Effect of spinal needle characteristics on measurement of spinal canal opening pressure.

OBJECTIVE: A wide variety of spinal needles are used in clinical practice. Little is currently known regarding the impact of needle length, gauge, and tip type on the needle's ability to measure spinal canal opening pressure. This study aimed to investigate the relationship between these factors and the opening-pressure measurement or time to obtain an opening pressure. METHODS: Thirteen distinct spinal needles, chosen to isolate the effects of length, gauge, and needle-point type, were prospectively tested on a lumbar puncture simulator. The key outcomes were the opening-pressure measurement…

[Relationship between developmental stenosis of cervical spinal canal and spinal cord injury].

A retrospective study of 55 patients with acute cervical spinal cord injury caused by trauma was performed. On radiographic examination of the cervical spine in each of 55 cases showed no fracture or dislocation but developmental stenosis determined on lateral view radiograms. Ninety five point nine percent of this cases had block of the dyeing columns myelographically both in prone and supine positions. Eighty point nine percent of 47 cases demonstrated segmental instability of the cervical spine on full extension and flexion radiograms. Thirty eight cases suffered a minor trauma but resulted

Conductive neuromagnetic fields in the lumbar spinal canal.

OBJECTIVE: To measure neuromagnetic evoked fields in the lumbar spinal canal. METHODS: Using a newly developed superconducting quantum interference device (SQUID) fluxmeter, neuromagnetic fields of 5 healthy male volunteers were measured at the surface of the lower back after stimulation of the tibial nerves at the ankles. For validation, we inserted a catheter-type electrode percutaneously in the lumbar epidural space in 2 of the subjects and measured cauda equina action potentials after tibial nerve stimulation. RESULTS: Neuromagnetic fields propagating from the intervertebral foramina int…

Cervical spinal canal narrowing and cervical neurological injuries.

Cervical spinal canal narrowing can lead to injury of the spinal cord and neurological symptoms including neck pain, headache, weakness and parasthesisas. According to previous and recent clinical researches, we investigated the geometric parameters of normal cervical spinal canal including the sagittal and transverse diameters as well as Torg ratio. The mean sagittal diameter of cervical spinal canal at C(1) to C(7) ranges from 15.33 mm to 20.46 mm, the mean transverse diameter at the same levels ranges from 24.45 mm to 27.00 mm and the mean value of Torg ratio is 0.96. With respect to narrow…

Operative treatment for degenerative lumbar spinal canal stenosis.

The authors have made a retrospective study of a cohort of patients who underwent surgery for spinal stenosis. A total of 85 patients were surgically treated for spinal stenosis between 1993 and 1997, and 79 patients were available for re-evaluation. The average time of follow-up was 79 months. Twenty patients with monosegmental stenosis underwent fenestration and undercutting, 16 patients had a hemilaminectomy or laminectomy and 43 patients had an instrumented fusion after decompression. The severity of the clinical complaints, the degree of stenosis and the extent of the instability determin

[Diagnosis and treatment of lumbar spinal canal stenosis].

Lumbar spinal canal stenosis (LSCS) was first described in 1954 by Verbiest, followed by the currently accepted international classification of LSCS in 1976 by Arnoldi. Briefly, LSCS is a nervous system syndrome that is characterized by neural symptoms in the lower extremities due to tightened cauda equina and spinal nerve root involvement. LSCS international classification consists of: (1) degenerative, (2) congenital developmental, (3) combined, (4) spondylolytic spondylolisthesis, (5) iatrogenic and (6) post traumatic stenosis. Degenerative stenosis-the most common type of LSCS-is caused by…

Acquired epidermoid tumour in the thoracic spinal canal.

Acquired spinal epidermoid tumours are rare, and probably caused by skin fragments that have been transplanted by trauma or lumbar puncture. They almost always occur in the region of the cauda equina. We present an acquired spinal epidermoid tumour at the T 10 level, with pathological confirmation. A skin fragment had migrated to a site where it developed into a tumour.

Imaging anatomy of the lateral lumbar spinal canal.

