A Novel clinical sign in Intraocular Tuberculosis: Active chorioretinitis within chorioretinal atrophy.

PURPOSE: To report a novel clinical sign in patients with intraocular tuberculosis. The current study is an observational consecutive case series of patients diagnosed with intraocular tuberculosis managed at a tertiary eye care centre from June 1, 2012 to December 31, 2015. OBSERVATIONS: The diagnosis of intraocular tuberculosis was made in 6 patients based on ocular features suggestive of tuberculosis along with a positive tuberculin skin testing and chest X-ray consistent with tuberculosis. All patients presented with decreased visual acuity ranging from 20/25 to 20/400, anterior chamber r

Early resolved acute syphilitic posterior placoid chorioretinitis.

PURPOSE: Acute syphilitic posterior placoid chorioretinitis (ASPPC) is a rare ocular manifestation of syphilis, and its natural course is not well understood. We herein present two unusual cases of ASPPC in which there was almost total resolution of the fundus and optical coherence tomography (OCT) findings during the early presentation before treatment was initiated. CASE REPORTS: Patient 1: A 44-year-old man presented with a 4-day history of decreased visual acuity in his left eye. Dilated fundus examination revealed a yellowish subretinal placoid lesion in the posterior pole, and OCT image

Reactivation West Nile virus infection-related chorioretinitis.

West Nile Virus is a relatively uncommon infection that can involve retinal and choroidal inflammation leading to photopsias, photophobia, and orbital pain. The diagnosis is made by clinical history, serology, and characteristic funduscopic exam and fluorescein angiography findings. Treatment involves primarily supportive care as there are no known effective anti-viral agents. Visual recovery is usually full. Here we present a case of West Nile Virus Infection Related chorioretinitis that demonstrated active linear chorioretinal lesions approximately one year after the initial infection was di

[Bilateral chorioretinitis after infection with Yersinia enterocolitica].

We report a case of severe, bilateral panuveitis with disseminated chorioretinis. With systemic steroid therapy the inflammation resolved within 4 weeks, resulting in chorioretinal scarring. Because the agglutination titer against Yersinia enterocolitica type 3 was increased to 6 times over normal, we consider Yersinia to be the most probable cause of the panuveitis. Concomitantly, serology revealed evidence for recent infection with mumps and shigella. Both can cause anterior uveitis. Therefore, we cannot exclude interactions, which may influence the clinical picture. After reviewing the lite

Vitrectomy for epiretinal membrane with Candida chorioretinitis.

Four eyes underwent vitrectomy and epiretinal membrane dissection for tractional retinal detachment (RD) or macular pucker associated with Candida chorioretinitis. The epiretinal membranes were vascularized and, in three of four eyes, were associated with full-thickness retinal scars, presumably at the site of previous active Candida chorioretinitis. The epiretinal membranes were successfully removed in all four cases. Visual results depended on the degree of macular pathology and the presence and location of a full-thickness retinal scar.

Peripapillary atrophy in patients with birdshot chorioretinitis.

AIMS: This study aims to analyse peripapillary atrophy (PPA), its frequency, extent and relation with measures of visual function in patients with birdshot chorioretinitis (BSCR). METHODS: Patients of the single-centre prospective COhort of patients with BIRDshot chorioretinitis (ClinicalTrials.gov Identifier: NCT05153057) were included. The areas of the optic nerve head (ONH) and PPA were measured on red-green and autofluorescence fundus images acquired by ultrawidefield retinal photography. The main outcome measure was the frequency of PPA. Correlations between the PPA/ONH ratio and disease

Syphilitic chorioretinitis presenting as a choroidal granuloma.

A rare case of syphilitic uveitis presenting as a choroidal granuloma is described in this case report. The clinical picture resembled that of a tubercular choroidal granuloma. However, the patient was positive for treponemal (treponema pallidum hemagglutination assay) as well as non-treponemal tests (venereal disease research laboratory test) for syphilis. Therefore, the patient was treated for ocular syphilis and responded to antisyphilitic therapy. There was a complete resolution of the lesion at the end of 14 days of treatment.

