Long-term results of treatment for axillary subclavian vein thrombosis.

Thrombolytic therapy is an effective method for reopening an occluded venous segment, but little is known about the long-term results of thrombolysis via direct intravenous route for axillary subclavian vein thrombosis (ASVT). Our experience with 13 cases of primary ASVT illustrates the possible advantages of a protocol consisting of lytic therapy, surgical decompression and percutaneous balloon angioplasty if a high-grade stenosis persists. If accompanied by adjunctive therapy to correct the underlying cause, thrombolytic therapy for ASVT results in acceptable long-term symptom relief and vei

Central venous and pulmonary artery catheterizations via the axillary vein.

We report the results of over 300 central venous and pulmonary artery catheterizations via the axillary vein. We found that the success rate with this method compares well with those of other catheterization routes, whilst the risk of mechanical injury, even for artificially ventilated patients, was virtually eliminated.

Single center experience with ultrasound guided axillary vein port placement.

PURPOSE: To retrospectively review a single institution experience of ultrasound guided axillary vein port placement. METHODS: In this retrospective study, a patient list was generated after searching our internal database from 1/1/2012 to 10/1/2018. Patients who had undergone axillary vein port placement were included. Chart review was performed to confirm approach, laterality and to gather demographic data, clinical indications, technical outcomes, and complications. Descriptive statistics were used to analyze this cohort. Chi-square statistics were used to compare outcomes by laterality. …

Hemorrhagic shock 3 days after catheterization from the axillary vein.

A 72-year-old woman presented pulmonary embolism. Continuous intravenous administration of heparin was controlled with values of activated clotting time targeted from 150 to 180 s. On the sixth intensive care unit (ICU) day, a central vein (CV) catheter was inserted through the right axillary vein. On the ninth ICU day, her blood pressure dropped and her right breast was obviously larger than the left. Chest computed tomography showed a large hematoma under the greater pectoral muscle. CV catheterization under anticoagulant therapy is risky for bleeding, but catheterization of the internal jug…

Acute subclavian or axillary vein occlusion during biventricular pacemaker implantation.

During biventricular pacemaker implantation, multiple punctures of the subclavian vein were performed and venous occlusion was apparent during the procedure, which in one case was stopped before lead insertion and in the other patient new access has to be forced through the occlusion by removing one of the already implanted leads. For implanting physicians, it is important to know that acute venous occlusion may occur during lead implantation.

The axillary vein central venous catheter in severely burned patients.

In severely burned patients the approach to the central vein is often difficult due to concomitant edema, but also due to the fact that the skin area, where commonly used approaches are performed, is burned as well, whereas the axillary region is often not involved. In order to perform an axillary approach to the central vein as an alternative to the commonly used approaches in patients, an anatomical dissection in fresh human cadavers was carried out. Considering the anatomical landmarks which were found during dissection of the axillary region, the axillary approach to the central vein was u

Transposed basilic vein versus polytetrafluorethylene for brachial-axillary arteriovenous fistulas.

BACKGROUND: Both transposed basilic vein (BV) and polytetrafluorethylene (PTFE) upper arm arteriovenous fistulas (AVF) are common angioaccess operations. To evaluate the patency and complication rates after AVF, a concurrent series of patients was reviewed. METHODS: Ninety-eight patients underwent brachial artery to axillary vein AVF: 30 BV and 68 PTFE. The PTFE grafts were performed in the standard fashion, whereas the basilic veins were translocated subcutaneously to the brachial artery. RESULTS: Risk factors were similar between the two groups. Basilic vein AVF had better patency at 24 mo…

Axillary vein thrombosis in a female backpacker: Paget-Schroetter syndrome.

In 1875 Sir James Paget reported the first case of primary axillary vein thrombosis. Since then numerous reports of axillary vein thrombosis have appeared citing various initiating events and possible causes. We report here a patient with axillary vein thrombosis. The thrombosis appears to have been provoked by carrying a backpack.

Axillary vein puncture versus cephalic vein cutdown for cardiac implantable electronic device implantation: A meta-analysis.

