Poor agreement between pulmonary capillary wedge pressure and left ventricular end-diastolic pressure in a veteran population.

BACKGROUND: Accurate determination of left ventricular filling pressure is essential for differentiation of pre-capillary pulmonary hypertension (PH) from pulmonary venous hypertension (PVH). Previous data suggest only a poor correlation between left ventricular end-diastolic pressure (LVEDP) and its commonly used surrogate, the pulmonary capillary wedge pressure (PCWP). However, no data exist on the diagnostic accuracy of PCWP in veterans. Furthermore, the effects of age and comorbidities on the PCWP-LVEDP relationship remain unknown. METHODS: We investigated the PCWP-LVEDP relationship in 1

Relationship between pulse pressure variation and echocardiographic indices of left ventricular filling pressure in critically ill patients.

BACKGROUND: Pulse pressure variation (PPV) is a dynamic index of fluid responsiveness. This parameter helps clinicians in improving haemodynamic status while avoiding potential fluid overload. Echocardiographic indices, such as E/E' ratio and left atrial (LA) strain by speckle tracking echocardiography (STE), are used to estimate left ventricular (LV) filling pressures. This study aimed at exploring the relationship between PPV and echocardiographic indices of LV filling pressures in critically ill patients. METHODS: Twenty-two patients (mean age of 50.9 ± 21.6, male/female = 15/7) admitted t

Left ventricular dynamic geometry and diastolic mechanics in a model of chronic cyanosis and right ventricular pressure overload.

Left ventricular (LV) mechanics and dynamic geometry were studied in eight dogs 1 to 151 days after creation of a model of cyanosis and increased right ventricular (RV) pressure. In each dog, an 8 mm conduit was interposed between the main pulmonary artery and left atrium; the pulmonary artery was then banded distal to this shunt. Ultrasonic dimension transducers were subsequently implanted for the measurement of LV equatorial minor axis and wall thickness. Catheter-tipped micromanometers were used to measure LV and pleural pressures; RV pressure was measured with a fluid-filled catheter and a

Imaging-based method to quantify left ventricular diastolic pressures.

AIMS: To establish an imaging-based method to quantify left ventricular (LV) diastolic pressures. METHODS AND RESULTS: In 115 patients suspected of coronary artery disease, LV pressure was measured by micromanometers and images by echocardiography. LV filling pressure was measured as LV pre-atrial contraction pressure (pre-A PLV). Based on previous observations, we hypothesized that pre-A PLV approximates the sum of minimum PLV and maximum transmitral pressure difference. Parameters used for pressure estimates included LV volumes and strain, left atrial strain, mitral flow velocities, systoli

Association of blood pressure control with atrial and ventricular ectopy in SPRINT.

BACKGROUND: Previous epidemiological studies demonstrated that premature atrial contractions (PACs) and premature ventricular contractions (PVCs) detected by single 12-lead ECGs can predict incident cardiovascular disease and death. The determinants of cardiac ectopy remain unknown, with some evidence that hypertension may contribute. OBJECTIVE: To determine if intensive blood pressure (BP) control reduces the incidence of cardiac ectopy. METHODS: We performed a post-hoc analysis of the Systolic Blood Pressure Intervention trial, which randomised hypertensive participants to standard treatme

Quantitative assessment of parenchymal and ventricular readjustment to intracranial pressure relief.

A 26-year-old patient underwent endoscopic third ventriculostomy for the treatment of obstructive hydrocephalus. 3D volume data sets were obtained at 3 T before surgery and three times after surgery. Off-line analysis of individual imaging data (initial linear registration, intensity adjustment, and final nonlinear registration of pre- to postoperative MR images) yielded 3D displacement fields representing the postoperative structural brain change. In principle, such an analysis technique can be used in any clinical follow-up for which careful observation of tissue readjustment is of particula

Evaluation of a noninvasive system for determining left ventricular filling pressure.

BACKGROUND: Measurement of left ventricular filling pressure (LVFP) provides an accurate assessment of left ventricular failure. Clinical and radiographic methods of estimating LVFP are unreliable. The noninvasive method of analyzing the decline in the arterial pressure during the strain phase of the Valsalva maneuver may be used to directly measure LVFP. OBJECTIVE: To examine the relationship and the level of accuracy of a noninvasive system (VeriCor) in directly determining left ventricular end diastolic pressure (LVEDP) using simultaneously recorded VeriCor and LVEDP measurements obtained

Periodic oscillation of intracranial pressure in ventricular dilation: a preliminary report.

Artificial pressure waves (PWs) were generated by manual inflation of a balloon in the trigonum of the lateral ventricle in seven adult mongrel dogs with normal cerebrospinal fluid (CSF) circulation. In 14 of 16 series of continuous appearances of artificial PWs, local shifts of the brain were successfully monitored using small strain-gauge sensors at the periventricular structures in these animals. Of the 14 series, 13 showed displacements of the periventricular structures, suggesting ventricular dilation. These results did not always correlate with macroscopic findings. They are thought to b

[Assessment of the functional parameter of left ventricular end-diastolic pressure].

