Survival status, mortality predictors, and scores in patients with severe traumatic brain injury in the intensive care unit.

BACKGROUND: Patients with traumatic brain injury (TBI) occupy an important place in intensive care units (ICUs) with high morbidity and mortality rates. The aim of this study was to determine the parameters predictive of mortality as the primary outcome and the distribution of these parameters as a secondary outcome in patients admitted to the ICU with TBI. METHODS: After ethics committee approval and informed consent of the patient's relatives, TBI patients over 18 years of age treated in the anesthesia and reanimation ICU of our hospital between 2021 and 2023 were retrospectively included i

Survival and analysis of prognostic factors for severe burn patients with inhalation injury: based on the respiratory SOFA score.

BACKGROUND: It is important to determine the severity of inhalation injury in severely burned patients. The oxygenation index PaO2/FiO2(PF) ratio is a key clinical indicator of inhalation injury. Sequential organ failure assessment (SOFA) is developed to assess the acute incidence of critical illness in the population. We hope to provide an assessment of survival or prognostic factor for severely burned patients with inhalation injury based on the respiratory SOFA score. METHODS: This is a retrospective cohort study of all admissions to Department of Burn and Plastic Surgery at West China Hos

Novel application of the Rotterdam CT score in the prediction of intracranial hypertension following severe traumatic brain injury.

OBJECTIVE: Severe traumatic brain injury (TBI) is associated with intracranial hypertension (ICHTN). The Rotterdam CT score (RS) can predict clinical outcomes following TBI, but the relationship between the RS and ICHTN is unknown. The purpose of this study was to investigate clinical and radiological factors that predict ICHTN in patients with severe TBI. METHODS: The authors performed a single-center retrospective review of patients who, between 2018 and 2021, had an intracranial pressure (ICP) monitor placed following TBI. Radiological and clinical characteristics related to the TBI and IC

The Mangled Extremity Severity Score Fails to be a Good Predictor for Secondary Limb Amputation After Trauma with Vascular Injury in Central Europe.

BACKGROUND: The Mangled Extremity Severity Score (MESS) was constructed as an objective quantification criterion for limb trauma. A MESS of or greater than 7 was proposed as a cut-off point for primary limb amputation. Opinions concerning the predictive value of the MESS vary broadly in the literature. The aim of this study was to evaluate the applicability of the MESS in a contemporary civilian Central European cohort. METHODS: All patients treated for extremity injuries with arterial reconstruction at two centres between January 2005 and December 2014 were assessed. The MESS and the amputat

Pediatric Maxillofacial Fractures: A 10-Year Retrospective Analysis of 549 Cases With Emphasis on Facial Injury Severity Score Validation and COVID-19 Impact.

BACKGROUND: Pediatric maxillofacial fractures (PMFs) represent a distinct clinical entity due to their unique features. However, objective tools that inform surgical decision-making and stratify risk in PMF remain limited. In particular, the clinical utility of the Facial Injury Severity Score (FISS) in predicting management strategies and outcomes, as well as its behavior during extraordinary periods such as the COVID-19 pandemic, remains unclear. PURPOSE: The purpose of this study was to measure the association between FISS and surgical intervention in PMF, including shifts during the COVID…

Posterior Ligament-Bone Injury Classification and Severity Score: A Novel Approach to Predict the Failure of Anterior-only Surgery for Subaxial Cervical Facet Dislocations.

STUDY DESIGN: Clinical case series. OBJECTIVE: The aim of this study was to propose a novel posterior ligament-bone injury classification and severity (PLICS) score system that can be used to reflect the severity of subaxial cervical fracture dislocations (SCFDs) and predict the failure of anterior-only surgery; and to measure the intraobserver and interobserver reliability of this system. SUMMARY OF BACKGROUND DATA: The approach selection for SCFDs is controversial. Although the anterior approach is familiar for most surgeons, postoperative hardware failure and/or delayed cervical deformity

When is the circumferential stabilization necessary for subaxial cervical fracture dislocations? The posterior ligament-bone injury classification and severity score: a novel treatment algorithm.

