Reduction of Rehospitalization with Addition of Geriatrics/Transitions of Care Consult Service.

Older adults are at high risk of rehospitalization after an acute event and at even higher risk of permanently losing an activity of daily living with each hospitalization. This is especially true in those with encephalopathy, delirium, dementia, falls, and failure to thrive. Although it is widely known that rehospitalization rates are higher in patients who discharge to skilled nursing and long-term care facilities, geriatrics consultations have not been shown to consistently decrease this risk. In this study, we added a novel component specific to transitions of care alongside a traditional …

A Knitted and MXenzyme-Integrated Dressing for Geriatrics Diagnosis and Ulcer Healing.

Integrated diagnostic and therapeutic dressings are desirable to relieve diabetic patients who often suffer from diabetic foot ulcers (DFUs) and peripheral vascular diseases (PVDs). However, it is highly difficult to monitor the pulse waves with fidelity under wet environments and connect the waveforms to diseases through a small strain sensor. Additionally, immobilizing MXenzyme to regulate spatially heterogeneous levels of reactive oxygen species (ROS) and applying active intervention to enhance ulcer healing on a single structure remain a complex task. To address these issues, we designed a

[Polymyalgia rheumatica-A challenge in geriatrics : Interdisciplinary presentation of diagnostics and treatment].

Polymyalgia rheumatica is the second most frequent inflammatory rheumatic disease in people aged over 50 years, after rheumatoid arthritis. It is characterized by pain and morning stiffness in the region of the shoulders, hip girdle and neck. It can be associated with giant cell arteritis (CGA). Treatment with glucocorticoids is indispensable. The duration of treatment varies and often exceeds 1 year. The additive administration of methotrexate is an option for saving glucocorticoids. The biologicals tocilizumab or secukinumab are very promising alternatives. The course of treatment should be

Anogenital Warts in Geriatrics: Immunosenescence and New Sexual Contacts? A Case Report.

INTRODUCTION: Anogenital warts (AGW) are sexually transmitted infections (STIs) caused by the human papillomavirus (HPV), particularly types … line in sexual function due to the aging process often occurs with advancing age. As a result, many geriatrics seek various solutions in order to maintain sexual function and activity in old age. Since STIs can occur at any age, early detection and education are essential, particularly for the geriatric population.

Nitric oxide and geriatrics: Implications in diagnostics and treatment of the elderly.

The nation's aging population is growing rapidly. By 2030, the number of adults age 65 and older will nearly double to 70 million. Americans are living longer and older adults can now live for many years with multiple chronic illnesses but with a substantial cost to health care. Twenty percent of the Medicare population has at least five chronic conditions i.e., hypertension, diabetes, arthritis, etc. Studies in experimental models and even humans reveal that constitutive production of nitric oxide (NO) is reduced with aging and this circumstance may be relevant to a number of diseases that pl

Geriatrics in family practice. A comparison of data from Wisconsin and Virginia.

For 109 Wisconsin family practices, 18% of all contacts were with patients 65 years of age and older. Of these contacts, 45% took place in the office and 40% in the hospital; the patient was female in 58.7%. The number of problems or diagnoses was 2.28 per patient contact. Comparison of the geriatric problem profile from the Wisconsin study with similar data from Virginia showed striking similarity. The diagnostic category "diseases of the circulatory system" contained more than 30% of patient problems. No other category contained 10% or more problems in either study. The problem profiles for

Pharmacological and non-pharmacological interventions of insomnia in geriatrics: A systematic review.

INTRODUCTION: Insomnia is the most prevalent sleep disorder in the geriatric population, affecting 30-48% of communitydwelling older adults and higher proportions in residential care settings. Its management is complicated by age-related pharmacokinetic changes, polypharmacy, and disproportionate risks associated with traditionally prescribed hypnotic agents. This systematic review and meta-analysis synthesise contemporary evidence to address two key clinical questions: (1) what pharmacological agents are effective and safe for treating insomnia in older adults; and (2) what non-pharmacologica

When expert agreement replaces evidence: The rise of Delphi consensus in geriatrics.

Expert consensus is intended to guide clinical reasoning when evidence is scarce. In contemporary geriatrics, however, the Delphi method is increasingly used where data are not absent but abundant. Sarcopenia and frailty exemplify this dilemma. Repeated consensus statements have redefined diagnostic criteria and thresholds, yet prevalence estimates range dramatically depending on the framework applied. The proliferation of overlapping definitions and tools has generated conceptual instability, fragmented research efforts, and hindered therapeutic development. When expert agreement becomes the …

Cannabis use for Sleep Disturbance Among Older Patients in a Geriatrics Clinic.

Cannabis use is growing among older adults to manage medical concerns including poor sleep. In this study, we characterized how patients seen at a geriatrics clinic use cannabis to address sleep disturbance. Specifically, we conducted an anonymous survey of 568 adults, including 83 who reported cannabis use within the past 3 years, to inquire about such use. We compared cannabis use characteristics between those using it for sleep disturbance versus all other conditions. We considered a p-value <.10 to be statistically significant. Among the cannabis users in our sample, 29% reported using can…

Prognostic nomogram for risk of mortality after hip fracture surgery in geriatrics.

PURPOSE: Hip fracture is a significant public health problem, with associated high morbidity and mortality. Orthopedic surgeons are concerned to improve prognosis and stratify mortality risk after hip fracture surgery. This study established a nomogram that combines the Charlson Comorbidity Index (CCI) with specific laboratory parameters to predict mortality risk after hip fracture surgery in geriatrics. METHODS: The records of consecutive patients who underwent hip fracture surgery from January 2015 through May 2020 at one medical center were reviewed for perioperative factors and mortality.…

Ferrets are considered geriatric at 3 years of age. Twice-yearly physical examinations with additional testing finds problems and leads to prompt treatments that extend quality of life for the ferret. Bloodwork, urinalysis, dental examination, and imaging including radiographs and ultrasonography should be performed at least annually. Specialized testing includes echocardiography, electrocardiology, blood pressure, computed tomography scanning, and dental radiography depending on the ferret. Common problems include dental disease, cardiomyopathy, renal disease, musculoskeletal disorders, chron

In 2016, consequences, contributing factors, and prevention of inappropriate drug prescribing were investigated. Two studies lift the veil on unexplained falls and syncope. A meta-analysis questions the need to adapting transfusion's threshold to comorbidities, and another one the performance of screening for swallowing disorders. The Framingham Heart Study reports the declining of the incidence of vascular dementia. The role played by microbleeds in cognitive deterioration was investigated and aducanumab showed encouraging effect in Alzheimer's disease. When vaccination against influenza for

Geriatric Resuscitation.

The geriatric population makes up a large portion of the emergency patient population. Geriatric patients have less reserve and more comorbid diseases. They are frequently on multiple medications and are more likely to require aggressive treatment during acute illness. Although it may not be obvious, it is important to recognize the signs of shock as early as possible. Special care and monitoring should be used when resuscitating the elderly. The use of bedside ultrasound and monitoring for coagulopathies are discussed. Clinicians should be constantly vigilant and reassess throughout diagnosis