This article reviews the normal anatomy of the bony and soft tissue structures of the lateral lumbar spinal canal. Thorough knowledge of these imaging anatomy helps in making the correct diagnosis of the lumbar spinal lesions. CT and MRI are excellent tools to diagnose lateral lumbar spinal canal stenosis. Both CT and MRI provide the accurate measurements of the midsagittal and lateral spinal canals and show the abnormalities of the nerve root course. This review provides anatomic and radiographic descriptions of the lateral lumbar spinal canal and the anatomically related joints.

Hemophilic pseudotumor of the spinal canal. Case report.

Hemophilic pseudotumor is an uncommon complication among hemophiliacs. Most of these lesions are located in the long bones and the pelvis. The authors describe a case of hemophilic pseudotumor in a patient who presented with symptoms of L-5 radiculopathy and evidence of a destructive lesion on computerized tomography scans. Histologically, the lesion consisted of an organizing hematoma with reactive fibrosis. The diagnosis requires a high index of suspicion. Surgery is recommended for symptomatic patients.

[The narrow spinal canal - medicolegal aspects (author's transl)].

The constitutional anomayl of a narrow spinal canal was found in a neuroradiological department in 31 cases. The cervical stenosis can be defined in the following way: Inside diameter of cervical canal (anterior-posterior) in relation to diameter of vertebral body. In normal cases the quotient is over 1 - in pathological narrowing under 1. Clinical symptoms mainly appear from 45th year onwards, when reactive-degenerative changes increase the space problem. In wiplash injuries or other adequate cervical trauma 7 cases were seen and described with acute incomplete tetraplegia or/and multilocular…

Extradural nodular fasciitis arising in the spinal canal.

STUDY DESIGN: Case report. OBJECTIVE: To describe a patient with nodular fasciitis arising in the lumbar extradural space. SUMMARY OF BACKGROUND DATA: Nodular fasciitis is a benign proliferation of fibroblasts and myofibroblasts. It commonly occurs in the subcutaneous tissue of an upper extremity, trunk, head, and neck, but rarely arises in the spinal canal. METHODS: A 7-year-old boy experienced gradually increasing intense radiating pain from the bilateral buttocks to the lower extremities after a bruise on his lower back. Computed tomography and magnetic resonance imaging demonstrated a r…

Vertebroplasty for osteoporotic fractures with spinal canal compromise.

BACKGROUND AND PURPOSE: Percutaneous vertebroplasty can aggravate spinal canal narrowing, especially in patients with preoperative retropulsion. The purpose of this study was to evaluate changes in spinal canal dimension during percutaneous vertebroplasty for osteoporotic fractures with retropulsion. MATERIALS AND METHODS: We reviewed all cases of osteoporotic vertebral fractures treated with vertebroplasty. Twenty-one patients (25 vertebrae) had a retropulsed fragment that compromised the dimension of the spinal canal on preoperative imaging. We measured the degree of retropulsion before and…

Narrowing of thoraco-lumbar spinal canal in achondroplasia.

The incidence of neurological deficits in achondroplastic subject is by no means negligible. We considered the clinico-radiological and therapeutic data of 35 achondroplasic patients (5 personal cases and 30 published in the available literature) harbouring myelo-radicular deficits related to narrowing of thoraco-lumbar canal. There was no significant sex prevalence. The clinical symptoms appear most frequently in the III or IV decades of life. Actually morphological anomalies of the spinal canal are already present at birth in achondroplasic dwarfs, and signs of cervical cord involvement are …

Cervical sagittal spinal canal size in spine injury.

This study investigated the relationship between cervical spine sagittal canal diameter and neurologic injury in cases of spinal fracture-dislocation. A group of 98 patients with such injuries was reviewed; 45 had no neurologic deficits, 39 had incomplete quadriplegia, and 14 had complete quadriplegia. Spinal canal sagittal diameter was measured in all, and large diameter and small canals were defined. Small diameter canals were correlated significantly with neurologic injury, while large diameter canals allowed protection from neurologic injury in cervical fracture dislocation.
