[Advancements in research on para-aortic lymph node dissection in colorectal cancer].

Para-aortic lymph node metastasis from colorectal cancer is relatively rare. Recent studies have shown that curative para-aortic lymph node dissection can lead to similar prognoses as stage III colorectal cancer for selected patients. However, there are still many unresolved debates regarding the appropriate surgical indications, extent of lymph node dissection, and treatment principles for colorectal cancer patients with para-aortic lymph node metastasis. With the continuous improvement of laparoscopic colorectal surgery techniques, the previously complex and high-risk procedure of para-aorti

Risk of readmission after laparoscopic vs. open colorectal surgery.

PURPOSE: Laparoscopic colorectal resection (LC) is associated with known recovery benefits and earlier discharge when compared to open colorectal resection (OC). Whether earlier discharge leads to a paradoxical increase in readmission has not been well characterized. The aim of this study is to compare the risk of readmission after the two procedures in a large, nationally representative sample. METHODS: Patients who underwent colorectal resection in 2011 were identified from the American College of Surgeons (ACS) National Surgical Quality Improvement Program (NSQIP) database. LC and OC patie

Laparoscopic surgery for synchronous double colorectal cancer with obstruction, plus small B cell lymphoma: a case report.

Synchronous colorectal cancer is a rare disease. It remains challenging for diagnosis and treatment. This paper reports a case of a 75-year-old Chinese male patient presenting with intestinal obstruction, alongside primary thrombocytopenia and a diagnosis of small B-cell lymphoma. Computed tomography scans revealed space-occupying lesions in both the sigmoid colon and ascending colon. The patient initially underwent a transverse colostomy procedure to alleviate the intestinal obstruction and then underwent laparoscopic radical tumor resection. The patient exhibited favorable prognosis and main

Abdominal approaches for rectal prolapse.

The management of full-thickness rectal prolapse involves surgical intervention in the majority of cases. Many procedures have been described employing both perineal and abdominal approaches. Abdominal procedures result in more durable repair of the prolapse; however, the procedures require general anesthesia and are reserved for younger healthier patients. Laparoscopy has been utilized in the treatment of rectal prolapse since its introduction for colorectal procedures; recent studies have found equivalent long-term results and short-term outcomes.

Impact on Patient Care of a Multidisciplinary Center Specializing in Colorectal and Pelvic Reconstruction.

Aim of the study: Many patients with an anorectal malformation (ARM) or pelvic anomaly have associated urologic or gynecologic problems. We hypothesized that our multidisciplinary center, which integrates pediatric colorectal, urologic, gynecologic and GI motility services, could impact a patient's anesthetic exposures and hospital visits. Methods: We tabulated during 2015 anesthetic/surgical events, endotracheal intubations, and clinic/hospital visits for all patients having a combined procedure. Main results: Eighty two patients underwent 132 combined procedures (Table 1). The median age at

The impact of the microbiome on colorectal cancer recurrence after resection.

Surgical resection of a colorectal adenocarcinoma remains a cornerstone in its treatment. However, despite proper patient selection and neoadjuvant and/or adjuvant chemotherapy/radiation, up to 30% of patients thought to be cured will develop a postoperative recurrence. Unfortunately, the outcomes in patients who develop a postoperative recurrence are poor and are associated with high morbidity and mortality. The gut microbiome has emerged to play a role in virtually all aspects of human health. In this manuscript, we critically examine the current literature implicating the gut microbiome's r

Feasibility of on-demand robotics in colorectal surgery: first cases.

INTRODUCTION: The key benefits of robotics are improved precision and control, thanks to fully articulated robotic instruments and enhanced, stable endoscope control. However, colorectal procedures also require large movements such as medialization of the colon where a robotic platform is not always needed. We present the world's first experience in colorectal surgery with a new open platform of on-demand robotics. METHODS AND PROCEDURES: Standard laparoscopic 3-D camera, insufflator, trocars and energy devices, available in all hospitals performing laparoscopic surgery, are used in combinati

Incidence and therapeutic implications of synchronous colonic pathology in colorectal adenocarcinoma.

BACKGROUND: The presence of synchronous benign and malignant colonic pathology may influence the magnitude of surgery for colorectal adenocarcinoma. The aim of this prospective study was to quantitate the need for a more extensive surgical procedure because of synchronous pathology in colorectal cancer patients. METHODS: Between 1984 and 1996, 235 consecutive patients were treated for colorectal adenocarcinoma. Preoperative survey of the colon in 228 patients included colonoscopy (91%) and double contrast barium enema (35.7%). Seven patients were excluded for incomplete preoperative survey be

A systematic review and meta-analysis of short-term outcomes comparing the efficacy of robotic versus laparoscopic colorectal surgery in obese patients.

