Congenital absence of the pulmonary valve.

The clinical, radiological, electrocardiographic, and haemodynamic findings in 7 patients with the syndrome of congenital absence of the pulmonary valve with ventricular septal defect are reported. Findings included a pansystolic murmur and a low-pitched delayed diastolic murmur at the left sternal border with no pulmonary valve closure sound, an rsR' pattern in V1 on the electrocardiogram, and a chest x-ray showing a large heart and gross dilatation of the main, left or right pulmonary arteries but normal or reduced vascularity of the peripheral lung fields. Cardiac catheterization showed a l…

[Percutaneous treatment of the pulmonary valve].

Right ventricular outflow tract diseases are historically outcomes of surgical reconstruction for heart defects in neonates or children (Tetralogy of Fallot, Ross surgery). This kind of surgery performed during childhood lead often to dysfunctional right ventricular outflow tract (stenosis, leak) in older infants or young adults. In this case, reintervention on the right ventricular outflow tract would be associated with a high surgical risk. Development of the first percutaneous valves in the year 2000 paved the way for the pulmonary revalvulation. This percutaneous procedure has emerged as a

Imaging of the pulmonary valve in the adults.

PURPOSE OF REVIEW: Pulmonary valve is the least imaged of the cardiac valves in adults. This review will address the strengths and the limitations of various imaging modalities that are commonly used for evaluation of pulmonary valve diseases in the adults. RECENT FINDINGS: Valvular pulmonary stenosis is mostly congenital and pulmonary regurgitation is usually an acquired pulmonary valve disease. Combined pulmonary stenosis and pulmonary regurgitation as sequel to previous surgeries for congenital heart diseases is the most common form of pulmonary valve disease in the adults. Transthoracic e…

Case report: Aortic valve endocarditis and recurrent pulmonary valve stenosis.

BACKGROUND: We discuss a rare case of an adult patient with different pathologies involving the aortic and pulmonary valves in need of surgery. CASE PRESENTATION: The patient had a history of congenital PV stenosis and surgical valvuloplasty. Almost 50 years later the patient underwent a complex second heart surgery due to infective endocarditis of the aortic valve and high-grade restenosis of the pulmonary valve. Replacement of the aortic and pulmonary valve, as well as reconstruction of the RVOT and closure of a persistent foramen ovale, followed. Postoperative course was uneventful and the…

Transcatheter Pulmonary Valve Replacement: A Review of Current Valve Technologies.

Transcatheter pulmonary valve replacement was first performed by Dr Philip Bonhoeffer, who implanted a Medtronic Melody valve in a human in 2000. Over the past 2 decades, there have been many advances in transcatheter pulmonary valve technology. This includes the use of the SAPIEN transcatheter heart valve in the pulmonary position, modifications and refinements to valve implantation procedures, and development of self-expanding valves and prestents to treat large diameter native or patched right ventricular outflow tracts. This article reviews the current transcatheter pulmonary valve technol…

Absent pulmonary valve, imperforate tricuspid valve, and dysplastic right ventricle.

We report a very rare case involving an absent pulmonary valve, an imperforate tricuspid valve, a ventricular septal defect (VSD), and a dysplastic right ventricle in a patient who underwent a Fontan operation successfully. Upon performing a bidirectional cavopulmonary anastomosis, we closed the VSD and coronary artery-to-right ventricle fistulae, and plicated the right ventricular free wall in order to obliterate the right ventricular cavity.

The Ross procedure: aortic valve replacement using autologous pulmonary valve.

The use of pulmonary autologous valve in aortic valve replacement pioneered by Dr. Donald Ross in 1967 has been employed in young patients for more than two decades with impressive results. This procedure is recommended for young patients with isolated aortic disease who are expected to have a longer life span. Because of its lack of tissue degenerative changes, the autologous pulmonary valve has the potential to function longer than other biologic valves and mechanical valves while exhibiting the same inherent hemodynamic advantages. The assessment of the long-term performance of the pulmonar…

Management of prosthetic pulmonary valve endocarditis should include the selective application of transcatheter pulmonary valve replacement.

OBJECTIVE: Prosthetic pulmonary valve endocarditis (PPVE) limits survival and quality of life in patients with repaired congenital heart disease. We describe patient outcomes with PPVE in a single center and the application of transcatheter pulmonary valve replacement (TPVR). METHODS: We performed a single-center retrospective review of patients with PPVE from 2010 to 2022. Subjects were stratified by initial treatment: group 1 comprised surgical pulmonary valve replacement (SPVR), group 2 comprised TPVR, and group 3 comprised initial medical therapy without plans for upfront surgical or proc…

Hybrid pulmonary valve implantation: injection of a self-expanding tissue valve through the main pulmonary artery.

An 8-year-old (35 kg) boy presented with progressive right ventricular outflow tract enlargement (28 mm) and progressive tricuspid regurgitation after transannular repair of tetralogy of Fallot and was scheduled for pulmonary valve replacement. To spare reoperation on full sternotomy, a transverse mini-thoracotomy through the third intercostal space was used to implant an injectable 29-mm stented porcine valve directly through an incision of the pulmonary artery bifurcation. The procedure was performed while rapid ventricular pacing and right ventricular unload by a short running femorally imp…

Intentional Fracture of Bioprosthetic Valve Frames in Patients Undergoing Valve-in-Valve Transcatheter Pulmonary Valve Replacement.

BACKGROUND: Percutaneous transcatheter pulmonary valve replacement (TPVR) has good clinical and hemodynamic outcomes in treating dysfunctional bioprosthetic valves (BPV) in the pulmonary position. Valve-in-valve therapy can further decrease the inner diameter (ID), potentially resulting in patient-prosthesis mismatch in patients with smaller BPVs. METHODS AND RESULTS: To evaluate feasibility and outcomes of intentional BPV fracture to enlarge the pulmonary valve orifice with TPVR, 37 patients from 13 centers who underwent TPVR with intended BPV fracture were evaluated. A control cohort (n=70)…

Transcatheter Pulmonary Valve Implantation Using Self-Expandable Percutaneous Pulmonary Valve System: 3-Year CE Study Results.

BACKGROUND: Pulmonary regurgitation is common during follow-up of patients after surgical repair of tetralogy of Fallot and other right ventricular outflow tracts (RVOTs). Many percutaneous pulmonary valves have been used but are limited to smaller RVOTs. OBJECTIVES: Since August 2016, a multicenter CE (Conformité Européenne) study was initiated to evaluate a self-expandable VenusP-valve. We aimed to report the acute and 3-year follow-up results. METHODS: A total of 81 patients with pulmonary regurgitation were recruited for VenusP-valve implantation and assessed for a 3-year period. RESULT…

Repair of congenital pulmonary incompetence by bicuspidization of the pulmonary valve.

We report the successful repair of pulmonary incompetence in an adult due to the congenital absence of the posterior leaflet of the pulmonary valve. The repair consisted of bicuspidization of the pulmonary valve, which achieved competence and eliminated the symptoms and echocardiographic manifestations of right ventricular overload while preserving the native valve.
