Incidence of hardware-related pain and its effect on functional outcomes after open reduction and internal fixation of ankle fractures.

OBJECTIVES: To document the incidence of late pain and hardware removal after open reduction and internal fixation (ORIF) of ankle fractures. To test the hypothesis that late pain overlying the distal tibial and fibular hardware is associated with poorer functional outcomes. DESIGN: Retrospective review. SETTING: Level II trauma center. PATIENTS: One hundred twenty-six skeletally mature patients undergoing ORIF of unstable malleolar fractures who were followed up for at least six months from injury were included. MAIN OUTCOME MEASUREMENTS: Analog pain score, Short Form-36 Health Survey (SF…

Outcome analysis following removal of locking plate fixation of the proximal humerus.

BACKGROUND: Concerning surgical management experience with locking plates for proximal humeral fractures has been described with promising results. Though, distinct hardware related complaints after fracture union are reported. Information concerning the outcome after removal of hardware from the proximal humerus is lacking and most studies on hardware removal are focused on the lower extremity. Therefore the aim of this study was to analyze the functional short-term outcome following removal of locking plate fixation of the proximal humerus. METHODS: Patients undergoing removal of a locking …

Complications in a Young Adult Attributable to a Retained Pediatric Dynamic Hip Screw.

INTRODUCTION: Orthopedic implants are used for many different conditions in the pediatric population. The literature on hardware removal is controversial and vague. CASE REPORT: We highlight a young adult male who underwent a dynamic hip screw (DHS) due to a motor vehicle accident at 11 years old. He healed the fracture and did well for years. He was lost to follow-up and the hardware was never removed. The patient presented to our facility with a periprosthetic subtrochanteric proximal femur fracture just distal to the retained hardware. The DHS was removed and the fracture fixed with an int…

Leon Chen's Modified Valsalva Technique (LCMVT) for Retrieval of Displaced Dental Implants: A Clinical Technique Report.

The retrieval of a displaced dental implant from the maxillary sinus-whether through an intraoral lateral window or a transnasal endoscopic route-invariably requires further surgical intervention and a subsequent healing period. For instance, in cases where a lateral window approach was not initially indicated, the accidental displacement of a dental implant into the maxillary sinus often necessitates the creation of such an access, which can lead to patient dissatisfaction. If removal through the lateral window proves unsuccessful, referral to an otolaryngologist may become necessary, and a t

Case Series of Comminuted Olecranon Fracture Treated by Plate Fixation; Do We Have to Remove the Plate?

Comminuted olecranon fracture requires surgical intervention. Plate fixation has been performed on the majority of cases. We reviewed the cases of comminuted olecranon fracture in young and middle age treated by plate osteosynthesis and analyzed the functional outcome, complications and ratio and timing of hardware removal. Fifteen cases of comminuted olecranon fractures treated by plate fixation were reviewed. Bone union was achieved in all cases, the average range of motion at the final follow up was -11° in extension, 133° in flexion, 89° in pronation and 88° in supination. Hardware removal…

Robotic-assisted total knee arthroplasty in post-traumatic and previously operated knees with retained hardware: Practical decision-making workflow.

Total knee arthroplasty (TKA) after periarticular fracture fixation, malunion, high tibial osteotomy, or other previous knee surgery differs substantially from routine primary TKA. Retained implants, extra-articular deformity, distorted landmarks, bone defects, scarred soft tissues, and infection risk may all compromise standard instrumentation and implant positioning. Robotic-assisted TKA (RA-TKA) is increasingly used in selected non-standard primary cases because computed tomography -based or imageless planning can define deformity, anticipate implant-hardware conflicts, guide bone resection

Bone Loss After Implant Removal.

This case illustrates the complex challenges of managing forearm trauma complicated by bone loss after implant removal. A long-standing radial shaft plate, integrated into cortical bone with overgrowth, had to be removed to treat a new peri-prosthetic fracture. Removal caused massive cortical loss, leaving poor bone stock for fixation. Dual plating was performed to bridge the defect and stabilize the distal radius fracture, but mechanical failure prompted a staged revision. A deliberately delayed second operation allowed time for biological preparation. Definitive reconstruction included corre

Severe pulmonary injury leading to death during thoracic rod removal: a case report.

PURPOSE: Removal of hardware procedures following posterior spinal fusion is most commonly performed for hardware irritation without overt infection. It is imperative that surgeons realize that serious complications may arise from this procedure. The purpose of this report is to report a case of a pneumothorax that developed in a thoracolumbar removal of hardware case that resulted in a patient death. METHODS: Retrospective review of a patient's medical record and imaging. RESULTS: A 74-year-old patient with a history of T4-10 anterior discectomy and fusion with rib autograft and T4-L2 poste

A call for fixed standards: a decade of orthognathic surgery, biting into revision rates and metalwork removal.

The removal rate of titanium miniplates following orthognathic surgery varies widely (3.2-27.5%) due to inconsistent study designs and mixed samples. Plate removal is not routinely conducted in the UK. A 2019-2020 meta-analysis reported a 13.4% removal rate, though regular audits are uncommon. This study evaluates whether audit of local plate removal rates against established benchmarks merits encouragement. A retrospective review was conducted of orthognathic surgery at a tertiary centre (2014-2024). Procedures included Le Fort osteotomy, bilateral sagittal split osteotomy (BSSO), bimaxillary

Complications related to fracture treatment in HIV patients: a case report.

We present the case report of a 40-year-old woman who was HIV-positive in Highly Active Anti-Retroviral Therapy (HAART) and affected by femural pertrochanteric fracture, which was treated by endomedullary nailing. Two years after the surgical operation, the woman developed an aseptic symptomatic osteolysis around the implant. Hardware removal was resolutive. Aseptic and septic hardware mobilization, hardware removal, and implant decision in HIV patients with pertrochanteric fractures is discussed. The authors suggest close follow-up and prompt hardware removal, as soon as X-rays demonstrate he…

Reasons for removal of rigid internal fixation devices in craniofacial surgery.

Despite the widespread use of rigid fixation techniques in craniofacial surgery, there is a paucity of studies in the literature that serve to better define the reasons for the subsequent removal of plates and screws. The current study appears to be the first to attempt to assess these issues among a broad range of craniofacial surgery patients. Fifty-five patients who underwent hardware removal following craniofacial surgery at the University of Michigan Medical Center between 1989 and 1995 were retrospectively studied via an in-depth chart review. Common reasons for hardware removal included…

A systematic review of volar locking plate removal after distal radius fracture.

BACKGROUND: Indication of volar locking plate (VLP) removal after bony healing of distal radius fracture (DRF) is controversial. Studies with various range of removal rate were reported. The purpose of this systematic review was to investigate the frequency and the reasons of hardware removal over the world. We hypothesized that more frequent VLP removal contribute to better clinical outcomes. METHODS: The authors searched all available literature in the PubMed and EMBASE databases for articles reporting on outcomes of treatment using VLP for DRF. Data collection included hardware removal rat…

First results with the immediate reconstructive strategy for internal hardware exposure in non-united fractures of the distal third of the leg: case series and literature review.

BACKGROUND: Fractures of the distal third of the leg are increasingly common and are often handled by open reduction and internal fixation. Exposure and infection of internal hardware could occur, especially after high energy traumas, requiring hardware removal and delayed soft tissue reconstruction. Nevertheless immediate soft tissue reconstruction without internal hardware removal is still possible in selected patients. In this study the effectiveness and the complications of immediate soft tissue reconstruction without internal hardware removal is analyzed. METHODS: 13 patients, affected b…
