Atrial fibrillation in athletes.

Many studies have shown a relationship between long-term endurance sport practice and atrial fibrillation. Inflammation, anatomic remodelling, alterations in the autonomic system and neurohormonal activation are all possible explanations for the increased prevalence of this arrhythmia in athletes. Atrial fibrillation may determine disabling symptoms like palpitations and impaired physical performance, compromising eligibility for competitive activities, but exclusion from sport is not necessary for all athletes. Limited data are available on drug therapy and recently ablation resulted to be a …

Caffeine-related atrial fibrillation.

We present a case of caffeine-induced atrial fibrillation which spontaneously reverted to normal sinus rhythm. Caffeine is a methylxanthine alkaloid that can cause various supraventricular and ventricular arrhythmias. It is important to improve public awareness of the potential adverse effects of high consumption of caffeine-containing products as fatal and serious events can occur.

[Anticoagulation in atrial fibrillation].

In this overview the actual guideline-recommendations for anticoagulation in atrial fibrillation and the problems of the currently available therapy are discussed. Furthermore an outlook over future developments in this field is given. Effective anticoagulation can prohibit thrombembolic events and is thus essential for the prognosis of patients suffering from atrial fibrillation. Until now vitamin-K-antagonists (VKAs) and acetylsalicylic acid (ASA) are available for oral anticoagulation in these patients. VKAs demonstrate a satisfying efficiency combined with rather high bleeding hazard. ASA …

Broad complex atrial fibrillation.

The management of broad complex atrial fibrillation is complex and may be a source of morbidity and mortality if not correctly recognized and treated appropriately. We present a case series of 3 patients who were managed in our emergency department after complaints of palpitations. They presented with varying forms of rapid atrial fibrillation that had broad complexes on the 12-lead electrocardiogram. The first 2 patients were treated with calcium channel blockers for rate control, and treatment was complicated by rapid arrhythmia that required cardioversion. The final patient was correctly tr…

Pilsicainide for atrial fibrillation.

Pilsicainide is a class IC antiarrhythmic drug, which has a pure sodium channel blocking action with slow recovery pharmacokinetics. In experimental studies, pilsicainide has a depressant effect on intra-atrial conduction and a prolonging effect on the atrial effective refractory period (ERP). In patients with paroxysmal atrial fibrillation (AF), pilsicainide significantly prolonged the ERP of the distal pulmonary vein (PV), PV-left atrium (LA) junction and LA, and the conduction time from the distal PV to the PV-LA junction. In some patients, PV-LA conduction block has been observed just befo…

Anticoagulation in atrial fibrillation.

Anticoagulation with warfarin is the most effective means of reducing stroke in AF. The generally recommended INR goal is 2-3. Aspirin provides a modest degree of stroke protection in AF but is inferior to warfarin. Assessment of stroke risk is critical in determining whether to prescribe warfarin therapy to a patient with AF. The most important risk factors for stroke in AF are age over 65 years, hypertension, prior stroke, and left ventricular dysfunction or heart failure. The risk of warfarin may be less than commonly believed, but increases when warfarin is combined with aspirin. Patients

Anticoagulation for atrial fibrillation.

For over two decades, valuable insights have been accumulated from epidemiologic studies and randomized trials about the risks for and prevention of AF-related stroke. AF substantially raises the risk of stroke, most likely through an atrio-embolic mechanism. Warfarin and other members of its class of oral anticoagulants targeted at an INR of 2.5 can abrogate the risk of stroke attributable to AF effectively and fairly safely. High-quality management of anticoagulation can be achieved in usual clinical care. These insights have important implications for the care of individual patients and mor

[Epidemiology of atrial fibrillation].

UNLABELLED: FREQUENCY: Atrial fibrillation is the most frequent sustained arrhythmia, especially in the elderly. The incidence accounts for 2% past the age of 40 years, 6% over 70, an approximately 15% over 90 years old. PATHOGENESIS: Primary cause in "idiopathic" atrial fibrillation is differentiated from secondary causes of cardiac and non-cardiac origin. Important predictive cardiovascular diseases to develop atrial fibrillation are coronary heart disease, hypertension or rheumatic heart disease. Electrophysiological mechanism behind atrial fibrillation is a reentry circuit. The clinical s…

Amiodarone in atrial fibrillation.

