Event related potentials elicited by violations of auditory regularities in patients with impaired consciousness.

Improving our ability to detect conscious processing in non communicating patients remains a major goal of clinical cognitive neurosciences. In this perspective, several functional brain imaging tools are currently under development. Bedside cognitive event-related potentials (ERPs) derived from the EEG signal are a good candidate to explore consciousness in these patients because: (1) they have an optimal time resolution within the millisecond range able to monitor the stream of consciousness, (2) they are fully non-invasive and relatively cheap, (3) they can be recorded continuously on dedic

Valacyclovir-induced Neurotoxicity in a Patient with a Preserved Renal Function.

Valacyclovir, a prodrug of acyclovir, is the first-line treatment for herpes zoster, but the renal function must be monitored, because acyclovir is metabolized by the kidneys. We herein report a case of valacyclovir-induced neurotoxicity with no preceding renal impairment. An 88-year-old man was admitted because of an impaired consciousness after the administration of valacyclovir at 3,000 mg daily for herpes zoster on the chest. His consciousness level gradually improved with hydration and valacyclovir withdrawal. It was later confirmed that the level of acyclovir on admission had been 35.45

Bridging neural circuits and clinical outcomes: a novel framework for perioperative disturbances of consciousness and cognition.

Disturbances of consciousness and cognition that happen during the perioperative period remain a critical challenge in anesthesiology, manifested as intraoperative awareness, delayed emergence, and postoperative delirium. These disorders arise from complex interactions among neural connectivity, neurotransmitter dynamics, and the pharmacological modulation of cortical and subcortical circuits. The thalamocortical network, brainstem arousal centers, and frontoparietal integration systems are crucial in facilitating transitions between drug-induced unconsciousness and recovery; however, their fu

Cerebellar Purkinje cell firing promotes conscious recovery from anesthesia state through coordinating neuronal communications with motor cortex.

Background: The neurobiological basis of gaining consciousness from unconscious state induced by anesthetics remains unknown. This study was designed to investigate the involvement of the cerebello-thalamus-motor cortical loop mediating consciousness transitions from the loss of consciousness (LOC) induced by an inhalational anesthetic sevoflurane in mice. Methods: The neural tracing and fMRI together with opto-chemogenetic manipulation were used to investigate the potential link among cerebello-thalamus-motor cortical brain regions. The fiber photometry of calcium and neurotransmitters, inclu

Acyclovir-associated encephalopathy triggered by nonsteroidal anti-inflammatory drugs.

OBJECTIVE: Both acyclovir (ACV) and valacyclovir (VACV) can cause drug-induced encephalopathy, which occurs primarily in patients with renal impairment because of delayed drug metabolism. Here, we report a case of ACV-associated encephalopathy precipitated by the addition of nonsteroidal anti-inflammatory drugs (NSAIDs). PATIENT: A 97-year-old woman with a 1-d history of altered consciousness was admitted to our hospital. After treatment with ACV and VACV for herpes zoster, NSAIDs were introduced for pain 5 d prior to admission. VACV and NSAIDs were subsequently administered by her primary ca

Acceptability of bispectral Index monitoring in end-of-life care for dementia.

BACKGROUND: Assessing consciousness might benefit the care for people with Alzheimer's disease and other types of dementia at the end of life by indicating distressing symptoms and moments of awareness. This could guide symptom control and increase interaction with the person with dementia. OBJECTIVE: This study aims to investigate the acceptability of a biosignal measurement of consciousness, the Bispectral Index monitoring (BIS), for persons with dementia at the end of life. METHODS: Qualitative study using focus groups and interviews with demonstrations of BIS monitoring, with 17 individu

Peri- and intraoperative EEG signatures in newborns and infants.

OBJECTIVE: The electroencephalographic derived indices have been developed for adult patients, however these monitors have not been validated for infants. METHODS: Frontal EEGs were recorded in 115 infants aged <1 year [0-3-months (N = 27), 4-6-months (N = 30), 7-9-months (N = 29) and 10-12-months (N = 29)] who received general anaesthesia with sevoflurane. Total power (µV2) and relative β-, α-, θ-, δ-power (%) were analyzed. Additionally, in 20 EEGs event marker were added (baseline, loss of consciousness, intraoperative situation, extubation) to assess perioperative EEG dynamics. RESULTS:

Argatroban Anticoagulation for Transcarotid TAVR Under Conscious Sedation in a Patient With Heparin-Induced Thrombocytopenia.

OBJECTIVES: To describe argatroban anticoagulation in a patient with heparin-induced thrombocytopenia undergoing transcarotid transcatheter aortic valve replacement under conscious sedation. KEY STEPS: Vitamin K antagonist therapy was discontinued and intravenous argatroban initiated, with anti-factor IIa monitoring. Procedural anticoagulation consisted of an argatroban bolus followed by continuous infusion guided by activated clotting time. An ultrasound-guided intermediate cervical plexus block with remifentanil sedation maintained consciousness for neurological assessment. Argatroban was r

Transient basilar artery occlusion monitored by transcranial color Doppler presenting with a spectacular shrinking deficit: a case report.