Multimodal imaging of west nile virus chorioretinitis.

PURPOSE: To report the results of multimodal imaging of West Nile virus chorioretinitis. METHODS: Three patients with West Nile virus chorioretinitis were evaluated by color fundus photography, fluorescein angiography, enhanced depth optical coherence tomography, indocyanine green angiography, and fundus autofluorescence. RESULTS: Imaging results demonstrate outer retinal and retinal pigment epithelial involvement with inner retinal sparing. CONCLUSION: Multiple fundus imaging modalities used during the diagnosis of West Nile chorioretinitis are consistent with outer retinal and pigment epi

Chickenpox chorioretinitis with retinal exudates and periphlebitis.

BACKGROUND: Chickenpox is rarely associated with posterior segment inflammation. We report on a case of unilateral chickenpox chorioretinitis with retinal exudates and periphlebitis. CASE PRESENTATION: A 21-year-old healthy man, who suffered from chickenpox 2 weeks prior to symptom development, exhibited mild anterior chamber cells, vitreous opacity, sheathing of retinal veins, and yellow-white exudates in his right eye. Varicella zoster virus DNA was detected by polymerase chain reaction in the aqueous humor. He was treated with intravenous acyclovir followed by oral prednisolone and valacic

Syphilitic chorioretinitis mimicking an anterior chiasmal syndrome.

PURPOSE: To report a case of syphilitic chorioretinitis mimicking an anterior chiasmal syndrome. METHODS: Observational case report. RESULTS: A 74-year-old man with a remote history of syphilitic chorioretinitis was noted to have an apparent junctional scotoma on a screening Humphrey visual field. Subsequent magnetic resonance imaging of the brain was normal, and the patient was referred for neuro-ophthalmologic evaluation. The fundus examination and a fluorescein angiogram demonstrated that the pattern of chorioretinal scarring corresponded to the visual field defects in both eyes. CONCLUS

Chorioretinitis as a first sign of SSPE.

Subacute sclerosing panencephalitis (SSPE) is a progressive neurodegenerative disorder. Ocular involvement in SSPE has been well known and might be seen in 42 to 50% of the patients. Visual findings are generally seen at stage III with neurological abnormalities. Ophthalmologic involvement might be preceding typical SSPE symptoms.

Bilateral fungal endophthalmitis masquerading as multifocal chorioretinitis.

PURPOSE: Spontaneous healing of ocular Candida infection is extremely rare, and early clinical manifestations may be misleading. This report explicates how to make the appropriate diagnosis when facing atypical presentation of the disease. METHODS AND PATIENTS: Case report. RESULTS: A 36-year-old woman presented with bilateral chorioretinitis. Intravenous drug abuse was not suspected, and a diagnosis of multifocal chorioretinitis was made. Oral corticosteroid treatment was started. Three months later, all previously seen lesions had healed on the right eye but had increased on the left with

Endogenous nocardial chorioretinitis in an immunocompetent patient.

PURPOSE: To describe the presentation and management of an immunocompetent patient with endogenous Nocardia chorioretinitis. METHODS: A previously healthy 55-year-old woman presented with reduced vision and metamorphopsia after accidental inhalation of Nocardia organisms from planting soil. She developed systemic nocardiosis with pulmonary and cerebellar involvement as well as bilateral chorioretinitis. Ocular infection resulted in choroidal neovascularization, which was successfully treated with intravitreal triamcinolone and thermal laser photocoagulation. DISCUSSION: This patient is one o

Cryptococcal chorioretinitis with immune reconstitution inflammatory syndrome.

Immune Reconstitution Inflammatory Syndrome (IRIS) is an inflammatory syndrome in the context of immune restoration directed at a specific infectious antigen despite resolution of infection, often associated with opportunistic infections. IRIS has been reported in 30% to 35% of HIV patients who have cryptococcosis in whom antiretroviral therapy was initiated. We would like to report a case of IRIS localized to the eye secondary to cryptococcal infection.
