Latest Concepts in Endodontic and Periodontal Management of Diabetic Patients.

The close interrelationship between chronic oral and dental inflammatory diseases, such as periodontal and apical abscesses (AA), with the patient's systemic condition, is one of the most interesting topics of study common between dentists and the medical staff. Chronic apical periodontitis and periodontal diseases are chronic infections of the oral cavity in which similar Gram-negative anaerobic microbiota are found and in both infectious diseases increased local inflammatory mediators, levels may influence overall systemic conditions. One of the most important systemic diseases linked to per

Fusobacterium nucleatum infection leading to rare hepatorenal abscess: a case report.

Fusobacterium nucleatum is a gram-negative anaerobic bacterium commonly associated with periodontal disease. However, its role in extraoral infections, particularly in immunocompetent individuals, is increasingly recognized. We report a rare case of hepatorenal abscess caused by F. nucleatum in a previously healthy woman, initially suspected to have a malignant tumor based on PET-CT findings. Next-generation sequencing (NGS) of abscess aspirate confirmed the pathogen. The patient responded well to targeted antibiotic therapy. This case highlights the importance of considering anaerobic pathoge

Parvimonas micra Bacteremia in the Setting of a Hepatic Abscess: A Case Report.

Parvimonas micra is a Gram-positive anaerobic coccus that typically colonizes the oral cavity and gastrointestinal tract in humans. Though P. micra is typically associated with periodontal abscesses, it can also be an unlikely cause of bacteremia. Here, we report a case of P. micra bacteremia in the setting of a hepatic abscess. Antibiotic treatment of the bacteremia was initiated, and the entry source of the infection was investigated using various imaging techniques in the inpatient setting. A hepatic abscess was suspected to be the origin of infection for the P. micra bacteremia. Successful…

Acute Prevertebral Abscesses Caused by Bacterial-infected Traumatic Tooth Fractures.

We report a case of acute prevertebral abscess caused by traumatic tooth fractures in a 77-year-old Japanese man. After being transferred to our hospital the patient was initially diagnosed with a neck hematoma; however, blood culture showed Streptococcus parasanguinis, an oral bacterium, and an MRI examination suggested prevertebral abscesses. Tooth fractures, severe periodontitis, and peri-implantitis with Streptococcus parasanguinis were observed. Antibiotics were administered and fractured teeth were extracted. The patient's condition then gradually improved. We concluded that bacteremia c

Comprehensive therapy of a fusion between a mandibular lateral incisor and supernumerary tooth: case report.

The term fusion is used to define a developmental anomaly characterised by the union of two adjacent teeth. In the case reported here, clinical and radiographic examinations suggested a unilateral fusion between the mandibular left permanent incisor and a super-numerary tooth. Radiographs showed that the fused teeth had two distinct pulp chambers and canals. A diagnosis of chronic periapical abscess of the supernumerary tooth was made. Before root canal therapy, a periodontal surgical procedure was performed to section the central incisor and its fused supernumerary. Also, odontoplasty was per

The Periodontium as a Potential Cause of Orofacial Pain: A Comprehensive Review.

INTRODUCTION: Orofacial pain of periodontal origin has a wide range of causes, and its high prevalence and negative effect on patients' quality of life make intervention mandatory. This review provides a periodontological overview of the field of orofacial pain, focusing on the entities which involve the periodontal tissues and may be the cause of this pain or discomfort. METHODS: The study comprised a literature search of these pathologies conducted in the MEDLINE/PubMed Database. Acute infectious entities such as gingival and periodontal abscesses are emergencies that require a rapid respon…

Atypical Presentation of an Odontogenic Brain Abscess Due to Eggerthella lenta and Bilophila wadsworthia Coinfection.

Odontogenic brain abscesses are uncommon. Eggerthella lenta and Bilophila wadsworthia are anaerobes that are normal components of the human intestinal microbiota. Both can also be found in the oral cavity. We report the case of an 81-year-old man with a brain abscess caused by coinfection of E. lenta and B. wadsworthia associated with periodontitis. The patient underwent a right frontal craniotomy with excision of the abscess. Ceftriaxone and metronidazole were then started, along with levetiracetam. Ceftriaxone and metronidazole were maintained for four weeks, and the antibiotic treatment was

Severe Fusobacteria infections (Lemierre syndrome) in two boys.