OBJECTIVE: To propose a novel classification and scoring system called the posterior ligament-bone injury classification and severity score (PLICS) that offers a quantitative score to guide the need for posterior stabilization in addition to anterior reconstruction for subaxial cervical fracture dislocations (SCFDs). METHODS: A total of 456 patients with SCFDs were prospectively included. Patients with PLICS ≥ 7 together with extremely unstable lateral mass fracture (EULMF) were classified as high-risk group, and the other patients were classified as low-risk group. For patients in the low-ri

Nationwide Evaluation of the Validity of the Trauma and Injury Severity Score Method in Korean Regional Trauma Centers Using Multi-Institutional Large-Scale Data.

BACKGROUND: The Trauma and Injury Severity Score (TRISS) method is a widely used tool for assessing patient severity and predicting survival probability in trauma care. However, its accuracy and applicability in the Korean context, particularly for neurotrauma patients, have not been thoroughly validated yet. Previous studies at a single institution have identified significant discrepancies between TRISS predictions and actual outcomes, particularly in severe neurotrauma cases. This study aimed to evaluate the accuracy of the TRISS method on a national scale using data from multiple regional t…

Procalcitonin/Albumin Ratio Predicts the Outcome After Severe Traumatic Brain Injury: A Propensity Score-Matched Analysis.

BACKGROUND: The procalcitonin/albumin ratio (PAR), a novel inflammation-based index, has been reported to predict the prognosis following cardiopulmonary bypass surgery and bacterial infection. However, whether PAR can predict the outcome of patients with severe traumatic brain injury (STBI) has not been fully elucidated. This study aimed to investigate the relationship between serum PAR levels and prognosis at 6 months after STBI. METHODS: We retrospectively enrolled 129 patients diagnosed with STBI and collected relevant clinical and laboratory data. Logistic regression analysis was used to

[Predictive value of the HISS-scoring system for estimation of trauma severity and the time off work after hand injuries].

BACKGROUND: The estimation of the time off work depending on the injury pattern and severity is of major interest in the treatment of hand injuries. The predictive value of the HISS score (Hand Injury Severity Scoring System) was evaluated. MATERIAL AND METHODS: According to this score, 184 work-related injuries (1999 to 2002) were analyzed prospectively, excluding injuries of both hands. The median age was 37.9 years (18 to 65 years), 11 % of the patients were female. RESULTS: A significant correlation was established between the HISS score and the time off work (p < 0.0001, r = 0.51). The

Outcome after severe head injury: focal surgical lesions do not imply a better Glasgow Outcome Score than diffuse injuries at 3 months.

BACKGROUND: Historically neurosurgeons have accepted head injured patients only in the presence of a mass lesion requiring surgical decompression. Underpinning this is an assumption that these patients have a better outcome than patients without a surgical lesion. This has meant that many patients without a surgical lesion have been managed locally in the referring hospital. However, there is now evidence that treatment of all head injured patients in a specialist centre leads to improved outcomes. Therefore, we have asked the question: does the presence of a surgical lesion imply better outco

Prognostic Value of MRI Anatomical Radiologic Scores for One-Year Neurological Outcome After Severe Traumatic Brain Injury.

BACKGROUND: Severe traumatic brain injury (sTBI) is a major cause of mortality and long-term disability, and early prognostic evaluation is essential to support therapeutic decision-making in intensive care. MRI is the reference imaging modality for detecting diffuse axonal injury (DAI), yet the prognostic accuracy and reproducibility of MRI-based radiologic scoring systems remain uncertain. This study aimed to compare the prognostic performance of five published MRI-based DAI scores for predicting 1-year neurological outcome, in a cohort of sTBI patients admitted to intensive care. METHODS:
