Anatomical and functional results five years after remanipulated Colles' fractures.

During 1988 we recorded all Colles' fractures (n = 530) in a prospective study in the city of Bergen. According to defined criteria, the fractures were treated by immobilisation in plaster cast with or without reduction; reduction and immobilisation by a Hoffman external fixation device; or open reduction and internal fixation. Included in the present study were 26 patients with secondary displacement after immobilisation in a plaster cast (8% of the reduced fractures). The fractures were remanipulated and splinted in a new cast for four more weeks. The patients were followed up five years aft

Influence of Ibuprofen on Bone Healing After Colles' Fracture: A Randomized Controlled Clinical Trial.

Non-steroidal anti-inflammatory drugs (NSAIDs) may delay bone healing. [Therefore, it is important to establish whether NSAID preparations delay bone healing and what correlations, if any, exist between different bone studies-DEXA-scanning, bone markers, roentgenology controls, and histological examination of newly formed bone]. The purpose of this prospective controlled study was to investigate whether ibuprofen affects bone mineral density, turnover biomarkers, and histomorphometric characteristics of the callus after a Colles' fracture. This study was a single-center, triple-blinded, random

The handshake technique: proposal of a closed manual reduction technique for Colles' wrist fracture.

Colles' fracture is a distal radius injury, with dorsal tilt of the distal radial fracture fragment. Its typical treatment involves reduction and plaster cast immobilization to restore its anatomical position. Sometimes, reduction maneuver may be difficult with poor outcomes, requiring repeated reduction maneuvers or eventual surgical treatment. The author proposes a simple, easy, and reliable manual reduction technique.

Complication of a closed Colles-fracture: necrotising fasciitis with lethal outcome. A case report.

We report a case of a 77-year-old female patient who died 4 days after a closed colles-fracture of the right wrist because of secondary emerged necrotising fasciitis. At the time of visiting our emergency department, the patient reports about untypical pain and progressive swelling of the entire right arm 3 days following a fall onto the outstretched hand where she sustained a closed distal radius fracture. Within 6 h, the patient developed hypotension and fever leading to cardiac and respiratory failure. The emergent-induced diagnostic presented a severe septic situation in the laboratory exa

Anterior fracture displacement in Colles' fractures after Kapandji wiring in women over 59 years.

The aim of this study was to assess the radiological results of Colles' fractures treated with Kapandji wiring and to determine the frequency of postoperative anterior fracture displacement. The X-rays of 89 fractures in women over 59 years of age were evaluated. Five weeks after injury, palmar shift of the distal fracture fragments and/or palmar tilt of more than 20 degrees were observed in 26 patients. In ten wrists dorsal tilt was not sufficiently corrected and measured more than 10 degrees . Increase in ulnar variance was more than 2 mm in 37 wrists and more than 5 mm in six wrists; this w

Association between Colles' fracture and low bone mass: age-based differences in postmenopausal women.

Colles' fracture (CF) in postmenopausal women has been linked to low bone mass at the lumbar spine and hip. However, the diverse methodological approaches of previous studies make the results difficult to compare and thus the implications of CF in osteoporosis daily clinical practice are not clear. We explored the association between CF and low bone mineral density (BMD) in an incident case-control study in 58 postmenopausal Spanish women aged 45-80 years with recent CF and in 83 population-based controls of the same age range. The BMD of ultradistal distal forearm, lumbar spine and hip was me

Rupture of the extensor pollicis longus tendon after Colles fracture and by rheumatoid arthritis.

During the period 1973-1986, 116 patients with rupture of the extensor pollicis longus tendon were operated upon. The lesions were categorised into three anatomical levels: proximal, intermediate and distal. Direct suture was performed for acute lesions; later reconstructive procedures were by means of tendon transfers or free tendon grafts.

Development of osteoarthritis after fixation of Colles' fracture (older type 4). A retrospective study.

Of 37 patients who were treated for a Colles' fracture (Older type 4) by fixation with either a Kirschner wire or a Rush pin, 29 (78%) subsequently developed osteoarthritis. There were no significant correlations between the subjective or objective end results and the development of osteoarthritis. We conclude that osteoarthritis in patients who have suffered from a Colles' fracture (Older type 4) develops soon after the fracture, but it seldom gives rise to symptoms or influences the function of the wrist.