In patients with an increase in enddiastolic pressure an impairment of left ventricular function is assumed. However, changes in left ventricular geometry (e.g. hypertrophy) or in elastic properties of the myocardium result in an increase in enddiastolic pressure, too. To differentiate the underlying mechanisms the elastic properties of the ventricle and myocardium, respectively, have to be determined. Our investigations show that the usually used determinations are not appropriate, if ventricular volumes are determined by means of angiography. The elastic properties can be calculated more exa

Non-invasive pressure-volume loop analysis in left ventricular load manipulation.

BACKGROUND: Clinical monitoring of patients with heart failure or cardiomyopathy is facilitated by detailed assessment of cardiac loading conditions. Specifically, alterations in preload and afterload may unmask pathology through effects on ventricular pressure-volume (PV) relations. Therefore, the aim was to assess non-invasive PV loops during left ventricular load manipulation in healthy participants and in patients with hypertrophic cardiomyopathy (HCM). METHODS: In total, n = 46 participants were studied in paired experiments at baseline and during load manipulation. Controls (n = 24) and…

Method for generating right ventricular pressure-volume loops in routine practice.

BACKGROUND: Analysis of pressure-volume (PV) loops from conductance catheterization is the gold standard for evaluating right ventricular (RV) function, but the complexity of conductance catheterization limits clinical implementation. This study validates a novel method for reconstructing RV PV loops from pressure waveforms acquired during routine right heart catheterization (RHC). METHODS: An algorithm was developed to estimate RV volume from pressure using the hydromotive source pressure model with external calibration. The method was validated against conductance catheterization in swine (

Right ventricular diastolic adaptation to pressure overload in different rat strains.

Different rat strains are used in various animal models of pulmonary hypertension and right ventricular (RV) failure. No systematic assessment has been made to test differences in RV response to pressure overload between rat strains. We compared RV adaptation to pulmonary trunk banding (PTB) in Wistar (W), Sprague Dawley (SD), and Fischer344 (F) rats by hemodynamic profiling focusing on diastolic function. Age-matched male rat weanlings were randomized to sham surgery (W-sham, n = 5; SD-sham, n = 4; F-sham, n = 4) or PTB (W-PTB, n = 8; SD-PTB, n = 8; F-PTB, n = 8). RV function was evaluated af

Non-invasive assessment of left ventricular filling pressure in aortic stenosis.

BACKGROUND: The assessment of left ventricular (LV) filling pressure (FP) is important for the management of aortic stenosis (AS) patients. Although, it is often restricted for predict LV FP in AS because of mitral annular calcification and a certain left ventricular hypertrophy. Thus, we tested the predictive ability of the algorithm for elevated LV FP in AS patients and also applied a recently-proposed echocardiographic scoring system of LV FP, visually assessed time difference between the mitral valve and tricuspid valve opening (VMT) score. METHODS: We enrolled consecutive 116 patients wi

Cardiac magnetic resonance identifies raised left ventricular filling pressure: prognostic implications.

AIMS: Non-invasive imaging is routinely used to estimate left ventricular (LV) filling pressure (LVFP) in heart failure (HF). Cardiovascular magnetic resonance (CMR) is emerging as an important imaging tool for sub-phenotyping HF. However, currently, LVFP cannot be estimated from CMR. This study sought to investigate (i) if CMR can estimate LVFP in patients with suspected HF and (ii) if CMR-modelled LVFP has prognostic power. METHODS AND RESULTS: Suspected HF patients underwent right heart catheterization (RHC), CMR and transthoracic echocardiography (TTE) (validation cohort only) within 24 h

Intensive Blood Pressure Lowering in Patients With Malignant Left Ventricular Hypertrophy.

BACKGROUND: Left ventricular hypertrophy (LVH) combined with elevations in cardiac biomarkers reflecting myocardial injury and neurohormonal stress (malignant LVH) is associated with a high risk for heart failure and death. OBJECTIVES: The aim of this study was to determine the impact of intensive systolic blood pressure (SBP) control on the prevention of malignant LVH and its consequences. METHODS: A total of 8,820 participants in SPRINT (Systolic Blood Pressure Intervention Trial) were classified into groups based on the presence or absence of LVH assessed by 12-lead ECG, and elevations in

Left ventricular retraining in corrected transposition: Relationship between pressure and mass.

BACKGROUND: A subset of patients with corrected transposition of the great arteries (CC-TGA) will require left ventricular (LV) retraining before undergoing a double-switch procedure. LV retraining results in an immediate increase in LV pressure but not in LV mass. The purpose of this study was to evaluate the relationship between LV pressure and mass during LV retraining. METHODS: This was a retrospective review of 36 patients with CC-TGA who were enrolled in LV retraining. The median age at enrollment was 12 months. The majority (82%) had an Ebstenoid tricuspid valve or moderate to severe t

Effects of multikinase inhibitors on pressure overload-induced right ventricular remodeling.

BACKGROUND: Little is known about the effects of current PAH therapies and receptor tyrosine kinase inhibitors on heart remodeling. We sought to investigate the effects of the multikinase inhibitors sunitinib (PDGFR-, VEGFR- and KIT-inhibitor) and sorafenib (raf1/b-, VEGFR-, PDGFR-inhibitor) on pressure overload induced right ventricular (RV) remodeling. METHODS: We investigated the effects of the kinase inhibitors on hemodynamics and remodeling in rats subjected either to monocrotaline (MCT)-induced PH or to surgical pulmonary artery banding (PAB). MCT rats were treated from days 21 to 35 wi
