Latissimus dorsi rotational flap combined with a custom-made scapular prosthesis after oncological surgical resection: a report of two patients.

BACKGROUND: Sarcomas that arise from the scapula or periscapular soft tissues often require a total scapulectomy. This often implies a large complex tissue defect that needs adequate reconstruction of both bone and soft tissue. Although various methods have been developed, no optimal procedure has emerged. Postoperative complications are common and functional recovery is not always satisfactory. This study aims to present a new surgical technique that combines a custom-made scapular prosthesis with a functional latissimus dorsi flap. CASE PRESENTATION: Two patients diagnosed with malignant tu

The significance of synovial biopsy in the diagnostic workup of the low-grade periprosthetic joint infection of shoulder arthroplasty.

INTRODUCTION: A common reason for painful shoulder arthroplasties and revision surgery is a low-grade periprosthetic joint infection (PJI). Diagnosing a low-grade infection is, however, a major diagnostic challenge. This applies even more to the shoulder, which differs from other large joints in terms of clinical features and microbiological spectrum. Aim of this study was to evaluate the diagnostic value of the synovial biopsy in the diagnostic workup of low-grade PJI of the shoulder. MATERIALS AND METHODS: A retrospective evaluation was conducted on 56 patients receiving revision surgery on

Suture anchor fixation strength in the Latarjet procedure: a biomechanical study in cadavers.

BACKGROUND: The use of metallic screws for graft fixation during the Latarjet procedure is not devoid of complications. The purpose of the present cadaver study was to determine the initial strength of coracoid graft fixation using suture anchors and compare it to that of the traditional screw fixation of the graft using a fresh frozen human shoulder cadaver model. MATERIALS AND METHODS: Twelve unpaired fresh frozen cadaver shoulders were utilized. In the first group, suture anchor fixation of the graft was used, while 3.75-mm cannulated screws were used in the second group. The specimens wer

Comparison of Curved and Straight Anchor Insertion for Bankart Repair.

The quality of Bankart repair may be compromised by the presence of glenoid perforation during suture anchor placement. The purpose of this study was to compare the rate of glenoid perforation and biomechanical strength of antero-inferior suture anchors placed with a curved vs a traditional straight technique through an anteroinferior portal. Ten bilateral pairs of fresh human cadaveric shoulders were randomized to either a curved or a straight suture anchor insertion technique. An anteroinferior portal was used to place a 1.5-mm soft anchor in the anteroinferior glenoid (5:30 position for rig

Rheumatoid arthritis of the shoulder.

The shoulder frequently is affected by rheumatoid arthritis (RA), often resulting in musculoskeletal dysfunction. The primary purpose of this article is to provide the clinician with an in-depth look at the pathological factors and treatment of RA as it affects the shoulder. The review includes sections on pathological factors, radiology, morbidity, clinical manifestations, and both conservative and surgical management. The secondary purpose of this article is to acquaint the clinician with some of the other rheumatic diseases in which shoulder dysfunction may occur. I hope that the presentati

Femoral Head Allograft Glenoid Bone Reconstruction in Reverse Shoulder Arthroplasty.

Glenoid bone loss remains a significant challenge in both primary and revision reverse shoulder arthroplasty (RSA), particularly following infection, erosion, or component explantation. Adequate reconstruction is critical to restore the native joint line, inclination, and version while ensuring baseplate fixation and long-term stability. Although smaller defects can be addressed with augmented baseplates or humeral head autograft, massive bone loss often requires structural allografting. Femoral head allograft offers a viable and reproducible solution in complex revision cases where autograft

Assessing the spreading potential of an undetected case of COVID-19 in orthopaedic surgery.

BACKGROUND: With the novel coronavirus-induced disease (COVID-19), there is the fear of nosocomial infections and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmissions to healthcare workers (HCW). We report the case of a 64-year-old male patient who underwent explantation of a shoulder prosthesis due to a periprosthetic infection. He was tested SARS-CoV-2 positive 7 days after admission to the orthopaedic department following strict infection control measures, routinely including screening all patients for multi-drug-resistant organism (MDRO) colonization upon admission. Vitamin D deficiency is associated with adverse medical outcomes following total shoulder arthroplasty.