This meta-analysis was conducted to evaluate and contrast the effectiveness of robotic-assisted and laparoscopic colorectal surgery in the treatment of obese patients. In February 2024, we carried out an exhaustive search of key global databases including PubMed, Embase, and Google Scholar, limiting our focus to studies published in English and Chinese. We excluded reviews, protocols lacking published results, articles derived solely from conference abstracts, and studies not relevant to our research objectives. To analyze categorical variables, we utilized the Cochran-Mantel-Haenszel method a

[Technical operation standards for modified Bacon procedure (2026 edition)].

In China, mid-to-low rectal cancers represent the majority of rectal cancer surgeries. Organ preservation, function preservation, and improvement of patients' quality of life are current directions in surgical development. The modified Bacon procedure, as an ultra-low extreme sphincter-preserving technique, offers advantages including no need for a diverting ileostomy, no auxiliary abdominal incision, and a low incidence of complications, leading to its increasingly widespread clinical application. Although the Expert consensus on modified Bacon procedure (2025 version) has clarified the defin

Temporary percutaneous ileostomy for faecal diversion after intestinal resection for acute abdomen in elderly: how to avoid the conventional loop ileostomy.

Conventional loop ileostomy (CLI) is a suitable procedure for transitory faecal diversion after colocolic or colorectal anastomosis, but it causes relevant morbidities (dehydration, discomfort, peristomal infections) and requires a second operation to be closed. We already described an alternative technique of temporary percutaneous ileostomy (TPI), which can be removed without surgery, as faecal diversion in low colorectal anastomosis. Now we report our experience with the TPI in protecting colocolic and colorectal anastomosis in urgency in elderly. From January 2012 to June 2014, 45 patients

Comparative analysis of marginal profit between da Vinci SP and multi-port systems in colorectal cancer surgery: a single-center descriptive cost-accounting study.

Because fixed costs such as capital investment, lease-related expenses, and depreciation vary substantially according to institutional case volume, timing of platform introduction, and local accounting policy, direct cross-platform comparison based on total cost may be difficult to interpret in routine practice. This study aimed to descriptively compare marginal profit rates of conventional laparoscopy (Lap), the da Vinci single-port (SP) system, and the da Vinci multi-port platforms in colorectal cancer surgery using institutionally standardized running costs. We retrospectively reviewed pati

Colorectal cancer surgery without preoperative histology does not compromise surgical quality-A nationwide cohort study.

BACKGROUND: In colorectal cancer care, planned surgery is sometimes performed without preoperative histological confirmation of malignancy, raising concerns that diagnostic uncertainty may affect surgical quality. Do surgeons maintain high quality oncological resection practices when colorectal cancer is suspected yet not histologically confirmed? DESIGN: We conducted a nationwide retrospective cohort study using the Danish Colorectal Cancer Group database, including patients undergoing planned colorectal cancer surgery between 2016 and 2020. Patients were grouped according to whether maligna

Wound Concerns and Healthcare Consumption of Resources after Colorectal Surgery: An Opportunity for Innovation?

BACKGROUND: Significant portions of patients undergoing colorectal surgical procedures have minor incision disturbances, yet very few meet definitions for surgical site infection (SSI). We sought to investigate the natural history of incision disturbances with a focus on the patient experience and resource utilization. We hypothesize that patients who have an incision disturbance consume frequent healthcare resources in the post-operative period despite the fact that most never receive a diagnosis of SSI. METHODS: A 24-month prospective observational study was undertaken at an academic instit…

Operative Time as a Universal Risk Factor for Infectious Complications after Colorectal Cancer Surgery: A Multicenter Stratified Analysis by Surgical Site.

OBJECTIVES: Postoperative infectious complications in colorectal cancer prolong hospital stays and increase the risk of recurrence. Given that risk factors may vary depending on the surgical procedure-namely, right-sided colon, left-sided colon, and rectal cancer-we aimed to identify both common and procedure-specific risk factors.. METHODS: This retrospective multicenter study included 2,636 patients who underwent elective colorectal cancer surgery between April 2015 and March 2023 at three affiliated institutions. The primary endpoint was the incidence of Clavien-Dindo grade ≥2 infectious c