A functional brace for treating Colles fractures; volunteer trial of a new design.

BACKGROUND: Presently used Colles' fracture treatments have similar outcomes with significant complications. Previous studies of a dynamic functional fracture brace, achieving similar or better results had no significant complications. A novel brace design is described to achieve optimal patient outcomes. METHODS: Patient forearm measurements combined with data from a previously tested brace provided parameters for a brace formed using computer assisted design. Fracture swelling was simulated using a 40 ml (ml) plastic water filled bag placed over the dorsum of the lower radius. The interface

Analgesia for the reduction of Colles fracture. A comparison of hematoma block and intravenous sedation.

OBJECTIVE: An alternative to general anesthesia was tested against conventional sedation by a double-blind, randomized clinical trial in reduction of Colles fracture. SETTINGS: A large teaching hospital where cases of Colles fracture are not different from those seen in district hospitals. PATIENTS: Sixty-six out of 80 consecutive cases with this fracture were selected from March to August 1990 on the basis of: 1) informed consent; 2) no contraindication to any method of analgesia; 3) no analgesic during the past 8 hours; 4) injury duration less than 96 hours; 5) no mental, auditory, or visu

Remanipulation or external fixation after slipped Colles' fractures? An anatomical study.

A series of 50 consecutive patients with Colles' fractures which redisplaced after reduction underwent either remanipulation and a plaster cast or remanipulation and external fixation. The external fixator group achieved a significantly better anatomical result than simple remanipulation and replastering. Where plaster immobilization has failed, external fixation gives a better anatomical result than remanipulation and replastering.

[Basket wires--a percutaneous surgical procedure for the treatment of unstable Colles' fractures].

Colles fractures are usually treated with closed reduction and forearm plaster. Even if reduced perfectly, some of these fractures tend to redislocate dorsally and radially, requiring repeated reduction maneuvers. Since K-wires cannot be firmly anchored in the distal fragment if comminution exists, Kapandji proposed a method whereby the distal fragment is not pinned at all. He introduced the pins through the fracture itself and into the medullary canal of the proximal fragment, anchoring the wires into the opposite cortices of the radius shaft. In order to "pre-stress" the K-wires, Böhler and

Repair of the triangular ligament in Colles' fracture. No effect in a prospective randomized study.

We present a prospective and randomized study of two different treatments of extraarticular Colles' fracture with a fractured ulnar styloid. The study comprised 41 patients with 2 years' follow-up; 22 patients were treated with closed manipulation and an above-the-elbow plaster cast, whereas in 19 patients the avulsed ulnar styloid was transfixed and/or the triangular ligament was repaired after closed reduction of the fractured radius. In all the operated on patients, a complete rupture of the triangular ligament was found. Good reduction of all the fractures was achieved primarily according

The effects of calcium on bone mineral density in postmenopausal women with colles fractures.

The increased incidence of fracture in postmenopausal women may be attributable, in part, to osteoporosis. Prediposing factors other than age include genetic constitution, physical activity, alcohol and caffeine abuse, and dietary calcium deficiency. A group of 15 postmenopausal women between the ages of 43 and 85 years were analyzed for bone mineral density within 4 weeks of an acute fracture of the distal radius. Dual photon absorptiometry scanning was performed on the first through fourth lumbar vertebrae. Results indicate that bone mineral densities below the fracture threshold (0.965 g/cm

Regional bone mineral density changes after Colles' and forehand fractures.

Patients who sustain a second Colles' fracture only in one of five instances refracture the previously injured wrist. In those who have sustained fractures of the metacarpals or phalanges of the hand (forehand) subsequent fractures of the forehand are twice as likely to be ipsilateral. We investigated whether persisting regional bone mineral changes could be the mechanism underlying these observations. Bilateral bone mineral density measurements were performed on twenty patients who had sustained a Colles' fracture and twenty-nine who had sustained forehand fractures more than one year previou

Simultaneous Colles' and scaphoid fractures: treatment by combined internal and external fixation.

The authors present the case of a young woman who sustained ipsilateral scaphoid and Colles' fractures that were treated by a combination of Herbert screw fixation of the scaphoid fracture and external fixation of the colles' fracture. This form of treatment provides optimal conditions for both the early return of wrist function and for the maintenance of the manipulated position.