INTRODUCTION: Cephalic vein cutdown (CVC) and axillary vein puncture (AVP) are both recommended for transvenous implantation of leads for cardiac implantable electronic devices (CIEDs). Nonetheless, it is still debated which of the two techniques has a better safety and efficacy profile. METHODS: We systematically searched Medline, Embase, and Cochrane electronic databases up to September 5, 2022, for studies that evaluated the efficacy and safety of AVP and CVC reporting at least one clinical outcome of interest. The primary endpoints were acute procedural success and overall complications. …

An Ectopic External Jugular Vein Draining into the Axillary Vein: a Rare Anatomical Variation with Clinical Implications.

OBJECTIVE: The external jugular vein drains a considerable part of the head and neck and constitutes a vessel implicated in various procedures in the cervical region. The aim of this study is to present an uncommon anatomical variation of the external jugular vein, and discuss the clinical implications of its presence. CASE REPORT: We present a rare case of an ectopic external jugular vein terminating into the axillary vein, that we came across during routine dissection of a male cadaver of Greek origin. CONCLUSION: The venous system of the external jugular vein is used during procedures for…

Infraclavicular access to the axillary vein - new possibilities for the catheterization of the central veins in the intensive care unit.

Central vein cannulation is one of the most commonly performed procedures in intensive care. Traditionally, the jugular and subclavian vein are recommended as the first choice option. Nevertheless, these attempts are not always obtainable for critically ill patients. For this reason, the axillary vein seems to be a rational alternative approach. In this narrative review, we evaluate the usefulness of the infraclavicular access to the axillary vein. The existing evidence suggests that infraclavicular approach to the axillary vein is a reliable method of central vein catheterization, especially …

Internal jugular to axillary vein bypass for subclavian vein thrombosis in the setting of brachial arteriovenous fistula.

Placement of central venous catheter is the most common cause of subclavian vein thrombosis. In the setting of a functioning ipsilateral bra … catheter. Transposition of the right internal jugular vein and end-to-side anastomosis to the right axillary vein provided prompt and effective venous outflow, with complete resolution of venous engorgement of the affected limb and preservation of the dialysis fistula.

A new method for localizing the landmark of axillary vein and its application.

BACKGROUND: Axillary vein puncture is a popular puncture site for pacemaker implantation. However, due to the lacking of body surface markers, the current puncture method is too complicated and affect the popularization and application of axillary vein puncture. Here, we performed a new body surface landmark to make the blind axillary vein puncture simple and easy. METHODS: The study population included 30 patients referred for pacemaker implantation using axillary vein puncture. Digital subtraction angiography (DSA) was used to determine the direction and the surface landmarks of the axillar…

The Chondroepitrochlearis Muscle: A Rare Cause of Axillary Vein Thrombosis and Lymphedema.

The chondroepitrochlearis muscle is a rare anomaly of the pectoralis major crossing over the neurovascular bundle in the axilla. Often associated with other supernumerary muscles like the arch of Langer, it has been reported in the past as a cause of restriction of arm abduction, cosmetic defects and compression of the ulnary nerve. This case report describes the first known vascular complication due to a chondroepitrochlearis muscle, causing intermittent compression of the axillary vein and thrombosis, associated with pain, upper limb lymphedema, and impaired movements. The diagnosis was susp…

[Ultrasound-guided infraclavicular venipuncture at the junction of the axillary and subclavian veins].

BACKGROUND: Compared to other access routes a central venous catheter inserted via the subclavian vein (VS) is advantageous in terms of pati … presented in which the infraclavicular insertion site is laterally shifted in the direction of the axillary vein (VA). RESULTS: When the vein is visualized by sonography in the long axis the accompanying artery and the pleura remain outside the ultrasound plane. Subclavian and axillary vein thrombosis following radiotherapy for carcinoma of the breast.

Two cases are presented of subclavian and axillary vein thrombosis in patients with carcinoma of the breast treated by radiotherapy and at least 2 years of therapy with tamoxifen. We suggest that radiation fibrosis may have been the major aetiological factor but that the treatment with tamoxifen might also have contributed to the venous thromboses.

Benign obstruction of subclavian and axillary veins possibly due to filariasis.

This paper analyses the clinical features of 8 patients with post-thrombotic obstruction of the subclavian and axillary veins demonstrated by phlebography. The classic physical signs of swelling of the arm with dilated superficial collateral veins were seen. The causes of thrombotic obstruction are discussed and the precipitating cause in this study was possibly filariasis.