BACKGROUND: Vitamin D deficiency, or hypovitaminosis D, is linked to poor surgical outcomes. Its effect on total shoulder arthroplasty (TSA) remains unclear. This study assesses the association between vitamin D deficiency and postoperative complications following TSA. METHODS: Following propensity score matching for demographics and comorbidities, a retrospective cohort of 6892 TSA patients from 95 institutions was analyzed, with 3446 patients in each group (vitamin D deficient and nondeficient). Complications were evaluated at 90 days, 1 year, and 2 years post-TSA, including acute kidney fa

Factors predictive of Cutibacterium periprosthetic shoulder infections: a retrospective study of 342 prosthetic revisions.

BACKGROUND: Cutibacterium are the most common cause of periprosthetic shoulder infections, as defined by ≥2 deep cultures. Established Cutibacterium periprosthetic infections cannot be resolved without prosthesis removal. However, the decision for implant removal must be made from an assessment of infection risk before the results of intraoperative cultures are finalized. We hypothesized that the risk for a Cutibacterium infection is associated with characteristics that are available at the time of revision arthroplasty. METHODS: In a retrospective review of 342 patients having prosthetic rev

No differences in early results of a hybrid glenoid compared with a pegged implant.

BACKGROUND: Glenoid component loosening after total shoulder arthroplasty is one of the most common causes of failure. A hybrid glenoid that uses peripherally cemented pegs and a central press-fit post may improve implant longevity. QUESTIONS/PURPOSES: We asked, compared with polyethylene pegged glenoid implants, do hybrid glenoid implants with a titanium post provide (1) better ingrowth with fewer radiolucencies, (2) better outcome and pain scores, and (3) lower risk of complications and revisions? METHODS: Between 2009 and 2010, 126 patients underwent primary total shoulder arthroplasty fo

A CT scan protocol for the detection of radiographic loosening of the glenoid component after total shoulder arthroplasty.

BACKGROUND AND PURPOSE: It is difficult to evaluate glenoid component periprosthetic radiolucencies in total shoulder arthroplasties (TSAs) using plain radiographs. This study was performed to evaluate whether computed tomography (CT) using a specific patient position in the CT scanner provides a better method for assessing radiolucencies in TSA. METHODS: Following TSA, 11 patients were CT scanned in a lateral decubitus position with maximum forward flexion, which aligns the glenoid orientation with the axis of the CT scanner. Follow-up CT scanning is part of our routine patient care. Glenoid

Total shoulder arthroplasty with a metal-backed, bone-ingrowth glenoid component. Medium to long-term results.

BACKGROUND: Loosening of a cemented glenoid component is an important cause of failure in shoulder arthroplasty. This study was developed to examine the outcome of patients managed with a metal-backed, bone-ingrowth glenoid component as an alternative to a cemented component. METHODS: The study group included eighty-three total shoulder arthroplasties with a metal-backed, bone-ingrowth glenoid component performed between 1989 and 1994. Seventy-four shoulders had a diagnosis of primary osteoarthritis, and nine shoulders had other diagnoses. All patients were followed radiographically and clini

[Hemi-arthroplasty--primary or secondary measure for 3- and 4-fragment fractures of the proximal humerus in the elderly?].

Successful treatment of three- and four-part proximal humerus fractures is a therapeutic challenge to the surgeon, particularly in the case of elderly patients. Open reduction and internal fixation have been advocated, but have not consistently produced acceptable results. The results of humeral head replacement as a salvage procedure after non-union or failed open reduction and internal fixation are less predictable. The outcome of hemiarthroplasty (Neer II) performed for three- and four-part proximal humerus fractures in elderly patients was studied. The average patient follow-up was 42.9 mo
