Introduction: Choledocholithiasis (CCL) is one of the most common serious health consequences of cholelithiasis. For years, evacuation of stones using endoscopic retrograde cholangiopancreatography (ERCP) has been the first-line treatment. In 10-15% of cases, gallstones cannot be removed using the above-mentioned method and auxiliary methods are necessary; these are so-called difficult gallstones. Surgical treatment is sometimes necessary. The European Society of Gastrointestinal Endoscopy (ESGE) has recommended the use of cholangioscopy, including direct peroral cholangiosc

Introduction: High-power laser settings are commonly employed for stone dusting techniques. Previous in vitro and in vivo studies have demonstrated that a toxic thermal dose can result from treatment within a renal calix without adequate irrigation. Hence, both laser power and irrigation rate must be considered together to determine safe laser lithotripsy parameters. The objective of this in vitro study was to map parameter safety boundaries and create guidelines for selection of safe laser and irrigation settings. Methods: The experimental system consisted of in vitro models simulating ureter

A 58-year-old woman presented with a 4 cm obstructing choledocolith within the proximal common bile duct. She had an extensive operative history resulting in common bile duct to jejunum anastomosis after cholecystectomy. Endoscopic retrograde cholangiopancreatography (ERCP) removal was unsuccessful, and surgical exploration was contraindicated due to multiple comorbidities. Interventional radiologist placement of bilateral transhepatic ureteral access sheaths allowed Urologist evaluation of the biliary tract by ureteroscope. Holmium laser lithotripsy was performed with successful fragmentation

Symptomatic prostatic calculi are a rare clinical entity with wide range of management options, however, there is no agreement about the preferred method for treating these symptomatic calculi. In this study we describe our experience of transurethral management of symptomatic prostatic calculi using holmium-YAG laser lithotripsy. Patients with large, symptomatic prostatic stones managed by transurethral lithotripsy using holmium-YAG laser over 3-year duration were included in this retrospective study. Patients were evaluated for any underlying pathological condition and calculus load was dete

PURPOSE: To report our approach for treating complicated biliary calculi by percutaneous transhepatic endoscopic biliary holmium laser lithotripsy (PTBL).
PATIENTS AND METHODS: Twenty-two symptomatic patients (11 men and 11 women, age range 51 to 88 years) with intrahepatic or common bile duct calculi underwent PTBL. Nine patients had undergone previous gastrectomy and small-bowel anastomosis, thus precluding endoscopic retrograde cholangiopancreatography. In the other 13 patients, stone removal attempts by ERCP failed due to failed access or very large calculi. We used a 7.5F flexible ureter

INTRODUCTION: Although the usage rates of ureterorenoscopy (URS) for treatment of urolithiasis have increased, extracorporeal shock wave lithotripsy (ESWL) remains a widely employed primary treatment option due to its less invasive character. However, in cases of primary ESWL failure, the optimal choice of secondary treatment, whether secondary ESWL (sESWL) or secondary URS (sURS) in terms of safety and efficacy, remains uncertain.
PATIENTS AND METHODS: A total of 62 urolithiasis patients who underwent sESWL (n = 31) or sURS (n = 31) at a single tertiary referral center between March 2014 and

INTRODUCTION: Holmium:yttrium-aluminum-garnet (Ho:YAG) laser is the gold standard for ureteroscopic lithotripsy, but faces challenges such as stone retropulsion. The emerging thulium fiber laser (TFL) offers advantages, including higher efficiency, smaller fiber size, and reduced retropulsion. Our study aims to compare the stone ablation efficiency of TFL and Ho:YAG laser.
METHOD: This study is a prospective, single-center, randomized controlled trial comparing TFL and Ho:YAG laser for ureteral stone lithotripsy in 35 patients. The primary outcome is stone ablation efficiency, with secondary

PURPOSE: The holmium-YAG (Ho:YAG) Laser system is the current gold standard for laser lithotripsy (LL). Super Pulse Thulium Fiber Laser (SPTF) has emerged as an effective alternative. We compared the temperature profile of both the 120 W Ho:YAG and the 60 W SPTF systems during ureteral lithotripsy.
METHODS: Antegrade ureteroscopy with LL was performed in ex-vivo porcine kidneys with 3 mm Begostones. Intra-ureteral temperature was measured using one probe proximal and one distal to the site of lithotripsy. LL was performed using a 200 μm core fiber at dusting (SPTF-0.1 J, 200 Hz, SP; Ho:YAG-0.

INTRODUCTION: Intrahepatic lithiasis (IHL) remains a challenging condition in populations, particularly in patients with surgically altered biliary anatomy or failed endoscopic interventions. Percutaneous laser lithotripsy (PLL) under flexible cholangioscopic guidance has emerged as a minimally invasive alternative for managing complex intrahepatic stones.
PURPOSE: To evaluate the efficacy and safety of flexible PLL for the treatment of intrahepatic stones in a multicenter Brazilian cohort with altered anatomy or failed ERCP.
MATERIALS AND METHODS: The primary outcome was technical success,

The central dogma of pain in patients with chronic pancreatitis revolves around the pathophysiology of ductal hypertension owing to stones that obstruct the pancreatic duct. Conventional modalities available to decompress the pancreatic duct are occasionally limited by failed selective pancreatic duct cannulation during endoscopic retrograde cholangiopancreatography. We describe a novel endoscopic approach of EUS-guided laser lithotripsy to assist in pancreatic duct (PD) stone fragmentation in two symptomatic patients with underlying chronic pancreatitis who had failed PD cannulation and extra

INTRODUCTION: The efficiency of holmium laser lithotripsy for urolithiasis depends upon several factors, including laser pulse energy and frequency and stone composition and retropulsion. This study investigates the complex interplay between these factors and quantifies lithotripsy efficiency using different laser settings in a benchtop kidney and ureter model.
MATERIALS AND METHODS: In vitro caliceal and ex vivo porcine ureteral models were constructed. Calcium oxalate monohydrate stones were fragmented using a 200-μm laser fiber. In the caliceal model, stone fragmentation and vaporization r

BACKGROUND: Hepatolithiasis is very common in East Asia. It is benign in nature, but has a high recurrence rate. It is likely to lead to biliary cirrhosis and increase the risk of cholangiocarcinoma. Hence, the treatment of hepatolithiasis is difficult but vital. In this report, we present a novel approach to manage hepatolithiasis using the choledochoscopic Frequency-Doubled Double pulse Nd:YAG (FREDDY) laser lithotripsy combined with or without hepatectomy.
METHODS: Between July 2009 and October 2012, 45 patients underwent choledochoscopic FREDDY laser lithotripsy combined with or without h
