysautonomia and 15 controls. In patients, changing from sitting to a standing position, decreased BP from 124