Oral amiodarone may be used in the prevention of recurrences of paroxysmal atrial fibrillation (AF) or in chronic AF following electrical cardioversion or for pharmacological cardioversion of AF. The use of amiodarone for rate control of AF is also discussed in this article. Recent reports have shown that low-dose amiodarone is effective in preventing recurrence following cardioversion, but there have been no long-term controlled or comparative studies to support this. Thus amiodarone is not recommended as a first-line drug for preventing recurrences of AF. Results on the use of amiodarone in …

Hypofibrinolysis in atrial fibrillation.

BACKGROUND: There is a high incidence of systemic embolism in patients with chronic atrial fibrillation. A hypercoagulable state has been demonstrated, but the fibrinolytic system is rarely studied. METHODS: Plasma levels of modified antithrombin III (ATM), tissue plasminogen activator (TPA), its inhibitor (PAI-1), TPA-PAI-1 complexes and plasmin-antiplasmin complexes (PAP), d -dimer, and fibrinogen were measured in plasma from 36 patients with chronic atrial fibrillation. Fifteen patients had rheumatic mitral stenosis and 21 had nonrheumatic atrial fibrillation. Levels were compared with tho…

[Cardioversion in atrial fibrillation].

Atrial fibrillation is often not only associated with palpitations, but also with hemodynamic detoriation and high incidence of thromboembolic complications. However, since the establishment or maintenance of a sinus rhythm with antiarrhythmic drugs may also lead to an increased mortality, sinus rhythm should only be achieved by external electrical cardioversion and should be limited to those patients who have been shown to, or may be expected to markedly improved by this intervention. Internal electrical cardioversion should be restricted to those patients who fail external cardioversion. Imp…

Anticoagulation and atrial fibrillation.

The decision to anticoagulate patients with atrial fibrillation (AF) involves weighting the risk of an embolic event without therapy versus the risk of a hemorrhagic event on therapy. Improved methods of monitoring anticoagulation with the International Normalized Ratio (INR), and recent evidence of the efficacy and safety of low-dose warfarin (INR range 2.0 to 3.0) have clarified the role of anticoagulation in AF. Over the past four years, five large prospective randomized trials in patients with nonvalvular atrial fibrillation (NVAF) have reported substantial reductions in stroke in patients…

Operations for atrial fibrillation.

Atrial fibrillation is the most common of all sustained cardiac arrhythmias, yet it has no effective medical or surgical therapy. During the past decade, multipoint computerized electrophysiological mapping systems were used to map both experimental and human atrial fibrillation. On the basis of these studies, a new surgical procedure was developed for atrial fibrillation. Between September 25, 1987, and May 1, 1991, this procedure was applied in 22 patients with either paroxysmal atrial flutter (n = 2), paroxysmal atrial fibrillation (n = 11), or chronic atrial fibrillation (n = 9) of 2 to 21…

Electroanatomic characteristics of atrial premature beats triggering atrial fibrillation in patients with persistent versus paroxysmal atrial fibrillation.

UNLABELLED: APBs in Persistent Versus Paroxysmal AF. BACKGROUND: Although the electrical disconnection between the left atrium (LA) and pulmonary veins (PVs) by radiofrequency catheter ablation has been proven to be effective in controlling atrial fibrillation (AF), the recurrence rate is higher in patients with persistent AF (PeAF) than with paroxysmal AF (PAF). We hypothesized that the origin of the atrial premature beats (APBs) that trigger AF and the pattern of their breakthrough into the LA differ between PAF and PeAF. METHODS: We mapped 75 APBs (53 APBs triggering AF, 22 isolated APBs)…

Mechanism of spontaneous transition from typical atrial flutter to atrial fibrillation: role of ectopic atrial fibrillation foci.

Paroxysmal AF has been known to be initiated by ectopic beats, especially in the pulmonary veins (PVs), and radiofrequency catheter ablation could cure it. We considered that the spontaneous transition from typical atrial flutter to AF also could be initiated by ectopic beats. Twenty patients (18 men, mean age 66 +/- 14 years) with episodes of spontaneous transition from typical atrial flutter to AF were included in this study. They underwent detailed mapping of both atria. All the patients had spontaneous AF initiated by ectopic beats, and all of them had typical atrial flutter and spontaneou

[Atrial fibrillation in the elderly].

The prevalence of atrial fibrillation increases dramatically with advancing age. Elderly adults with atrial fibrillation-even those with high bleeding risk- benefit from anticoagulation. The risk of ischemic stroke or systemic embolism was significantly lower with new direct oral anticoagulants (DOACs) than warfarin as was the risk of major bleeding and intracranial bleeding. Antiplatelet therapy should have a limited role for stroke prevention in elderly adults. Left atrial appendix occlusion may be a suitable alternative for those with contraindications to oral anticoagulants. For symptom ma…