INTRODUCTION: We describe the case of a 79-year-old Caucasian Italian woman with a transient basilar occlusion monitored by transcranial Doppler, with subsequent recanalization and clinical shrinking deficit. This is the first case of transient basilar occlusive disease diagnosed and monitored by transcranial Doppler. This case is important and needs to be reported because transient basilar occlusion may be easily diagnosed if transcranial Doppler is performed. CASE PRESENTATION: A 79-year-old woman affected by chronic atrial fibrillation and not treated with oral anticoagulants, cardioverted

Creutzfeldt-Jakob Disease Presenting as Nonconvulsive Status Epilepticus.

Creutzfeldt-Jakob disease is a rare, rapidly progressive spongiform encephalopathy in humans. EEG plays an important role in diagnosing this disease. In some patients, epileptic activity and encephalopathy from various aetiologies may share morphological features on EEG. This similarity could create difficulties in EEG interpretation, especially if the patient presents with disturbed consciousness. In this case report, a 74-year-old female with Creutzfeldt-Jakob disease presented initially with rapidly progressive impairment of consciousness and focal epileptiform activity on EEG. An EEG perfo

External Validation of a Pharmacokinetic Model of Propofol for Target-Controlled Infusion in Children under Two Years Old.

BACKGROUND: Previously, a linked pharmacokinetic-pharmacodynamic model (the Kim model) of propofol with concurrent infusion of remifentanil was developed for children aged 2-12 years. There are few options for pharmacokinetic-pharmacodynamic model of propofol for children under two years old. We performed an external validation of the Kim model for children under two years old to evaluate whether the model is applicable to this age group. METHODS: Twenty-four children were enrolled. After routine anesthetic induction, a continuous infusion of 2% propofol and remifentanil was commenced using t

[Correlation of occlusion pressure with sedation in non-neurocritical patients].

BACKGROUND: Mechanical ventilation is used in conjunction with sedation, which is titrated with clinical scales based on the therapeutic objective. It is difficult to find a reliable marker of respiratory drive in critically ill patients, so determining the correlation between the depth of sedation and airway occlusion pressure at 100 milliseconds (P0.1) would help titrate the depth of sedation and regulate respiratory drive. OBJECTIVE: To determine whether P0.1 correlates with the depth of sedation measured with a clinical RASS scale and the bispectral index. MATERIAL AND METHODS: A prospec

Comparison of bispectral index and patient state index during general anesthesia with remimazolam.

BACKGROUND: Processed electroencephalographic indices, such as the Bispectral Index (BIS) and Patient State Index (PSI), are widely used to assess anesthetic depth; however, their comparative behavior under remimazolam remains unclear. In practice, either index may deviate from its target range despite stable anesthesia, and the response of the companion monitor is uncertain. This study examined whether the two indices moved in parallel or diverged when one departed from its nominal range during remimazolam anesthesia. METHODS: In this prospective, single-center study, BIS and PSI were simult

Correlation between bispectral index, observational sedation scale, and lower esophageal sphincter pressure in volunteers using dexmedetomidine or propofol.

BACKGROUND: Many anesthetics reduce lower esophageal sphincter pressure (LESP) and consequently the gastro-esophageal pressure gradient (GEPG); thus they may promote gastro-esophageal reflux and contribute to aspiration pneumonia. Our goals were to evaluate the association between LESP and 2 measures of sedation: bispectral index (BIS) and the responsiveness component of the Observer's Assessment of Alertness score (OAA/S). MATERIAL/METHODS: Eleven healthy volunteers were each sedated on 2 separate days. Subjects were given sedative infusions of increasing target plasma concentrations of dexm

[Carotid endarterectomy under cervical epidural anesthesia].

General anesthesia for carotid endarterectomy (CEA) imposes the need for various monitorings of cerebral function during occlusion of the carotid artery. However, there is no ideal method for detecting all the signs of cerebrovascular insufficiency under general anesthesia. We suggest that the patient's neurological state is the best monitor of cerebral function. Therefore, our patients underwent cervical epidural anesthesia (EDA) enabling them to remain conscious while experiencing analgesia of the operated areas throughout the course of the operation. Five patients with cervical internal car

Comparison of time-series models for predicting physiological metrics under sedation.

This study presents a comprehensive comparison of multiple time-series models applied to physiological metric predictions. It aims to explore the effectiveness of both statistical prediction models and pharmacokinetic-pharmacodynamic prediction model and modern deep learning approaches. Specifically, the study focuses on predicting the bispectral index (BIS), a vital metric in anesthesia used to assess the depth of sedation during surgery, using datasets collected from real-life surgeries. The goal is to evaluate and compare model performance considering both univariate and multivariate scheme

Bispectral index monitoring for sedation in scheduled adult colonoscopy: A randomized controlled trial.

BACKGROUND: Patients should be closely monitored during procedures under sedation outside the operating room, but it is unclear which type of monitoring is best. We investigated the efficacy and safety of BIS monitoring vs conventional monitoring for sedation during colonoscopy. METHODS: We performed a double-blind clinical trial in 180 patients undergoing elective colonoscopy. Patients were randomized to 1) the BIS group or 2) a control group, in which sedation was monitored with a BIS monitor or the Ramsay Sedation Score, respectively. The primary outcome was the rate of sedation-induced ad