UNLABELLED: Abscess formation is a rare cause of febrile illness in childhood but always has to be considered in such clinical presentations. Belonging to the resident flora of the oropharyngeal region, Fusobacteria are known to cause local infections; from here they may extend to other sites via the bloodstream or are aspirated into the lung (Lemierre disease). We report on two boys with Lemierre disease due to infection by Fusobacteria in monoculture causing two different clinical phenotypes. Case 1 presented with a large subphrenic abscess and pneumonic infiltration of the right middle lobe

Streptococcus mitis Peritonitis in a Peritoneal Dialysis Patient: A Case Report Highlighting the Importance of Dental Hygiene.

Viridans-group streptococci, including the Streptococcus mitis/oralis subgroup, can cause peritoneal dialysis (PD)-related peritonitis. The link between dental pathology and PD-related peritonitis remains to be fully elucidated. We report a case of an 83-year-old man undergoing nocturnal intermittent PD due to kidney failure from diabetic nephropathy who developed S. mitis peritonitis and septicemia traced back to a periodontal abscess. Despite having no prior history of peritonitis and maintaining good nutritional status, the patient presented with generalized abdominal pain and a low-grade f…

Femoral osteomyelitis caused by oral anaerobic bacteria with mixed bacteremia of Campylobacter rectus and Parvimonas micra in a chronic periodontitis patient: a case report.

BACKGROUND: Campylobacter rectus is a gram-negative rod, and Parvimonas micra is a gram-positive coccus, both of which are oral anaerobes that cause chronic periodontitis. Chronic periodontitis can cause bacteremia and systemic diseases, including osteomyelitis. Hematogenous osteomyelitis caused by anaerobic bacteria is uncommon, and to date, there have been no reports of mixed bacteremia with C. rectus and P. micra. Here, we report the first case of osteomyelitis of the femur caused by anaerobic bacteria with mixed bacteremia of C. rectus and P. micra caused by chronic periodontitis. CASE PR

The protective effect of recombinant FomA-expressing Lactobacillus acidophilus against periodontal infection.

A number of studies have shown that the outer membrane protein FomA found in Fusobacterium nucleatum demonstrates great potential as an immune target for combating periodontitis. Lactobacillus acidophilus is a useful antigen delivery vehicle for mucosal immunisation, and previous studies by our group have shown that L. acidophilus acts as a protective factor in periodontal health. In this study, making use of the immunogenicity of FomA and the probiotic properties of L. acidophilus, we constructed a recombinant form of L. acidophilus expressing the FomA protein and detected the FomA-specific I

Nonsteroidal anti-inflammatory drugs misleading the diagnosis of periapical abscess.

Periapical abscesses are common but may rarely be a subclinical infection in dental clinics. If an infection progresses rapidly or slowly as a tooth ache of chronic periodontitis with clinical features misleading the diagnosis, the dentists are able to recognize the salient signs and identify the patients at risk. This article reviews a case of a rare innocuous periapical abscess presenting as angioedema of upper lip with history of consuming nonsteroidal anti-inflammatory drugs such as diclofenac sodium whenever needed for arthralgia on a short-term basis.

Septic pulmonary embolism associated with periodontal disease: a case report and literature review.

BACKGROUND: Periodontal disease, including periodontitis, has been reported to be a rare cause of septic pulmonary embolism (SPE). It is however extremely difficult to isolate the causative pathogen of periodontal disease-associated SPE from blood cultures of these patients. CASE PRESENTATION: In this study, an 85-year-old Japanese man was admitted with fever and worsening malaise. He was later noted to have multiple bilateral subpleural pulmonary nodules on chest computed tomography scan. After admission, Parvimonas micra (P. micra) was isolated from his blood culture. This was followed by a
